    Revelation and exegesis in the theological writings of Emanuel Swedenborg

    This essay contains nine chapters of which the first is mainly biographical and includes a short account of all the theological, writings laid under contribution in the later chapters. The second chapter deals with the important question of Swedenborg’s experiences of the spiritual world. These chapters are designed to lay a minimum of factual groundwork upon which appreciation of Swedenborg’s distinctive contribution to all matters of theology must be based. Some critical review of the spiritual experiences accompanies their description. Thereafter the work proceeds directly to exposition of the fundamental teachings which impinge upon Swedenborg’s doctrine of the Word. Of necessity his doctrine of God, that is, the doctrine of the sole Deity of the Lord Jesus Christ, the Tri-une God, is examined. It is further shown that the order of creation determines the order of revelation and that "degrees" and "correspondences" afford the means by which the Divine descends into the created universe. The doctrine of the Word is then examined in the light of these teachings and his statements about the internal sense of the Sacred Scriptures are found to be harmonious with them. The question of the canon of the Word is raised and considered, the last two chapters are concerned with Swedenborg’s exegesis. First it is shown that exegesis of the letter is supremely important, distinguishing however between historical and literary criticism and exegesis of the sense of the letter. In the concluding chapter four passages, two from each of Swedenborg’s major published exegetical works, are presented, showing how the internal sense of the chosen texts has been reached. Next a study is made of his treatment of the symbol of the serpent in passages from various parts of the Scriptures and finally certain experiments are made under the guidance of his principles of interpretation

    Evaluation of delirium screening tools in geriatric medical inpatients: a diagnostic test accuracy study

    Introduction: screening all unscheduled older adults for delirium is recommended in national guidelines, but there is no consensus on how to perform initial assessment. Aim: to evaluate the test accuracy of five brief cognitive assessment tools for delirium diagnosis in routine clinical practice. Methods: a consecutive cohort of non-elective, elderly care (older than 65 years) hospital inpatients admitted to a geriatric medical assessment unit of an urban teaching hospital. Reference assessments were clinical diagnosis of delirium performed by elderly care physicians. Routine screening tests were: Abbreviated Mental Test (AMT-10, AMT-4), 4 A's Test (4AT), brief Confusion Assessment Method (bCAM), months of the year backwards (MOTYB) and informant Single Question in Delirium (SQiD). Results: we assessed 500 patients, mean age 83 years (range = 66−101). Clinical diagnoses were: 93 of 500 (18.6%) definite delirium, 104 of 500 (20.8%) possible delirium and 277 of 500 (55.4%) no delirium; 266 of 500 (53.2%) were identified as definite or possible dementia. For diagnosis of definite delirium, AMT-4 (cut-point < 3/4) had a sensitivity of 92.7% (95% confidence interval (CI): 84.8–97.3), with a specificity of 53.7% (95% CI: 48.1–59.2); AMT-10 (<4/10), MOTYB (<4/12) and SQiD showed similar performance. bCAM had a sensitivity of 70.3% (95% CI: 58.5–80.3) with a specificity of 91.4% (95% CI: 87.7–94.3). 4AT (>4/12) had a sensitivity of 86.7% (95% CI: 77.5–93.2) and specificity of 69.5% (95% CI: 64.4–74.3). Conclusions: short screening tools such as AMT-4 or MOTYB have good sensitivity for definite delirium, but poor specificity; these tools may be reasonable as a first stage in assessment for delirium. The 4AT is feasible and appears to perform well with good sensitivity and reasonable specificity

    Nucleic acid extraction (NAE) plays a crucial role for diagnostic testing procedures. For decades, dried blood spots (DBS) have been used for serology, drug monitoring, and molecular studies. However, extracting nucleic acids from DBS remains a significant challenge, especially when attempting to implement these applications to the point-of-care (POC). To address this issue, we have developed a paper-based NAE method using cellulose filter papers (DBSFP) that operates without the need for electricity (at room temperature). Our method allows for NAE in less than 7 min, and it involves grade 3 filter paper pre-treated with 8% (v/v) igepal surfactant, 1 min washing step with 1× PBS, and 5 min incubation at room temperature in 1× TE buffer. The performance of the methodology was assessed with loop-mediated isothermal amplification (LAMP), targeting the human reference gene beta-actin and the kelch 13 gene from P. falciparum. The developed method was evaluated against FTA cards and magnetic bead-based purification, using time-to-positive (min) for comparative analysis. Furthermore, we optimised our approach to take advantage of the dual functionality of the paper-based extraction, allowing for elution (eluted disk) as well as direct placement of the disk in the LAMP reaction (in situ disk). This flexibility extends to eukaryotic cells, bacterial cells, and viral particles. We successfully validated the method for RNA/DNA detection and demonstrated its compatibility with whole blood stored in anticoagulants. Additionally, we studied the compatibility of DBSFP with colorimetric and lateral flow detection, showcasing its potential for POC applications. Across various tested matrices, targets, and experimental conditions, our results were comparable to those obtained using gold standard methods, highlighting the versatility of our methodology. In summary, this manuscript presents a cost-effective solution for NAE from DBS, enabling molecular testing in virtually any POC setting. When combined with LAMP, our approach provides sample-to-result detection in under 35 minutes

    Frontoparietal function in young people with dysthymic disorder (DSM-5: Persistent depressive disorder) during spatial working memory

    Background Dysthymic disorder (DD) is a depressive disorder characterised by persistent low and/or irritable mood and has been identified as a major risk factor for developing major depressive disorder (MDD). MDD and DD have been associated with executive function difficulties of working memory and attention. Little is known about how executive function networks in the brain are affected in children and adolescents with MDD and even less in DD. This study used fMRI and two spatial working memory paradigms to investigate associated brain function in young people with DD and an age-, gender- and IQ- matched typically developing group. Methods Nineteen male patients with DD (mean age 11.2±1.5 years) diagnosed according to DSM-IV criteria and 16 typically developing boys (mean age 10.5±1.1 years) performed a mental rotation and a delay-match to sample (DMTS) task while undergoing fMRI. All participants were medication-naïve at the time of testing. Results Compared to typically developing young people, the DD group showed less activation in left frontal regions including left ventro- and dorsolateral prefrontal cortices (PFC) during mental rotation. Medial frontal regions including dorsomedial PFC, anterior cingulate cortex and frontal pole also showed relatively reduced activation. During the DMTS task patients showed significantly more activation in the right precuneus and posterior cingulate cortex. Limitations This was a cross-sectional study with a small sample limiting the generalizability of the results. Conclusions The results complement previous findings in adults with MDD that have shown differential activation of left PFC regions during working memory tasks. Additionally, altered function of cortical midline structures in young patients with DD was identified. This supports findings in children, adolescents and adults with MDD suggesting that the pathophysiology of depressive disorders extends to DD as a risk factor for MDD and exhibits continuity over the lifespan

    Exponential suppression of thermal conductance using coherent transport and heterostructures

    We consider coherent thermal conductance through multilayer photonic crystal heterostructures, consisting of a series of cascaded non-identical photonic crystals. We show that thermal conductance can be suppressed exponentially with the number of cascaded crystals, due to the mismatch between photonic bands of all crystals in the heterostructure.Comment: 15 pages, 4 figure

    Study of fuel systems for LH2-fueled subsonic transport aircraft, volume 1

    Several engine concepts examined to determine a preferred design which most effectively exploits the characteristics of hydrogen fuel in aircraft tanks received major emphasis. Many candidate designs of tank structure and cryogenic insulation systems were evaluated. Designs of all major elements of the aircraft fuel system including pumps, lines, valves, regulators, and heat exchangers received attention. Selected designs of boost pumps to be mounted in the LH2 tanks, and of a high pressure pump to be mounted on the engine were defined. A final design of LH2-fueled transport aircraft was established which incorporates a preferred design of fuel system. That aircraft was then compared with a conventionally fueled counterpart designed to equivalent technology standards
