    Potential use of the anticonvulsants in the outpatient treatment of alcohol dependence

    Currently three medications (disulfiram, naltrexone and acamprosate) are approved by the FDA to treat alcohol dependence by the FDA. The classical anticonvulsive drugs are rarely employed as an alternative because of their side effects, but the latest generation of anticonvulsants could be useful. The anticonvulsants can be a alternative to BZD and other pharmacological treatments in the prevention of complications during the detoxification therapy, because of the absence of addictive properties and a better adverse effects profile than classical anticonvulsant drugs. Anticonvulsants such as carbamazepine, valproic acid, gabapentin and topiramate have shown to be excellent treatment for alcohol withdrawal and for the prevention of alcohol relapse. Although none of these agents have been approved by the FDA yet, there is growing evidence in the literature to support their use.Atualmente três medicações (dissulfiram, naltrexona e acamprosato) são aprovadas pela Food and Drug Administration (FDA) para tratar a dependência de álcool. As drogas anticonvulsivantes clássicas são raramente empregadas como alternativa por causa dos seus efeitos colaterais, mas a sua última geração pode ser útil. Os anticonvulsivantes podem ser uma alternativa aos benzodiazepínicos (BZD) e a outros tratamentos farmacológicos na prevenção de complicações na desintoxicação por apresentarem ausência de propriedades aditivas e um melhor perfil de efeitos adversos do que os anticonvulsivantes clássicos. Anticonvulsivantes como carbamazepina, ácido valpróico, gabapentina e topiramato demonstraram-se excelentes tratamentos para síndrome de abstinência do álcool e prevenção de recaídas.     A phenomenological model for structural phase transitions in incommensurate alkane/urea inclusion compounds

    n-Alkane/urea inclusion compounds are crystalline materials in which n-alkane ‘guest’ molecules are located within parallel one-dimensional ‘host’ tunnels formed by a helical hydrogen-bonded arrangement of urea molecules. The periodic repeat distance of the guest molecules along the host tunnels is incommensurate with the periodic repeat distance of the host substructure. The structural properties of the high-temperature phase of these materials (phase I), which exist at ambient temperature, are described by a (3 + 1)-dimensional superspace. Recent publications have suggested that, in the prototypical incommensurate composite systems, n-nonadecane/urea and n-hexadecane/urea, two low-temperature phases II and ‘III’ exist and that one or both of these phases are described by a (3 + 2)-dimensional superspace. We present a phenomenological model based on symmetry considerations and developed in the frame of a pseudo-spin–phonon coupling mechanism, which accounts for the mechanisms responsible for the I ↔ II ↔ ‘III’ phase sequence. With reference to published experimental data, we demonstrate that, in all phases of these incommensurate materials, the structural properties are described by (3 + 1)-dimensional superspace groups. Around the temperature of the II ↔ ‘III’ transition, the macroscopic properties of the material are not actually associated with a phase transition, but instead represent a ‘crossover’ between two regimes involving different couplings between relevant order parameters

    Pseudo spins with dimensionality larger than two may be used to describe disordered molecular, or ionomolecular, crystals in which the molecules have several equivalent orientations. We show that such models predict complex ordering schemes for these crystals, we describe the various problems related to the construction of these schemes and apply this technique in detail to the case of NH3(CH2)sNH3CdC1an4d briefly to other similar layered perovskites

    Cytomegalovirus Cell-Mediated Immunity: Ready for Routine Use?

This includes improved risk stratification during transplantation compared to relying solely on CMV serostatus, as well as determining the optimal duration of antiviral prophylaxis, deciding on antiviral therapy when asymptomatic replication occurs, and estimating the risk of recurrence. In this review, we initially provide an overlook of the current concepts into the immune control of CMV after transplantation. We then summarize the existent literature on the clinical experience of the use of immune monitoring in organ transplantation, with a particular interest on the outcomes of interventional trials. Current evidence indicates that cell-mediated immune assays are helpful in identifying patients at low risk for replication for whom preventive measures against CMV can be safely withheld.     Dipole-active optical phonons in YTiO_3: ellipsometry study and lattice-dynamics calculations

    The anisotropic complex dielectric response was accurately extracted from spectroscopic ellipsometry measurements at phonon frequencies for the three principal crystallographic directions of an orthorhombic (Pbnm) YTiO_3 single crystal. We identify all twenty five infrared-active phonon modes allowed by symmetry, 7B_1u, 9B_2u, and 9B_3u, polarized along the c-, b-, and a-axis, respectively. From a classical dispersion analysis of the complex dielectric functions \tilde\epsilon(\omega) and their inverses -1/\tilde\epsilon(\omega) we define the resonant frequencies, widths, and oscillator strengths of the transverse (TO) and longitudinal (LO) phonon modes. We calculate eigenfrequencies and eigenvectors of B_1u, B_2u, and B_3u normal modes and suggest assignments of the TO phonon modes observed in our ellipsometry spectra by comparing their frequencies and oscillator strengths with those resulting from the present lattice-dynamics study. Based on these assignments, we estimate dynamical effective charges of the atoms in the YTiO_3 lattice. We find that, in general, the dynamical effective charges in YTiO_3 lattice are typical for a family of perovskite oxides. By contrast to a ferroelectric BaTiO_3, the dynamical effective charge of oxygen related to a displacement along the c-axis does not show the anomalously large value. At the same time, the dynamical effective charges of Y and ab-plane oxygen exhibit anisotropy, indicating strong hybridization along the a-axis.Comment: 8 pages, 7 figure

    Unconventional T cells and kidney disease

    Unconventional T cells are a diverse and underappreciated group of relatively rare lymphocytes that are distinct from conventional CD4+ and CD8+ T cells, and that mainly recognize antigens in the absence of classical restriction through the major histocompatibility complex (MHC). These non-MHC-restricted T cells include mucosal-associated invariant T (MAIT) cells, natural killer T (NKT) cells, γδ T cells and other, often poorly defined, subsets. Depending on the physiological context, unconventional T cells may assume either protective or pathogenic roles in a range of inflammatory and autoimmune responses in the kidney. Accordingly, experimental models and clinical studies have revealed that certain unconventional T cells are potential therapeutic targets, as well as prognostic and diagnostic biomarkers. The responsiveness of human Vγ9Vδ2 T cells and MAIT cells to many microbial pathogens, for example, has implications for early diagnosis, risk stratification and targeted treatment of peritoneal dialysis-related peritonitis. The expansion of non-Vγ9Vδ2 γδ T cells during cytomegalovirus infection and their contribution to viral clearance suggest that these cells can be harnessed for immune monitoring and adoptive immunotherapy in kidney transplant recipients. In addition, populations of NKT, MAIT or γδ T cells are involved in the immunopathology of IgA nephropathy and in models of glomerulonephritis, ischaemia–reperfusion injury and kidney transplantation

    Etiology of liver cirrhosis in Brazil: chronic alcoholism and hepatitis viruses in liver cirrhosis diagnosed in the state of Espfrito Santo

    OBJECTIVES: To report the etiology of liver cirrhosis cases diagnosed at the University Hospital in Vitoria, Espirito Santo, Brazil. METHODS: The medical charts of patients with liver cirrhosis who presented to the University Hospital in Vitoria were reviewed. Chronic alcoholism and the presence of hepatitis B or C infections (HBV and HCV, respectively) were pursued in all cases. RESULTS: The sample consisted of 1,516 cases (male:female ratio 3.5:1, aged 53.2±12.6 years). The following main etiological factors were observed: chronic alcoholism alone (39.7%), chronic alcoholism in association with HBV or HCV (16.1 %), HCV alone (14.5%) and in association with alcoholism (8.6%) (total, 23.1 %), and HBV alone (13.1%) and in association with alcoholism (7.5%, total 20.6%). The remaining etiologies included cryptogenic cases (9.8%) and other causes (6.0%). The mean patient age was lower and the male-to-female ratio was higher in the cirrhosis cases that were associated with alcoholism or HBV compared with other causes. Intravenous drug abuse and a history of surgery or blood transfusion were significantly associated with HCV infection. Hepatocellular carcinoma was present at the time of diagnosis in 15.4% of cases.     Second-order nonlinear optical properties induced by thermal poling in photonic oxide glasses and transparent glass-ceramics

    In recent years there has been a resurgence of interest in oxide glasses due to advances in lasers for information transport. Oxide glasses combine low cost of fabrication and good compatibility with silica glass fibers, which offer the opportunity for developing structures with nonlinear optical properties in integrated optical devices. The creation of an axial symmetry under thermal poling is currently necessary to induce Second-Order NonLinear (SONL) optical properties in glasses. A description of theoretical models which have been proposed for charge migration during thermal poling is presented. A review of SONL efficiencies which have been obtained for different glass compositions by this method is reported. Correlations between SONL properties and structural modifications under poling are also presented. Finally, we focus on the challenging fabrication of transparent glass-ceramic composites, especially when they are obtained by the precipitation of ferroelectric nanoparticle phases in the glassy matrix which adds the advantageous SONL properties of ferroelectric crystals

    Optical conductivity spectra of single crystals of Ca_1-xSr_xVO_3 have been studied to elucidate how the electronic behavior depends on the strength of the electron correlation without changing the nominal number of electrons per vanadium atom. The effective mass deduced by the analysis of the Drude-like contribution do not show critical enhancement, even though the system is close to the Mott transition. Besides the Drude-like contribution, two anomalous features were observed in the optical conductivity spectra of the intraband transition within the 3d band. These features can be assigned to transitions involving the incoherent and coherent bands near the Fermi level. The large spectral weight redistribution in this system, however, does not involve a large mass enhancement.Comment: 12 pages in a Phys. Rev. B camera-ready format with 16 EPS figures embedded.
