    Respiratorni simptomi u radnica na preradi riba na obali Jadrana u Hrvatskoj

    This article describes respiratory symptoms and lung function in 98 fish processing female workers employed in a fish processing plant located on the Croatian Adriatic coast and 95 matching controls. The study included chronic and acute respiratory symptoms which developed during the shifts. Lung function measurements included forced vital capacity (FVC), one-second forced expiratory volume (FEV1) and maximal expiratory rates at 50 % and the last 25 % (FEF50, FEF25). Chronic respiratory symptoms were significantly dominant in fish processing workers compared to controls. The most common chronic symptoms were hoarseness (57.1 %), nasal catarrh (51.0 %), chronic cough (42.9 %), chronic phlegm (34.7 %), and frequent chest cold (35.7 %). Exposed smokers and nonsmokers had a similar prevalence of chronic respiratory symptoms. Acute symptoms over the work shift were high, with headache in lead (smokers: 62.5 %; nonsmokers: 56.1 %). Most of the ventilatory capacity parameters were significantly lower than predicted, FEF25 in particular, indicating obstructive changes predominantly in the smaller airways. These findings suggest that fish processing workers are prone to developing acute and chronic respiratory symptoms as well as to lung function changes. This calls for medical and technical preventive measures to be introduced in the work environment of the fi sh processing plant.Cilj je ovoga istraživanja bio ispitati respiratorne simptome i plućnu funkciju radnica zaposlenih na preradi riba u industriji na obali Jadranskog mora u Hrvatskoj. U istraživanje je uključeno 98 radnica zaposlenih na preradi riba i 95 žena neizložene kontrolne skupine. Ispitivani su kronični i akutni respiratorni simptomi koji se razvijaju tijekom radne smjene. Mjerena je plućna funkcija registriranjem forsiranoga vitalnog kapaciteta (FVC), forsiranoga ekspiracijskog volumena u prvoj sekundi (FEV1) te maksimalnoga ekspiracijskog protoka pri 50 % i zadnjih 25 % forsiranoga vitalnog kapaciteta (FEF50, FEF25) na krivulji maksimalni ekspiracijski protok-volumen (MEPV). Učestalost većine kroničnih respiracijskih simptoma bila je značajno viša u eksponiranih u usporedbi s kontrolnom skupinom. U eksponiranih radnica utvrđena je i visoka prevalencija akutnih simptoma koji se razvijaju tijekom radne smjene, posebno za promuklost (57,1 %) i katar nosa (51 %), potom slijedi kronični kašalj (42,9 %), kronični iskašljaj (34,7 %), upale sinusa (32 %) i česte prehlade (35,7 %). S obzirom na naviku pušenja pušači i nepušači imali su sličnu prevalenciju kroničnih respiratornih simptoma. Izložene radnice imale su visoku prevalenciju akutnih simptoma tijekom radne smjene i to naročito za glavobolju (pušači 62,5 %; nepušači 56,1 %). Ventilacijska funkcija pluća bila je značajno smanjena u usporedbi s predviđenim normalnim vrijednostima posebice za FEF25 % upućujući na opstruktivne promjene pretežno u manjim dišnim putovima. Naši podaci upućuju na opasnost razvoja kroničnih i akutnih respiracijskih simptoma i promjena plućne funkcije u radnika koji rade u industriji na preradi riba. Medicinske i tehničke preventivne mjere u radnom okolišu treba preporučiti u industriji prerade riba

    Efecto insecticida de los aceites esenciales de algunas lamiáceas sobre tecia solanivora povolny en condiciones de laboratorio

    La papa, el producto agrícola más importante en la economía delos andes colombianos, tiene en la polilla guatemalteca (Teciasolanivora Povolny) su principal plaga, cuyo control se basa enel uso intensivo de plaguicidas. Una alternativa potencial a laaplicación de estos insumos es el uso de productos vegetales.En condiciones controladas, el presente trabajo evaluó el efectode los aceites esenciales de Ocimum basilicum, O. majorana,M. piperita, O. vulgare, P. cablin, R. officinalis, S. officinalis yT. vulgaris sobre los huevos, larvas y adultos de T. solanivoraal ser aplicados directamente sobre los tubérculos, y sobre lasuperficie del suelo. No se encontró ningún efecto de los tratamientossobre el desarrollo larval ni sobre la longevidad delos adultos. En las dos modalidades de aplicación estudiadas,los aceites de P. cablin y T. vulgaris en una concentración del0,25% produjeron una reducción significativa en la viabilidadde los huevos. Al aumentar la humedad del suelo y el volumende aplicación de los aceites, los aceites de O. basilicum, O.majorana, M. piperita, R. officinalis también tuvieron acciónovicida. Estos resultados son evidencia del potencial de estosaceites como controladores de Tecia solanivora

    We are presented with the case report of a newborn baby with gastroschisis, which is a congenital handicap characterized by the incomplete closure of the abdominal wall. The newborn was admitted to the neonatal ICU for stabilization and ulterior surgical closure of the defect. After surgery and during the hospital stay, an individualized caring plan based on the NANDA diagnosis was applied, interventions according to NIC classification, and expected results according to NOC classification. Thanks to the applied plan the newborn was discharged from hospital being suction fed and with the proper gastrointestinal transit.Se presenta el caso clínico de un recién nacido con gastrosquisis, que es un defecto congénito caracterizado por el cierre incompleto de la pared abdominal, que ingresa en la UCI Neonatal para su estabilización y posterior cierre quirúrgico del defecto. Tras la intervención y durante su estancia se aplicó un plan de cuidados individualizado basado en los diagnósticos NANDA, las intervenciones según la clasificación NIC y los resultados esperados según la clasificación NOC. Gracias al plan aplicado fue dado de alta tomando alimentación por succión y con adecuado transito gastrointestinal

    A retrospective, multicenter study of the efficacy of lapatinib plus trastuzumab in HER2-positive metastatic breast cancer patients previously treated with trastuzumab, lapatinib, or both: the Trastyvere study

    [Purpose]: To evaluate the efficacy and safety of lapatinib (L) and trastuzumab (T) combination in HER2-positive metastatic breast cancer (MBC) patients previously treated with T and/or L.[Materials and methods]: We conducted a retrospective, post-authorized, multicenter study including patients with HER2-positive MBC or locally advanced breast cancer (ABC) treated with the combination of L–T. Concomitant endocrine therapy, as well as brain metastasis and/or prior exposure to L, were allowed.[Results]: One hundred and fifteen patients from 14 institutions were included. The median age was 59.8 years. The median number of prior T regimens in the advanced setting was 3 and 73 patients had received a prior L regimen. The clinical benefit rate (CBR) was 34.8% (95% CI 26.1–43.5). Among other efficacy endpoints, the overall response rate was 21.7%, and median progression-free survival (PFS) and overall survival were 3.9 and 21.6 months, respectively. Heavily pretreated and ≥ 3 metastatic organ patients showed lower CBR and PFS than patients with a low number of previous regimens and < 3 metastatic organs. Moreover, CBR did not significantly change in L-pretreated compared with L-naïve patients (31.5% versus 40.5% for L-pretreated versus L-naïve). Grade 3/4 adverse events were reported in 19 patients (16.5%).[Conclusion]: The combination of L–T is an effective and well-tolerated regimen in heavily pretreated patients and remains active among patients progressing on prior L-based therapy.     Static and Dynamic Lung Volumes in Swimmers and Their Ventilatory Response to Maximal Exercise

    Purpose While the static and dynamic lung volumes of active swimmers is often greater than the predicted volume of similarly active non-swimmers, little is known if their ventilatory response to exercise is also different. Methods Three groups of anthropometrically matched male adults were recruited, daily active swimmers (n = 15), daily active in fields sport (Rugby and Football) (n = 15), and recreationally active (n = 15). Forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), and maximal voluntary ventilation (MVV) was measured before and after exercise to volitional exhaustion. Results Swimmers had significantly larger FVC (6.2 ± 0.6 l, 109 ± 9% pred) than the other groups (5.6 ± 0.5 l, 106 ± 13% pred, 5.5 ± 0.8, 99% pred, the sportsmen and recreational groups, respectively). FEV1 and MVV were not different. While at peak exercise, all groups reached their ventilatory reserve (around 20%), the swimmers had a greater minute ventilation rate than the recreational group (146 ± 19 vs 120 ± 87 l/min), delivering this volume by breathing deeper and slower. Conclusions The swimmers utilised their larger static volumes (FVC) differently during exercise by meeting their ventilation volume through long and deep breaths

    Effect of obesity on constant workrate exercise in hyperinflated men with COPD

    <p>Abstract</p> <p>Background</p> <p>Chronic obstructive pulmonary disease (COPD) and a high body mass index (BMI) can both affect pulmonary volumes as well as exercise tolerance, but their combined effect on these outcomes is not well known. The aim of this study was to investigate the effects of increased BMI during constant workrate cycle ergometry in patients with COPD.</p> <p>Methods</p> <p>Men with COPD and hyperinflation were divided according to World Health Organization BMI classification: 84 normal BMI (NBMI), 130 overweight (OW) and 64 obese (OB). Patients underwent spirometric and lung volumes assessment and an incremental cycling exercise test. This was followed by a constant workrate exercise test (CET) at 75% of peak capacity. Inspiratory capacity and Borg dyspnea scores were measured at baseline, during and at the end of CET.</p> <p>Results and discussion</p> <p>FEV<sub>1 </sub>% predicted was not different across BMI classes. Total lung capacity and functional residual capacity were significantly lower in OB and OW compared to NBMI patients. Peak VO<sub>2 </sub>in L·min<sup>-1 </sup>was significantly higher in OB and OW patients than in NBMI patients. CET time was not different across BMI classes (p = 0.11). Changes in lung volumes and dyspnea during CET were not different between BMI categories.</p> <p>Conclusions</p> <p>OB and OW patients with COPD had a higher peak VO<sub>2 </sub>than their lean counterparts. Endurance time, dyspnea and changes in lung volumes during CET were similar between BMI categories.</p
