A introdução da odontologia hospitalar através do treinamento de um procedimento operacional padrão de higiene bucal em unidade de terapia intensiva pediátrica

Defesa : Curitiba, 04/04/2018Inclui referências: p.118-135Área de concentração: Terapia Intensiva PediátricaResumo: Introdução: A atenção à saúde bucal realizado por profissional de odontologia em pacientes internados na Unidade de Terapia Intensiva (UTI) vai além da promoção da higiene bucal, visando também a prevenção de doenças, tanto bucais quanto sistêmicas, que possam estar associadas ou agravadas em decorrência de condições bucais inadequadas. O cirurgião-dentista é o profissional de responsabilidade técnica e científica na prevenção e tratamento de doenças orais, que pode contribuir no cuidado integral de pacientes criticamente enfermos. Objetivos: Descrever o perfil da equipe multidisciplinar de atendimento da Unidade de Terapia Intensiva Pediátrica do Complexo Hospital de Clínicas da Universidade Federal do Paraná (UTIP-CHC/UFPR). Verificar o conhecimento prévio da equipe sobre higiene bucal, procedimento operacional padrão de higiene bucal (POP de HB) preconizada anteriormente na Unidade, prevenção de lesões bucais e as consequências de uma higiene bucal inadequada. Analisar os resultados do treinamento da equipe multidisciplinar na rotina da prática do POP de HB, verificando a frequência com que as práticas foram realizadas. Métodos: Estudo observacional, analítico, transversal, com coleta prospectiva de dados para descrever o perfil da equipe de atendimento da UTIP-CHC/UFPR; Estudo do tipo antes e depois para verificar a prática do POP de HB. Foi aplicado um questionário à 56 profissionais responsáveis pela assistência ao paciente internado na UTIP-CHC/UFPR. Posteriormente, 58 profissionais da equipe multiprofissional passaram por treinamento do novo POP de HB. Resultados: 89,0% dos profissionais que responderam o questionário não tiveram treinamento de higiene bucal anterior; 61,0% não conhecia o POP de HB usado na UTIP e 94,6% consideraram a HB como uma forma de evitar lesões bucais em pacientes internados em hospital. Foram treinados 97,0% dos profissionais da equipe de enfermagem (enfermeiros e técnicos); e após o treinamento, houve um aumento para 86,0% na utilização da escova dental, redução para 18,3% de frequência de saburra lingual e aumento em 44,3% no uso de hidrante labial nos pacientes acompanhados. Mais de 30,0% dos prontuários ficaram sem registro da realização de HB ao longo do estudo. Conclusão: A equipe multiprofissional da UTIP-CHC/UFPR possuía integrantes com longo tempo de formação profissional e experiência em UTIP, embora a maioria não tivesse passado por treinamento de higiene bucal. A equipe multidisciplinar da UTIP-CHC/UFPR possuía conhecimento prévio superficial sobre higiene bucal e saúde bucal de crianças em condições críticas; e desconhecia de forma geral, o POP de HB instituído. O treinamento do novo POP de HB aconteceu com êxito, atingindo um número expressivo de profissionais. Foi um trabalho pioneiro dentro desta instituição e oportunizou a construção multidisciplinar de um bundle de prevenção a PAV e possibilitou a disseminação de conhecimento técnico científico a outros setores. Houve melhora na execução do novo POP de higiene bucal, porém a equipe não conseguiu cumpri-lo na íntegra. Palavras-chave: Unidade de Terapia Intensiva Pediátrica. Higiene Bucal. Prevenção de Infecção Hospitalar.Abstract: Introduction: The performance of the dentist in the Intensive Care Unit (ICU) goes beyond the promotion of oral hygiene, aiming also at the prevention of diseases, both oral and systemic, that may be associated or aggravated, due to inadequate oral conditions. The dental surgeon is the professional of technical and scientific responsibility in the prevention and treatment of oral diseases, which can contribute to the integral care of critically ill patients. Objectives: To describe the profile of the multidisciplinary care team of the Pediatric Intensive Care Unit of the Hospital de Clinicas Complex of the Federal University of Paraná; To check the prior knowledge of the team on oral hygiene, standard operating procedure of oral hygiene previously recommended in the Unit, prevention of oral lesions and the consequences of inadequate oral hygiene; To analyze the results of the training of the multidisciplinary team in the routine practice of the standard oral hygiene operating procedure, verifying the frequency with which the practices were performed Methods: An observational, analytical, cross-sectional study with prospective data collection to describe the profile of the care team of the Pediatric Intensive Care Unit of the Hospital de Clinicas Complex of the Federal University of Paraná; b) Before-and-after study to verify the practice of the standard oral hygiene operating procedure. A questionnaire was applied to 56 professionals responsible for patient care at the PICU of a public hospital in Curitiba. Subsequently, 58 professionals from the multiprofessional team underwent training in the new POP of oral hygiene. Results: 89% of the professionals who answered the questionnaire did not have previous oral hygiene training; 61% did not know the POP of oral hygiene used in the PICU and 94.6% considered oral hygiene as a way to avoid oral lesions in hospital patients; 97% of the nursing staff (nurses and technicians) were trained; post-training, there was an increase to 86% in toothbrush use, there was a reduction to 18.3% in patients with lingual flap, there was a 44.3% increase in the use of lip balm in the patients followed up. More than 30% of medical records were unrecorded for oral hygiene over the course of the study. Conclusion: The PICU-CHC/UFPR multiprofessional team had members with a long time of professional training and experience in PICUs, although most did not have oral hygiene training. The multidisciplinary team at PICU-CHC / UFPR had prior superficial knowledge about oral hygiene and oral health of children under critical conditions; and the team did not know the oral hygiene POP established. The training of the new oral hygiene POP was successful, reaching a significant number of professionals. It was a pioneering work within this institution and made possible the multidisciplinary construction of a Bundle for prevention to VAP, made possible the dissemination of scientific technical knowledge to other sectors. There was improvement in the execution of the new POP of oral hygiene, but the team could not perform all the items. Key words: Pediatric Intensive Unit Care. Oral Hygiene. ODONTOLOGIA NA ALDEIA: SAÚDE BUCAL DE QUALIDADE

Segundo a OMS (Organização Mundial da Saúde) a saúde é um estado de completo bem estar físico, mental, social e não somente a ausência de enfermidades. Esta pesquisa tem como objetivo identificar as principais alterações bucais encontradas nas populações indígenas e com base em conhecimentos adquiridos informar a causa de certas doenças orais e sua prevenção, respeitando os hábitos culturais das aldeias indígenas. Foram utilizados como método de pesquisa um questionário e exame bucal, com o intuito de obter informações sobre a dieta, cultura, hábitos de higiene e a presença da doença cárie nas aldeias, onde foram identificados altos índices de doença cárie devido à escassez de uma assistência odontológica, seus hábitos alimentares e uma má higiene bucal. Portanto é necessária a melhoria das condições bucais através de programas de saúde e prevenção, seguido de orientações do cirurgião dentista sobre uma dieta não cariogênica e uma boa higienização oral

Etude des dalles sur sols renforcés au moyen d'inclusions rigides ou non

Soft soil reinforcement by inclusion is a growing technique caracterized by a pile grid and a granular embankment introduced between the reinforced soil and the structure. Unlike traditionnal methods, the load is partially transferred to the pile heads by arching in the embankment. The application area of this research focuses on the shallow foundations case, in which the thickness of the embankment is small. The litterature review shows that only a few studies were dedicated to that case, and that fundamental questions remains concerning the load transfer in the embankment. Chosen method for this research consists in two-dimensionnal physical modelling, analysis of the conducted simulations, and development of an analytical model in order to predict the load transfer to the piles by arching in the embankment. The results of this PhD thesis provide original elements of evidence of the load transfer in the studied system, proposes an analytical model based on block division of the granular embankment by shear bands - which is in good agreement with experimental data - and lead to a better understanding of arching in soils.Doctorat en Sciences de l'ingénieurinfo:eu-repo/semantics/nonPublishe
