    Shortened Time to Identify Staphylococcus Species from Blood Cultures and Methicillin Resistance Testing Using CHROMAgar

    The ability to rapidly differentiate coagulase-negative staphylococcus (CoNS) from Staphylococcus aureus and to determine methicillin resistance is important as it affects the decision to treat empiric antibiotic selection. The objective of this study was to evaluate CHROMagar S. aureus and CHROMagar MRSA (Becton Dickinson) for rapid identification of Staphylococcus spp. directly from blood cultures. Consecutive blood culture bottles (BacT Alert 3D SA and SN, bioMérieux) growing gram-positive cocci in clusters were evaluated. An aliquot was plated onto CHROMagar MRSA (C-MRSA) and CHROMagar S. aureus (C-SA) plates, which were read at 12 to 16 hours. C-SA correctly identified 147/147 S. aureus (100% sensitivity); 2 CoNS were misidentified as S. aureus (98% specificity). C-MRSA correctly identified 74/77 MRSA (96% sensitivity). None of the MSSA isolates grew on C-MRSA (100% specificity). In conclusion, CHROMagar is a rapid and sensitive method to distinguish MRSA, MSSA, and coagulase-negative Staphylococcus and may decrease time of reporting positive results

    Malnourished HIV-infected patients starting antiretroviral therapy (ART) are at high risk of early mortality, some of which may be attributed to altered electrolyte metabolism. We used data from a randomised controlled trial of electrolyte-enriched lipid-based nutritional supplements to assess the association of baseline and time-varying serum phosphate and K concentrations with mortality within the first 12 weeks after starting ART. Baseline phosphate results were available from 1764 patients and there were 9096 subsequent serum phosphate measurements, a median of 6 per patient. For serum K there were 1701 baseline and 8773 subsequent measures, a median of 6 per patient. Abnormally high or low serum phosphate was more common than high or low serum K. Controlling for other factors found to affect mortality in this cohort, low phosphate which had not changed from the previous time interval was associated with increased mortality; the same was not true for high phosphate or for high or low K. Both increases and decreases in serum electrolytes from the previous time interval were generally associated with increased mortality, particularly in the electrolyte-supplemented group. The results suggest that changes in serum electrolytes, largely irrespective of the starting point and the direction of change, were more strongly associated with mortality than were absolute electrolyte levels. Although K and phosphate are required for tissue deposition during recovery from malnutrition, further studies are needed to determine whether specific supplements exacerbate physiologically adverse shifts in electrolyte levels during nutritional rehabilitation of ill malnourished HIV patients

By examining the sound of sex in Tsai Ming-liang’s 2005 film, The Wayward Cloud, this article will argue that the aural is privileged over the visual and explore its implications for female subjectivity, sexual intimacy and gender politics. It suggests that the film challenges us to think whether it might be possible to forge what Mary Ann Doane calls ‘a political erotics of the voice’, but in a wayward manner that deploys comatose bodies that have no voice, that fragments the unity of voice and body and that privileges the representation of the sonic over the visual in a cinematic tradition that generally dictates otherwise

    Aerogel Scattering Filters for Cosmic Microwave Background Observations

    We present the design and performance of broadband and tunable infrared-blocking filters for millimeter and sub-millimeter astronomy composed of small scattering particles embedded in an aerogel substrate. The ultra-low-density (< 100 mg/cu cm) aerogel substrate provides an index of refraction as low as 1.05, removing the need for anti-reflection coatings and allowing for broadband operation from DC to above 1 THz. The size distribution of the scattering particles can be tuned to provide a variable cutoff frequency. Aerogel filters with embedded high-resistivity silicon powder are being produced at 40-cm diameter to enable large-aperture cryogenic receivers for cosmic microwave background polarimeters, which require large arrays of sub-Kelvin detectors in their search for the signature of an inflationary gravitational-wave background
