    Clinical Presentation and Treatment Outcomes of Children and Adolescents with Low Back Pain in Physical Therapy

    Purpose: Low back pain (LBP) is a common condition in adolescents, and a specific pathoanatomical origin for the symptoms cannot always be determined. The purpose of this work was to examine the clinical presentation and outcomes of rehabilitation for adolescents with LBP. Methods: This study was completed in three phases. Phase 1 was a retrospective review of 25 patients under the age of 18 who were seen for treatment of LBP. Information regarding the patients' diagnoses, history, examination, and outcome were collected from chart review. Phase 2 was a review of 99 patients under the age of 18 with LBP. Numerical Pain Rating Scale (NPRS) and the Modified Oswestry Questionnaire (OSW) were recorded. Phase 3 consisted of a prospective study of treatment-based classification (TBC) of 34 adolescent patients seen in physical therapy for LBP. Treatment duration and content were at the clinician's discretion. Patients completed an OSW and NPRS before and after physical therapy. Patients were classified using a TBC algorithm, and effectiveness of classification on outcome was examined. Results: In study 1, initial pain scores were lower if a specific pathology was present (P=.001). Initial pain and OSW scores were poorly correlated (r= 0.16). 44% (n=11) of patients scored under the floor value of 12% on OSW. A second examination of the OSW in study 2 concluded that OSW was moderately correlated with NPRS (0.59). Chronbach's alpha was 0.86. All 10 items in OSW appeared to load onto two latent factors. In study 3, a classification decision was able to be made with a moderate degree of reliability (0.53 (0.28, 0.79) ≤ κ ≤ 0.89 (.74, 1.0)) in all of the 34 patients. Stabilization was the most commonly prescribed treatment by clinicians. Those who were matched to their TBC classification experienced fewer numbers of visits than those who were not. Conclusions: It appears the OSW is a valid and reliable tool for assessing clinical outcome of physical therapy intervention for adolescents with LBP. These results also suggest that a TBC approach to treatment of LBP in this population may be effective for improvements in clinical outcome

    Dietary nitrate and population health: a narrative review of the translational potential of existing laboratory studies

    BACKGROUND: Dietary inorganic nitrate (NO(3)(−)) is a polyatomic ion, which is present in large quantities in green leafy vegetables and beetroot, and has attracted considerable attention in recent years as a potential health-promoting dietary compound. Numerous small, well-controlled laboratory studies have reported beneficial health effects of inorganic NO(3)(−) consumption on blood pressure, endothelial function, cerebrovascular blood flow, cognitive function, and exercise performance. Translating the findings from small laboratory studies into ‘real-world’ applications requires careful consideration. MAIN BODY: This article provides a brief overview of the existing empirical evidence basis for the purported health-promoting effects of dietary NO(3)(−) consumption. Key areas for future research are then proposed to evaluate whether promising findings observed in small animal and human laboratory studies can effectively translate into clinically relevant improvements in population health. These proposals include: 1) conducting large-scale, longer duration trials with hard clinical endpoints (e.g. cardiovascular disease incidence); 2) exploring the feasibility and acceptability of different strategies to facilitate a prolonged increase in dietary NO(3)(−) intake; 3) exploitation of existing cohort studies to explore associations between NO(3)(−) intake and health outcomes, a research approach allowing larger samples sizes and longer duration follow up than is feasible in randomised controlled trials; 4) identifying factors which might account for individual differences in the response to inorganic NO(3)(−) (e.g. sex, genetics, habitual diet) and could assist with targeted/personalised nutritional interventions; 5) exploring the influence of oral health and medication on the therapeutic potential of NO(3)(−) supplementation; and 6) examining potential risk of adverse events with long term high- NO(3)(−) diets. CONCLUSION: The salutary effects of dietary NO(3)(−) are well established in small, well-controlled laboratory studies. Much less is known about the feasibility and efficacy of long-term dietary NO(3)(−) enrichment for promoting health, and the factors which might explain the variable responsiveness to dietary NO(3)(−) supplementation between individuals. Future research focussing on the translation of laboratory data will provide valuable insight into the potential applications of dietary NO(3)(−) supplementation to improve population health

    Effects of partial penectomy for penile cancer on sexual function: A systematic review.

    Penile cancer is a rare but debilitating condition, which often requires aggressive treatment. Partial penectomy is considered as a treatment option when a sufficient portion of the penile shaft can be maintained to preserve functionality. This systematic review, which followed the PRIMSA guidelines, aimed to evaluate the effects of partial penectomy for penile cancer on sexual function-the maintenance of which is often a priority in patient groups-and to identify potential factors which may moderate these effects. A systematic search of PubMed, The Cochrane Library, and Open Grey as well as MEDLINE, CINAHL and Open Dissertations via EBSCOhost was conducted from inception through to 24th March, 2022. Studies were required to include adults aged ≥18 years who had undergone partial penectomy for the treatment of penile cancer, with a quantitative measure of sexual function available pre- and post-surgery. Four eligible articles were identified for inclusion in this review, three of which reported a decrease in sexual function pre- to post-surgery across all domains of the International Index of Erectile Function (IIEF) questionnaire (erectile function, orgasmic function, sexual desire, intercourse satisfaction and overall satisfaction). Conversely, one study reported an increase in sexual function across IIEF domains, except for orgasmic function, which decreased, pre- to post-surgery. Greater penile length was associated with higher post-operative sexual function, whilst increasing age and higher anxiety levels were associated with lower post-operative sexual function levels in one study. Despite the overall drop in sexual function, many patients were still able to maintain satisfactory sex lives following partial penectomy. Given the limited research in this area and small sample sizes across studies, additional well-controlled investigations are warranted to provide further evidence on the effects of partial penectomy for penile cancer on sexual function

    The Mediterranean dietary pattern for optimising health and performance in competitive athletes: A narrative review

    Nutrition plays a key role in training for, and competing in, competitive sport, and is essential for reducing risk of injury and illness, recovering and adapting between bouts of activity, and enhancing performance. Consumption of a Mediterranean diet (MedDiet) has been demonstrated to reduce risk of various non-communicable diseases and increase longevity. Following the key principles of a MedDiet could also represent a useful framework for good nutrition in competitive athletes under most circumstances, with potential benefits for health and performance parameters. In this review, we discuss the potential effects of a MedDiet, or individual foods and compounds readily available in this dietary pattern, on oxidative stress and inflammation, injury and illness risk, vascular and cognitive function, and exercise performance in competitive athletes. We also highlight potential modifications which could be made to the MedDiet (whilst otherwise adhering to the key principles of this dietary pattern) in accordance with contemporary sports nutrition practices, to maximise health and performance effects. In addition, we discuss potential directions for future research

    Genome-Wide Transcriptional Profiling Reveals MicroRNA-Correlated Genes and Biological Processes in Human Lymphoblastoid Cell Lines

    Expression level of many genes shows abundant natural variation in human populations. The variations in gene expression are believed to contribute to phenotypic differences. Emerging evidence has shown that microRNAs (miRNAs) are one of the key regulators of gene expression. However, past studies have focused on the miRNA target genes and used loss- or gain-of-function approach that may not reflect natural association between miRNA and mRNAs.To examine miRNA regulatory effect on global gene expression under endogenous condition, we performed pair-wise correlation coefficient analysis on expression levels of 366 miRNAs and 14,174 messenger RNAs (mRNAs) in 90 immortalized lymphoblastoid cell lines, and observed significant correlations between the two species of RNA transcripts. We identified a total of 7,207 significantly correlated miRNA-mRNA pairs (false discovery rate q<0.01). Of those, 4,085 pairs showed positive correlations while 3,122 pairs showed negative correlations. Gene ontology analyses on the miRNA-correlated genes revealed significant enrichments in several biological processes related to cell cycle, cell communication and signal transduction. Individually, each of three miRNAs (miR-331, -98 and -33b) demonstrated significant correlation with the genes in cell cycle-related biological processes, which is consistent with important role of miRNAs in cell cycle regulation.This study demonstrates feasibility of using naturally expressed transcript profiles to identify endogenous correlation between miRNA and miRNA. By applying this genome-wide approach, we have identified thousands of miRNA-correlated genes and revealed potential role of miRNAs in several important cellular functions. The study results along with accompanying data sets will provide a wealth of high-throughput data to further evaluate the miRNA-regulated genes and eventually in phenotypic variations of human populations
