    Barriers as experienced by WITS first year B Ed students when accessing computers and ICTs (Information and Communication Technologies).

    In this study the possible barriers / obstacles to or use of ICTs that first year B Ed students on the Wits School of Education campus are explored. Through the means of a questionnaire requiring both quantitative and qualitative responses the students’ comments are analysed for possible trends. The issue of age being a possible barrier is evaluated in light of the literature and data provided. As two groups of students, a young pre-service group and an older in-service group, emerge, the differences and similarities in their Information and Communications Technology (ICT) experience on campus is detailed. Conclusions are drawn as to what barriers each of these groups of students face and solutions are proposed as to how these students can be assisted to overcome these hurdles in order to promote a more effective ICT experience

    Emergence of daptomycin resistance in daptomycin-naïve rabbits with methicillin-resistant Staphylococcus aureus prosthetic joint infection is associated with resistance to host defense cationic peptides and mprF polymorphisms.

    BackgroundPrevious studies of both clinically-derived and in vitro passage-derived daptomycin-resistant (DAP-R) Staphylococcus aureus strains demonstrated the coincident emergence of increased DAP MICs and resistance to host defense cationic peptides (HDP-R).MethodsIn the present investigation, we studied a parental DAP-susceptible (DAP-S) methicillin-resistant Staphylococcus aureus (MRSA) strain and three isogenic variants with increased DAP MICs which were isolated from both DAP-treated and DAP-untreated rabbits with prosthetic joint infections. These strains were compared for: in vitro susceptibility to distinct HDPs differing in size, structure, and origin; i.e.; thrombin-induced platelet microbicidal proteins [tPMPs] and human neutrophil peptide-1 [hNP-1]; cell membrane (CM) phospholipid and fatty acid content; CM order; envelope surface charge; cell wall thickness; and mprF single nucleotide polymorphisms (SNPs) and expression profiles.ResultsIn comparison with the parental strain, both DAP-exposed and DAP-naive strains exhibited: (i) significantly reduced susceptibility to each HDP (P<0.05); (ii) thicker cell walls (P<0.05); (iii) increased synthesis of CM lysyl-phosphatidylglycerol (L-PG); (iv) reduced content of CM phosphatidylglycerol (PG); and (v) SNPs within the mprF locus No significant differences were observed between parental or variant strains in outer CM content of L-PG, CM fluidity, CM fatty acid contents, surface charge, mprF expression profiles or MprF protein content. An isolate which underwent identical in vivo passage, but without evolving increased DAP MICs, retained parental phenotypes and genotype.ConclusionsTHESE RESULTS SUGGEST: i) DAP MIC increases may occur in the absence of DAP exposures in vivo and may be triggered by organism exposure to endogenous HDPs: and ii) gain-in-function SNPs in mprF may contribute to such HDP-DAP cross-resistance phenotypes, although the mechanism of this relationship remains to be defined

    Comparative in vitro activity of Meropenem, Imipenem and Piperacillin/tazobactam against 1071 clinical isolates using 2 different methods: a French multicentre study

    <p>Abstract</p> <p>Background</p> <p>Meropenem is a carbapenem that has an excellent activity against many gram-positive and gram-negative aerobic, facultative, and anaerobic bacteria. The major objective of the present study was to assess the <it>in vitro </it>activity of meropenem compared to imipenem and piperacillin/tazobactam, against 1071 non-repetitive isolates collected from patients with bacteremia (55%), pneumonia (29%), peritonitis (12%) and wound infections (3%), in 15 French hospitals in 2006. The secondary aim of the study was to compare the results of routinely testings and those obtained by a referent laboratory.</p> <p>Method</p> <p>Susceptibility testing and Minimum Inhibitory Concentrations (MICs) of meropenem, imipenem and piperacillin/tazobactam were determined locally by Etest method. Susceptibility to meropenem was confirmed at a central laboratory by disc diffusion method and MICs determined by agar dilution method for meropenem, imipenem and piperacillin/tazobactam.</p> <p>Results</p> <p>Cumulative susceptibility rates against <it>Escherichia coli </it>were, meropenem and imipenem: 100% and piperacillin/tazobactam: 90%. Against other <it>Enterobacteriaceae</it>, the rates were meropenem: 99%, imipenem: 98% and piperacillin/tazobactam: 90%. All <it>Staphylococci</it>, <it>Streptococci </it>and anaerobes were susceptible to the three antibiotics. Against non fermeters, meropenem was active on 84-94% of the strains, imipenem on 84-98% of the strains and piperacillin/tazobactam on 90-100% of the strains.</p> <p>Conclusions</p> <p>Compared to imipenem, meropenem displays lower MICs against <it>Enterobacteriaceae</it>, <it>Escherichia coli </it>and <it>Pseudomonas aeruginosa</it>. Except for non fermenters, MICs90 of carbapenems were <4 mg/L. Piperacillin/tazobactam was less active against <it>Enterobacteriaceae </it>and <it>Acinetobacter </it>but not <it>P. aeruginosa</it>. Some discrepancies were noted between MICs determined by Etest accross centres and MICs determined by agar dilution method at the central laboratory. Discrepancies were more common for imipenem testing and more frequently related to a few centres. Overall MICs determined by Etest were in general higher (0.5 log to 1 log fold) than MICs by agar dilution.</p

    Rapid Host Defense against Aspergillus fumigatus Involves Alveolar Macrophages with a Predominance of Alternatively Activated Phenotype

    The ubiquitous fungus Aspergillus fumigatus is associated with chronic diseases such as invasive pulmonary aspergillosis in immunosuppressed patients and allergic bronchopulmonary aspergillosis (ABPA) in patients with cystic fibrosis or severe asthma. Because of constant exposure to this fungus, it is critical for the host to exercise an immediate and decisive immune response to clear fungal spores to ward off disease. In this study, we observed that rapidly after infection by A. fumigatus, alveolar macrophages predominantly express Arginase 1 (Arg1), a key marker of alternatively activated macrophages (AAMs). The macrophages were also found to express Ym1 and CD206 that are also expressed by AAMs but not NOS2, which is expressed by classically activated macrophages. The expression of Arg1 was reduced in the absence of the known signaling axis, IL-4Rα/STAT6, for AAM development. While both Dectin-1 and TLR expressed on the cell surface have been shown to sense A. fumigatus, fungus-induced Arg1 expression in CD11c+ alveolar macrophages was not dependent on either Dectin-1 or the adaptor MyD88 that mediates intracellular signaling by most TLRs. Alveolar macrophages from WT mice efficiently phagocytosed fungal conidia, but those from mice deficient in Dectin-1 showed impaired fungal uptake. Depletion of macrophages with clodronate-filled liposomes increased fungal burden in infected mice. Collectively, our studies suggest that alveolar macrophages, which predominantly acquire an AAM phenotype following A. fumigatus infection, have a protective role in defense against this fungus

    Increased management through domestication is the predicted, and often necessary, commercialisation outcome of a wild resource which is subject to a demanding market that promotes competition amongst producers and the depletion of wild stocks. This has been the case for commercial buchu (Agathosma betulina and Agathosma crenulata), a historically wild collected plant which has been cultivated on a large-scale in selected areas of the Western Cape Province of South Africa. Buchu is an endemic, aromatic shrub around which a lucrative industry spanning diverse and distant markets has developed. Alongside its medicinal properties, buchu is primarily valued for its essential oil which is exported for use in international flavour and fragrant industries. The aim of this study was to conduct an overview of the local buchu industry with a focus on how cultivation has impacted on the general trade, the different actors involved and the conservation of the plant. A shift in buchu production to large-scale, agricultural enterprises raises certain questions for the involvement of rural harvesters in the trade, especially with regard to their inclusion and the sharing of benefits arising from commercialisation. Accordingly, this research sought to identify the social and economic impacts of buchu cultivation while also exploring the environmental impacts associated with large-scale farming of the plant. The methods employed in this research were primarily qualitative, based on semi-structured interviews conducted with key actors involved in the buchu trade, including harvesters, farmers, industry representatives and environmental authorities. The study revealed that while the harvesting of buchu is an important economic activity for harvester communities, the cultivation of buchu has played a limited role in local livelihoods with cultivation mainly being confined to large-scale, commercial operations in the hands of wealthy farmers and private processing companies. The findings of this research also shed light on the shortcomings of national access and benefit-sharing legislation which has failed to secure commercial benefits for the rural poor involved in the trade. From an environmental perspective, the cultivation of buchu has contributed to the conservation of the plant in the wild through offsetting harvesting pressures experienced by wild populations, but has also contributed to the destruction of naturally occurring vegetation

    Perceptions of climate change and barriers to adaptation amongst commonage and commercial livestock farmers in the semi-arid Eastern Cape Karoo

    Climate change is expected to adversely affect agriculture in South Africa. A rise in variable and unfavourable conditions is likely to surpass the limits of current coping mechanisms of farmers, compelling them to implement more resilient adaptive measures to decrease their vulnerability to heightened risks. This study assessed the perceptions and understanding of climate change amongst livestock farmers in the semi-arid Eastern Cape Karoo and their responses to perceived changes and associated risks. Questionnaires and semi-structured interviews revealed that, while commercial famers were well informed regarding global climate change, commonage farmers had little or no understanding of the phenomenon. Although changes in local climate parameters were identified by farmers, perceived changes were mostly ascribed to the expected climate variability of the region. It was found that commonage farmers are particularly vulnerable to future climate change, whereas commercial farmers, having experience in dealing with the already variable and marginal conditions of the region and facing fewer asset and informational constraints, possess a greater adaptive capacity to respond to projected changes. Furthermore, the results demonstrated that lack of funds, lack of government support and access to information on climate change and potential adaptation strategies are the main factors constraining farmers from adaptation.Keywords: adaptive capacity, climate variability, livestock farmers, vulnerabilityAfrican Journal of Range &amp; Forage Science 2014, 31(1): 1–1

    The combination of levofloxacin and rifampin has been recommended for the treatment of staphylococcal prosthetic infection. In a rabbit model of prosthetic knee infection due to a susceptible clinical strain of Staphylococcus aureus, the combination of levofloxacin and rifampin was bactericidal, significantly reduced bacterial titers in bone compared with levels for rifampin and controls (P < 0.05), sterilized 6 of 12 animals, and prevented the selection of resistant mutants that was observed with rifampin alone, validating clinical recommendations

    Validation of semiautomatic measurement of the extent of breast tumors using contrast-enhanced magnetic resonance imaging

    The aim of this study was to assess whether the semiautomatic measurement of breast-tumor extent using contrast-enhanced magnetic resonance imaging (CE-MRI) decreases measurement variation compared with manual evaluation and increases precision with respect to tumor extent measured at histopathology. Forty-three patients who underwent breast-conserving therapy for 46 tumors were consecutively included. Extended histopathology analysis was performed on the excision specimens. Two experienced breast-MR radiologists independently manually assessed the largest diameter of the tumors at preoperative CE-MRI. Two observers used a semiautomatic segmentation technique to independently obtain the largest diameter and the volume of the tumors. Semiautomatic volumetric measurement of tumor volume was more accurately correlated with histopathology (adjusted R-square 0.84 and 0.81) compared with conventional measurement (adjusted R-square 0.56 and 0.70) and semiautomatic measurement (adjusted R-square 0.63 and 0.60) of the largest diameter of the tumor. The interobserver variability between the volumetric measurements was significantly smaller than the interobserver variability between the largest-diameter measurements of tumor extent obtained by the radiologists (P <0.001). Semiautomatic volumetric measurement of breast-tumor extent at CE-MRI provides a more accurate assessment of tumor extent with respect to histopathology and reduces measurement variation compared with manual assessment of the largest diameter.     On the feasibility of MRI-guided navigation to demarcate breast cancer for breast-conserving surgery

    The aim of this study was to investigate the feasibility of image-guided navigation approaches to demarcate breast cancer on the basis of preacquired magnetic resonance (MR) imaging in supine patient orientation. Strategies were examined to minimize the uncertainty in the instrument-tip position, based on the hypothesis that the release of instrument pressure returns the breast tissue to its predeformed state. For this purpose, four sources of uncertainty were taken into account: (1) U(ligaments): Uncertainty in the reproducibility of the internal mammary gland geometry during repeat patient setup in supine orientation; (2) U(r_breathing): Residual uncertainty in registration of the breast after compensation for breathing motion using an external marker; (3) U(reconstruction): Uncertainty in the reconstructed location of the tip of the needle using an optical image-navigation system (phantom experiments, n = 50); and (4) U(deformation): Uncertainty in displacement of breast tumors due to needle-induced tissue deformations (patients, n = 21). A Monte Carlo study was performed to establish the 95% confidence interval (CI) of the combined uncertainties. This region of uncertainty was subsequently visualized around the reconstructed needle tip as an additional navigational aid in the preacquired MR images. Validation of the system was performed in five healthy volunteers (localization of skin markers only) and in two patients. In the patients, the navigation system was used to monitor ultrasound-guided radioactive seed localization of breast cancer. Nearest distances between the needle tip and the tumor boundary in the ultrasound images were compared to those in the concurrently reconstructed MR images. Both U(reconstruction) and U(deformation) were normally distributed with 0.1 +/- 1.2 mm (mean +/- 1 SD) and 0.1 +/- 0.8 mm, respectively. Taking prior estimates for U(ligaments) (0.0 +/- 1.5 mm) and U(r_breathing) (-0.1 +/- 0.6 mm) into account, the combined impact resulted in 3.9 mm uncertainty in the position of the needle tip (95% CI) after release of pressure. The volunteer study showed a targeting accuracy comparable to that in the phantom experiments: 2.9 +/- 1.3 versus 2.7 +/- 1.1 mm, respectively. In the patient feasibility study, the deviations were within the 3.9 mm CI. Image-guided navigation to demarcate breast cancer on the basis of preacquired MR images in supine orientation appears feasible if patient breathing is tracked during the navigation procedure, positional uncertainty is visualized and pressure on the localization instrument is released prior to verification of its positio
