    A meta-analysis and systematic review of randomized controlled trials with degarelix versus gonadotropin-releasing hormone agonists for advanced prostate cancer

    Our aim was to systematically evaluate the benefits of degarelix as antagonist versus agonists of gonadotropin-releasing hormones (GnRH) for the treatment of advanced prostate cancer (PC). This comparison was performed either in terms of biochemical or oncological or safety profiles. To this end we, carried out a systematic review and meta-analysis of the literature.We selected only studies directly and prospectively analyzing the two treatments in the same population (randomized phase III studies). We followed the Preferred Reporting Items for Systematic Reviews and meta-analyses process for reporting studies.After we eliminated studies according to the exclusion criteria, 9 publications were considered relevant to this review. These articles described 5 clinical trials that were eligible for inclusion. The follow-up duration in all trials did not exceed 364 days. This meta-analysis and review comprised a total of 1719 men, 1061 randomized to degarelix versus 658 to GnRH agonists treatment for advanced PC. Oncological results were evaluated only in 1 trial (CS21:408 cases) and they were not the primary endpoints of the study. Treatment emerging adverse events were reported in 61.4% and 58.8% of patients in the degarelix and GnRH agonists group, respectively (odds ratio, OR = 1.17; 95% confidence interval, 95% CI: 0.78-1.77, P > 0.1). Treatment related severe cardiovascular side effects were reported (trial CS21-30-35) in 1.6% and 3.6% of patients in the degarelix and GnRH agonists group, respectively (OR = 0.55, 95% CI: 0.26-1.14, P > 0.1).Our analysis evidences relevant limitations in particular for the comparative evaluation of the efficacy and the oncological results related to degarelix

    A rare case report of hypertrophic cardiomyopathy induced by catecholamine-producing tumor

    RATIONALE: Catecholamine-producing tumors are rare, occurring in less than 0.2% of patients with hypertension, but can have relevant cardiovascular morbidity and mortality. PATIENT CONCERNS: A 37-year-old woman presented with a history of dyspnea, chest pain, palpitations, and paroxysmal hypertension. Electrocardiogram, echocardiogram, and cardiac magnetic resonance showed severe LVH with a prevalent involvement of the anterior portion of interventricular septum. Endomyocardial biopsy found severe hypertrophy with disarray of cardiomyocytes and ultrastructural evidence of contraction and necrosis of myocytes. Hormone investigations revealed high values of 24-hours urinary metanephrines. Abdominal computed tomography (CT) showed an enlarged left adrenal gland with a strong uptake of I-metaiodobenzylguanidine at scintigraphy scan. INTERVENTIONS:Thus, the adrenal tumor was surgically removed. OUTCOMES: At follow-up examination, the patient's metanephrines levels were normalized and the transthoracic echocardiogram showed a reduction of LVH.     Removal of veterinary antibiotics in swine manure wastewater using microalgae-bacteria consortia in a pilot scale photobioreactor

    Producción CientíficaScenedesmus almeriensis microalgae-bacteria consortia were evaluated for the removal of a mixture of tetracycline (TET), ciprofloxacin (CIP), and sulfadiazine (SDZ) from the real liquid fraction of pig slurry in a pilot scale photobioreactor. After 15 days of operation, the reactor was spiked with a mixture of /L of each antibiotic. The experiment ran for 20 additional days. From the liquid phase, antibiotic removal were 77 ± 5 %, 90 ± 14 %, and 60 ± 27 % for TET, CIP, and SDZ, respectively. The antibiotics found in the solid phase were 979 ± 382 ng/g for TET and 192 ± 69 ng/g for SDZ; CIP was not detected in the biomass. The parameters analyzed before and after antibiotic addition showed that the antibiotics did not have a negative effect on the reactor biomass. The removal efficiencies of the analyzed parameters were 64.6 ± 0.6 % for TOC, 56.9 ± 0.6 % for IC, 63.9 ± 0.6 % for TN, 88.6 ± 0.9 % for N-NH, 64.9 ± 0.6 % for N-NO, and 30.1 ± 0.3 % for P-PO. This study demonstrated the good performance of microalgae-based technology for swine manure wastewater treatment, not only in terms of organic matter and nutrient removal, but also regarding the removal of antibiotics. The mass balance analysis of the entire process is presented.     Optimisation of Scenedesmus almeriensis production using pig slurry as the sole nutrient source

    Pig slurry was used as the sole nutrient source to produce biomass of the microalga Scenedesmus almeriensis using laboratory-scale bubble column photobioreactors. No differences in terms of biomass productivity were observed between fresh and digested pig slurry. The optimum dilution rate to enable the processing of the largest amount of pig slurry per litre was 5%, which led to a biomass productivity rate of 0.68 g·L-1·day-1, comparable to that of the standard growth medium formulated using pure chemicals (0.70 g·L-1·day-1). The inlet N-NH4+ concentration was approximately 180 mg·L-1, with inhibitory effects being observed at concentrations higher than 200 mg·L-1. The N-NH4+, N-NO3-, P-PO43-, and chemical oxygen demand removal rates were 34.1, 0.5, 2.1, and 519.3 mg·L-1·day-1. The observed turbidity of the media did not affect microalgal growth at the studied dilutions. Ozonation effectively reduced the chemical oxygen demand concentration of the media, but no effect on microalgal growth was observed. Overall, pig slurry was an effective nutrient source for the mass production of S. almeriensis, and this strain was shown to be robust with a N-NH4+ tolerance up to 200 mg·L-1 and the potential use in the bioremediation of agro-industrial wastes

    Objectives: To evaluate MRI diagnostic performance in detecting clinically significant prostate cancer (csPCa) in peripheral-zone PI-RADS 4 lesions, comparing those with clearly restricted diffusion (DWI-score 4), and those with equivocal diffusion pattern (DWI-score 3) and positive dynamic contrast-enhanced (DCE) MRI. Methods: This observational prospective study enrolled 389 men referred to MRI and, if positive (PI-RADS 3 with PSA-density [PSAD] ≥ 0.15 ng/mL/mL, 4 and 5), to MRI-directed biopsy. Lesions with DWI-score 3 and positive DCE were classified as "PI-RADS 3up," instead of PI-RADS 4. Univariable and multivariable analyses were implemented to determine features correlated to csPCa detection. Results: Prevalence of csPCa was 14.5% and 53.3% in PI-RADS categories 3up and 4, respectively (p < 0.001). MRI showed a sensitivity of 100.0%, specificity 40.9%, PPV 46.5%, NPV 100.0%, and accuracy 60.9% for csPCa detection. Modifying the threshold to consider MRI positive and to indicate biopsy (same as previously described, but PI-RADS 3up only when associated with elevated PSAD), the sensitivity changed to 93.9%, specificity 57.2%, PPV 53.0%, NPV 94.8%, and accuracy 69.7%. Age (p < 0.001), PSAD (p < 0.001), positive DWI (p < 0.001), and PI-RADS score (p = 0.04) resulted in independent predictors of csPCa. Conclusions: Most cases of PI-RADS 3up were false-positives, suggesting that upgrading peripheral lesions with DWI-score 3 to PI-RADS 4 because of positive DCE has a detrimental effect on MRI accuracy, decreasing the true prevalence of csPCa in the PI-RADS 4 category. PI-RADS 3up should not be upgraded and directed to biopsy only if associated with increased PSAD. Key points: • As per PI-RADS v2.1 recommendations, in case of a peripheral zone lesion with equivocal diffusion-weighted imaging (DWI score 3), but positive dynamic contrast-enhanced (DCE) MRI, the overall PI-RADS score should be upgraded to 4. • The current PI-RADS recommendation of upgrading PI-RADS 3 lesions of the peripheral zone to PI-RADS 4 because of positive DCE decreased clinically significant prostate cancer detection rate in our series. • According to our results, the most accurate threshold for setting indication to prostate biopsy is PI-RADS 3 or PI-RADS 3 with positive DCE both associated with increased PSA density

    Production of Scenedesmus almeriensis Using Pilot-Scale Raceway Reactors Located inside a Greenhouse

    The microalga S. almeriensis was produced in spring and summer using 80 m2 raceway reactors located inside a greenhouse in Almería, Spain. This microalga was selected because it is a fast-growing and robust strain with potential applications in the production of functional foods and feeds or as a high-value agricultural biostimulant. Overall, the biomass productivity obtained in summer was 24.9 ± 0.9 g·m−2·day−1. This value was higher than that obtained in spring, with an average value of 21.4 ± 1.3 g·m−2·day−1 (p < 0.05). The Fv/Fm value of the cultures at the stationary phase was comparable and around 0.6, which is the optimum of this strain. No major differences in the macromolecular composition of the biomass were observed between seasons, with an average protein, lipid, ash, and carbohydrate content of 37.9, 4.6, 10.8, and 46.7%, respectively. The data reported herein were used to validate a previously described model. The differences between the experimental and the predicted biomass productivities were below 5% in spring and 8% in summer, although a larger dataset is needed to validate the model. Overall, results supported the robustness of the selected strain and its utilisation in different industrial sectors

    Characterisation of the volatile profile of microalgae and cyanobacteria using solid-phase microextraction followed by gas chromatography coupled to mass spectrometry

    [EN] Microalgae and microalgae-derived ingredients are one of the top trends in the food industry. However, consumers' acceptance and purchase intention of a product will be largely affected by odour and flavour. Surprisingly, the scientific literature present a very limited number of studies on the volatile composition of microalgae and cyanobacteria. In order to fill the gap, the main objective of the present study was to elucidate the volatile composition of seven microalgal and cyanobacterial strains from marine and freshwaters, with interest for the food industry while establishing its potential impact in odour. Among the seven selected strains, Arthrospira platensis showed the highest abundance and chemical diversity of volatile organic compounds (VOCs). Aldehydes, ketones, and alcohols were the families with the highest diversity of individual compounds, except in Arthrospira platensis and Scenedesmus almeriensis that showed a profile dominated by branched hydrocarbons. Marine strains presented a higher abundance of sulfur compounds than freshwater strains, while the ketones individual profile seemed to be more related to the taxonomical domain.     Microalgae Derived Astaxanthin: Research and Consumer Trends and Industrial Use as Food

    Astaxanthin is a high-value carotenoid currently being produced by chemical synthesis and by extraction from the biomass of the microalga Haematococcus pluvialis. Other microalgae, such as Chlorella zofingiensis, have the potential for being used as sources of astaxanthin. The differences between the synthetic and the microalgae derived astaxanthin are notorious: not only their production and price but also their uses and bioactivity. Microalgae derived astaxanthin is being used as a pigment in food and feed or aquafeed production and also in cosmetic and pharmaceutical products. Several health-promoting properties have been attributed to astaxanthin, and these were summarized in the current review paper. Most of these properties are attributed to the high antioxidant capacity of this molecule, much higher than that of other known natural compounds. The aim of this review is to consider the main challenges and opportunities of microalgae derived products, such as astaxanthin as food. Moreover, the current study includes a bibliometric analysis that summarizes the current research trends related to astaxanthin. Moreover, the potential utilization of microalgae other than H. pluvialis as sources of astaxanthin as well as the health-promoting properties of this valuable compound will be discussed

    Role of Microalgae in the Recovery of Nutrients from Pig Manure

    Animal production inevitably causes the emission of greenhouse gases and the generation of large amounts of slurry, both representing a serious environmental problem. Photosynthetic microorganisms such as microalgae and cyanobacteria have been proposed as alternative strategies to bioremediate agricultural waste while consuming carbon dioxide and producing valuable biomass. The current study assessed the potential of the microalga Scenedesmus sp. to remove nutrients from piggery wastewater (PWW) and the influence of the microalga on the microbial consortia. Maximum N-NH4+ consumption was 55.3 ± 3.7 mg·L−1·day−1 while P-PO43− removal rates were in the range 0.1–1.9 mg·L−1·day−1. N-NH4+ removal was partially caused by the action of nitrifying bacteria, which led to the production of N-NO3−. N-NO3− production values where lower when microalgae were more active. This work demonstrated that the photosynthetic activity of microalgae allows us to increase nutrient removal rates from PWW and to reduce the coliform bacterial load of the effluent, minimising both their environmental impact and health risks. Microalgae assimilated part of the N-NH4+ present in the media to produce biomass and did not to convert it into N-NO3− as in traditional processes
