    Limited stage follicular lymphoma: Current role of radiation therapy

    Radiation therapy (RT) alone has been considered for a long time as the standard therapeutic option for limited stage FL, due to its high efficacy in terms of local disease control with a quite significant proportion of “cured” patients (without further relapses at 10–15 years). Multiple therapeutic choices are currently accepted for the management of early stage FL at diagnosis, and better staging procedures as well as better systemic therapy partially modified the role of RT in this setting. RT has also changed in terms of prescribed dose as well as treatment volumes. In this review, we present and discuss the current role of RT for limited stage FL in light of the historical data and the modern RT concepts along with the possible combination with systemic therapy

    The role of radiotherapy is well established in combined modality programs for early stage Hodgkin's lymphoma, but still debated with regards to late toxicity issues. Modern radiotherapy prescribing attitudes include lower doses and smaller fields, together with the implementation of sophisticated and dedicated delivery techniques. Aim of this review is to briefly discuss the current role of radiotherapy in this field and the potential future developments. Major trials conducted in recent years in early stage Hodgkin's lymphoma are critically reviewed and discussed with a focus on radiotherapy-related issues and with an attention to current treatment options by a “young” radiation oncologists’ perspective

    Radiation-induced breast angiosarcoma: report of two patients after accelerated partial breast irradiation (APBI) and review of the literature

    BACKGROUND: Angiosarcoma may rarely complicate radiotherapy of breast cancer. This so-called radiation-induced angiosarcoma (RIAS) occurs in less than 0.3% of patients that underwent breast conservation surgeries, usually years after completion of radiotherapy. CASE PRESENTATION: we introduce two cases of invasive ductal carcinoma who underwent lumpectomy and accelerated partial breast irradiation (APBI) as an alternative protocol to whole breast irradiation (WBI). They received adjuvant partial breast radiotherapy on tumor cavity for a total dose of 38.5 Gy in 10 fractions in 5 days using 3D-external-beam RT. In both cases, RIAS occurred eight years after radiotherapy, in the sub-cicatricial area in one patient and outside the irradiated area in the other one. They both underwent radical surgery and chemotherapy was performed in one patient. DISCUSSION: The underlying mechanism for development of RIAS is not well known, but its incidence seems to be increasing. RIAS after partial breast irradiation is very rare and has been reported in two cases so far. As it may be suggested in case 2, it is still a matter of debate if the risk of radiation-induced sarcoma is radiation-dose dependent. Although mastectomy is considered as a standard treatment, choice of treatment should be made according to the patient’s specifications. CONCLUSION: There are very few studies in the literature that report RIAS after APBI. Present study is the only one reporting two cases after the external 3D technique APBI. Prognosis of RIAS remains poor. Only a careful evaluation in a multidisciplinary context can offer to the patients the best result in terms of local control and surviva

    Lattice radiation therapy in clinical practice: a systematic review

    Purpose: Lattice radiation therapy (LRT) is an innovative type of spatially fractionated radiation therapy. It aims to increase large tumors control probability by administering ablative doses without an increased toxicity. Considering the rising number of positive clinical experiences, the objective of this work is to evaluate LRT safety and efficacy. Method: Reports about LRT clinical experience were identified with a systematic review conducted on four different databases (namely, Medline, Embase, Scopus, and Cochrane Library) through the August 2022. Only LRT clinical reports published in English and with the access to the full manuscript text were considered as eligible. The 2020 update version PRISMA statement was followed. Results: Data extraction was performed from 12 eligible records encompassing 7 case reports, 1 case series, and 4 clinical studies. 81 patients (84 lesions) with a large lesion ranging from 63.2 cc to 3713.5 cc were subjected to exclusive, hybrid, and metabolism guided LRT. Excluding two very severe toxicity with a questionable relation with LRT, available clinical experience seem to confirm LRT safety. When a complete response was not achieved 3-6 months after LRT, a median lesion reduction approximately ≥50 % was registered. Conclusion: This systematic review appear to suggest LRT safety, especially for exclusive LRT. The very low level of evidence and the studies heterogeneity preclude drawing definitive conclusions on LRT efficacy, even though an interesting trend in terms of lesions reduction has been described

    Post-ABVD/pre-radiotherapy 18F-FDG-PET provides additional prognostic information for early-stage Hodgkin lymphoma: A retrospective analysis on 165 patients

    OBJECTIVE: To evaluate the prognostic role of both interim fluorine-18 fludeoxyglucose positron emission tomography (i-(18)F-FDG-PET) and end-of-chemotherapy fluorine-18 fludeoxyglucose positron emission tomography (eoc-(18)F-FDG-PET) in patients with early-stage Hodgkin lymphoma (HL). METHODS: We screened 257 patients with early-stage HL treated with combined modality therapy between March 2003 and July 2011. All were staged using fluorine-18 fludeoxyglucose positron emission tomography ((18)F-FDG-PET) before chemotherapy and after two doxorubicin, bleomycin, vinblastine and dacarbazine cycles (i-(18)F-FDG-PET); 165 patients were also evaluated by (18)F-FDG-PET at the end of chemotherapy (eoc-(18)F-FDG-PET). RESULTS: After revision, 85% of patients were negative for i-(18)F-FDG-PET and 15% were positive. After eoc-(18)F-FDG-PET revision, 23 patients had a positive scan. The median follow-up was 56 months. The 5-year overall survival (OS) and progression-free survival (PFS) for the whole cohort were 97.5% and 95.6%, respectively. For i-(18)F-FDG-PET-negative and i-(18)F-FDG-PET-positive patients, the 5-year PFS rates were 98% and 84%, respectively; for eoc-(18)F-FDG-PET-negative and eoc-(18)F-FDG-PET-positive patients, the 5-year PFS rates were 97% and 78%, respectively. Combining the i-(18)F-FDG-PET and eoc-(18)F-FDG-PET results, the 5-year PFS were 97%, 100% and 82% in negative/negative, positive/negative and positive/positive groups, respectively. The 5-year OS rates were 98% and 83% in eoc-(18)F-FDG-PET-negative and eoc-(18)F-FDG-PET-positive patients, respectively; the 5-year OS was 98%, 100% and 83% in negative/negative, positive/negative and positive/positive groups, respectively. CONCLUSION: This study provides additional information on the prognostic role of i-(18)F-FDG-PET and eoc-(18)F-FDG-PET in early-stage HL. As data are accumulating and the clinical scenario is rapidly evolving, we might need to rethink the use of (18)F-FDG-PET as a prognostic marker for early-stage HL in the near future. ADVANCES IN KNOWLEDGE: This study provides additional information on the prognostic role of i-(18)F-FDG-PET and eoc-(18)F-FDG-PET in early-stage HL. On the basis of the present data, we may suggest to use eoc-(18)F-FDG-PET as a strong prognostic marker, especially for patients with i-(18)F-FDG-PET positivity

    Treatment Outcome of metastatic lesions from renal cell carcinoma underGoing Extra-cranial stereotactic body radioTHERapy: The together retrospective study

    Abstract Objectives stereotactic body radiation therapy (SBRT) use has increased overtime for the management of metastatic renal cell carcinoma (mRCC) patients, with a likely good control of irradiated lesions. We planned a retrospective multicenter Italian study, with the aim of investigating the outcome of treatment with SBRT for non-brain secondary lesions in mRCC patients. Methods all consecutive metastatic non-brain lesions from mRCC that underwent SBRT at nine Italian institutions from January 2015 to June 2017 were considered. The primary endpoint of the study was the lesion-PFS, calculated from SBRT initiation to the local progression of the irradiated lesion. Results 57 extracranial metastatic lesions from 48 patients with primary mRCC were treated with SBRT. At the median follow-up of 26.4 months, the median lesion-PFS was not reached (43 censored); 72.4% of lesions were progression-free at 40 months, with significantly better lesion-PFS for small metastatic lesions ( Conclusions consistently with the previous literature, our findings support the use of SBRT in selected mRCC patients
