    Bacterial community analysis in upflow multilayer anaerobic reactor (UMAR) treating high-solids organic wastes

    A novel anaerobic digestion configuration, the upflow multi-layer anaerobic reactor (UMAR), was developed to treat high-solids organic wastes. The UMAR was hypothesized to form multi-layer along depth due to the upflow plug flow; use of a recirculation system and a rotating distributor and baffles aimed to assist treating high-solids influent. The chemical oxygen demand (COD) removal efficiency and methane (CH4) production rate were 89% and 2.10 L CH4/L/day, respectively, at the peak influent COD concentration (110.4 g/L) and organic loading rate (7.5 g COD/L/day). The 454 pyrosequencing results clearly indicated heterogeneous distribution of bacterial communities at different vertical locations (upper, middle, and bottom) of the UMAR. Firmicutes was the dominant (>70%) phylum at the middle and bottom parts, while Deltaproteobacteria and Chloroflexi were only found in the upper part. Potential functions of the bacteria were discussed to speculate on their roles in the anaerobic performance of the UMAR system

    The Genome Sequence of 'Mycobacterium massiliense' Strain CIP 108297 Suggests the Independent Taxonomic Status of the Mycobacterium abscessus Complex at the Subspecies Level

    Members of the Mycabacterium abscessus complex are rapidly growing mycobacteria that are emerging as human pathogens. The M. abscassus complex was previously composed of three species, namely M. abscessus sensu strict, 'M. massiliense', and M. bolletii', In 2011, 'M. massiliense' and 'M. bolletre' were united and reclassified as a single subspecies within M. abscessus: M. abscessus subsp. bolletii. However, the placement of 'M. massiliense' Within the boundary of M. abscessus subsp. balletii remains highly controversial with regard to clinical aspects. In this study, we revisited the taxonomic status of members of the M. abscessus complex based on comparative analysis of he whole-genome sequences of 53 strains, The genome sequence of the previous type strain of 'Mycobacterium massiliense' (CIP 108297) was determined using next-generation sequencing. The genome tree based on average nucleotide identity (AN I) values supported the differentiation of M. bolletii' and M. massiliense' at the subspecies level. The genome tree also clearly illustrated that 'M. bolletil' and 'M. massiliense' form a distinct phylogenetic clade within the radiation of the M. abscessus complex. The genomic distances observed in this study suggest that the current M. abscessus subsp. bolletii taxon should be divided into two subspecies, M. abscessus subsp. massiliense subsp. nov. and M. abscessus subsp. bolletii, to correspondingly accommodate the previously known 'M. assiliense' and 'M. bolletii' strains.

    Predictors and patterns of problematic Internet game use using a decision tree model

    Background and aims Problematic Internet game use is an important social issue that increases social expenditures for both individuals and nations. This study identified predictors and patterns of problematic Internet game use. Methods Data were collected from online surveys between November 26 and December 26, 2014. We identified 3,881 Internet game users from a total of 5,003 respondents. A total of 511 participants were assigned to the problematic Internet game user group according to the Diagnostic and Statistical Manual of Mental Disorders Internet gaming disorder criteria. From the remaining 3,370 participants, we used propensity score matching to develop a normal comparison group of 511 participants. In all, 1,022 participants were analyzed using the chi-square automatic interaction detector (CHAID) algorithm. Results According to the CHAID algorithm, six important predictors were found: gaming costs (50%), average weekday gaming time (23%), offline Internet gaming community meeting attendance (13%), average weekend and holiday gaming time (7%), marital status (4%), and self-perceptions of addiction to Internet game use (3%). In addition, three patterns out of six classification rules were explored: cost-consuming, socializing, and solitary gamers. Conclusion This study provides direction for future work on the screening of problematic Internet game use in adults

    Integration of Genetic Programming and TABU Search Mechanism for Automatic Detection of Magnetic Resonance Imaging in Cervical Spondylosis

    Cervical spondylosis is a kind of degenerative disease which not only occurs in elder patients. The age distribution of patients is unfortunately decreasing gradually. Magnetic Resonance Imaging (MRI) is the best tool to confirm the cervical spondylosis severity but it requires radiologist to spend a lot of time for image check and interpretation. In this study, we proposed a prediction model to evaluate the cervical spine condition of patients by using MRI data. Furthermore, to ensure the computing efficiency of the proposed model, we adopted a heuristic programming, genetic programming (GP), to build the core of refereeing engine by combining the TABU search (TS) with the evolutionary GP. Finally, to validate the accuracy of the proposed model, we implemented experiments and compared our prediction results with radiologistā€™s diagnosis to the same MRI image. The experiment found that using clinical indicators to optimize the TABU list in GP+TABU got better fitness than the other two methods and the accuracy rate of our proposed model can achieve 88% on average. We expected the proposed model can help radiologists reduce the interpretation effort and improve the relationship between doctors and patients

    Endovascular Treatment for Intracranial Aneurysms: A Nationwide Survey in Korea

    Purpose In Korea, endovascular treatment (EVT) for intracranial aneurysms (IAs) has increased steadily. We conducted a nationwide survey to evaluate the current status of EVT for IAs and to identify treatment preference in the real world.Materials and Methods A Google online survey was distributed to representative clinicians at hospitals treating IAs, where members of Korean Society of Interventional Neuroradiology (KSIN). The data was collected from October 2017 to December 2017. The responding hospitals were divided into 2 groups (tertiary and non-tertiary hospitals). And variable factors involved in decision making for treatment were evaluated. Results In total, 73 hospitals (tertiary: 37, non-tertiary: 36) responded to the survey. Most hospitals that responded had over 100 cases of diagnostic angiography (93%) and over 50 cases of EVT for IAs (74%) performed in 2016. The proportion of EVT for ruptured aneurysms in non-tertiary hospitals was significantly higher than tertiary hospitals (49% vs. 9%). The proportion of EVT for unruptured aneurysms at non-tertiary hospitals was significantly higher than tertiary hospitals (66% vs. 44%). Most physicians tended to make decision for treatment on location, shape, and size of unruptured IAs and patientsā€™ age, more than the results from previous clinical trials for unruptured IAs. Although EVT was preferred for older patients (age >70) with unruptured IAs (99%), surgical clipping was still considered as the first treatment of choice for younger patients (age 30 to 50 years) at considerable rates (56%). Over two-thirds of respondents preferred surgical clipping for middle cerebral artery aneurysms, while EVT was preferred initially at other locations. Conclusion This nationwide survey showed that EVT is considered as the first treatment modality for IAs and there is a discrepancy between current guidelines and real-world practice for decision making of treatment options

    Factors associated with stroke in patients with paroxysmal atrial fibrillation beyond CHADS2 score

    Background: This study was conducted to investigate factors associated with stroke in paĀ­tients with paroxysmal atrial fibrillation (PAF) beyond CHADS2 score in terms of left ventricular (LV) diastolic function or left atrial (LA) function. Methods: One hundred and sixty-one patients with PAF and age less than 75 (mean age 61 Ā± 10; 69 male) who underwent transthoracic echocardiography were investigated. Patients were divided into two groups according to the stroke status (group 1 ā€” no stroke vs. group 2 ā€” presence of stroke). Baseline echocardiographic parameters and LA segmental (4 segments: basal septal, lateral, inferior, and anterior) strain rate (SR) during normal sinus rhythm were analyzed. Results: CHAD score (except S2) was similar between the two groups (0.6 Ā± 0.7 vs. 0.9 Ā± 0.7, p = 0.125). Patients with stroke had slightly lower body mass index (24.5 Ā± 2.7 vs. 23.4 Ā± Ā± 2.4, p = 0.052). Echocardiographic parameters did not show any differences in both systolic and diastolic functions between the two groups, however elevated E/Eā€™ ratio was noted (9.5 Ā± Ā± 3.8 vs. 11.6 Ā± 3.9, p = 0.010) due to higher E velocity (63.5 Ā± 15.9 vs. 70.9 Ā± 16.0 cm/s, p = 0.046). In the analysis of LA SR, there are no differences of SR among the 4 segments. However, standard deviations (SD) of time to peak SR (SD of tA-SR) of the 4 segments were higher in patients with stroke (10.9 Ā± 9.9 vs. 22.1 Ā± 18.1 ms, p = 0.009) which indicates dyssynchronous contraction of LA. In multivariate analysis, SD of tA-SR (OR 1.074, CI 1.024ā€“1.128, p = 0.004) and elevated E/Eā€™ (OR 1.189, CI 1.006ā€“1.406, p = 0.048) were independently associated with stroke in patients with PAF. Conclusions: Elevated E velocity, E/Eā€™ and SD of tA-SR were associated with occurrence of stroke in patients with PAF even with similar CHAD scores. Increased SD of tA-SR and E/Eā€™ were independently associated with stroke in patients with PAF.

    Nanofibrous insulin/vildagliptin core-shell PLGA scaffold promotes diabetic wound healing

    Introduction: Slow wound repair in diabetes is a serious adverse event that often results in loss of a limb or disability. An advanced and encouraging vehicle is wanted to enhance clinically applicable diabetic wound care. Nanofibrous insulin/vildagliptin core-shell biodegradable poly (lactic-co-glycolic acid) (PLGA) scaffolds to prolong the effective drug delivery of vildagliptin and insulin for the repair of diabetic wounds were prepared.Methods: To fabricate core-shell nanofibrous membranes, vildagliptin mixture with PLGA, and insulin solution were pumped via separate pumps into two differently sized capillary tubes that were coaxially electrospun.Results and Discussion: Nanofibrous core-shell scaffolds slowly released effective vildagliptin and insulin over 2Ā weeks in vitro migration assay and in vivo wound-healing models. Water contact angle (68.3 Ā± 8.5Ā° vs. 121.4 Ā± 2.0Ā°, p = 0.006) and peaked water absorbent capacity (376% Ā± 9% vs. 283% Ā± 24%, p = 0.003) of the insulin/vildagliptin core-shell nanofibrous membranes remarkably exceeded those of a control group. The insulin/vildagliptin-loaded core-shell nanofibers improved endothelial progenitor cells migration in vitro (762 Ā± 77Ā cells/mm2 vs. 424.4 Ā± 23Ā cells/mm2, p < 0.001), reduced the Ī±-smooth muscle actin content in vivo (0.72 Ā± 0.23 vs. 2.07 Ā± 0.37, p < 0.001), and increased diabetic would recovery (1.9 Ā± 0.3Ā mm2 vs. 8.0 Ā± 1.4Ā mm2, p = 0.002). Core-shell insulin/vildagliptin-loaded nanofibers extend the drug delivery of insulin and vildagliptin and accelerate the repair of wounds associated with diabetes
