    Background: Saddle thrombosis is less frequently detected in carotid arteries than in peripheral arterial embolism. The clot and the distal vessel patency have to be promptly recognized in these cases, because if the carotid vessel is open distally, chances may arise for successful emergent surgical procedures to remove the thrombus. At conventional static imaging, mobile floating thrombi may be difficult to differentiate from thrombosis on carotid complicated lesions of atherosclerotic origin. High-resolution ultrasound (US), with its unique capability of real-time imaging, adds fundamental data for interpretation of the findings. Methods: Carotid ultrasound has been performed in acute stroke patients with high-resolution probes. Real-time clips are analyzed and imaging is presented. Results: Saddle carotid bifurcation thrombosis of cardiac origin has been identified in 2 patients with acute homolateral ischemic stroke, with prompt successful surgical removal in one case. Moreover, an example of a thrombus attached on the ruptured surface of a complicated atherosclerotic plaque in an acute symptomatic stroke patient that was successfully operated in emergency is presented. Conclusions: Early high-resolution ultrasound with real-time imaging can easily identify peculiar characteristics of carotid vulnerable diseases in acute stroke phase. Different clinical implications result from the early identification of these different conditions, modifying the therapeutical strategies. © 2012 Elsevier GmbH

    Effect of different management protocols for grouping does on aggression and dominance hierarchies

    The study aimed at evaluating different management protocols of grouping does in regard to aggressive interactions and the establishment of a hierarchy under commercial conditions. Fifty-seven multiparous rabbit does of the Hycole hybrid maternal line were randomly distributed into three different management protocols (MP) for a total of 5 consecutive trials: MP12 with grouping 12 days after parturition, MP18 after 18 days and MP22 after 22 days. Video recordings were made during the first 24 h after grouping and after 6 and 10 days, and used to score aggressive interactions of the animals with exception of treatment 3 where day 10 was omitted because it was after the weaning of the kits. For MP12 and MP18 the frequency of different categories of aggressive interactions were significantly higher on the day of grouping than afterwards, with a strong decrease on day 6 (P < 0.02), but almost no change was found between days 6 and 10. This was especially true for biting, with a high frequency of occurrence just after grouping and a subsequent decrease on day 6 under all management schedules (P = 0.005). The change in the number of aggressive interactions between 6 and 10 days after grouping was not significantly different between MPs. However, the time point of 6 days after grouping seemed to be fundamental in reducing the number of aggressive events. Although a longer separation for 18 and 22 days after parturition did not reduce the total number of aggressive interactions, MP18 and MP22 showed more mild aggressive interactions with respect to MP12 on the day of regrouping. Moreover, when comparing MPs concerning the development of hierarchy, hierarchies appeared very stable independently of the length of grouping (P < 0.01)

    Solid biofuels production from agricultural residues and processing by-products by means of torrefaction treatment: the case of sunflower chain

    The high heterogeneity of some residual biomasses makes rather difficult their energy use. Their standardisation is going to be a key aspect to get good quality biofuels from those residues. Torrefaction is an interesting process to improve the physical and chemical properties of lignocellulosic biomasses and to achieve standardisation. In the present study torrefaction has been employed on residues and by-products deriving from sunflower production chain, in particular sunflower stalks, husks and oil press cake. The thermal behaviour of these materials has been studied at first by thermogravimetric analysis in order to identify torrefaction temperatures range. Afterwards, different residence time and torrefaction temperatures have been tested in a bench top torrefaction reactor. Analyses of raw and torrefied materials have been carried out to assess the influence of the treatment. As a consequence of torrefaction, the carbon and ash contents increase while the volatilisation range reduces making the material more stable and standardised. Mass yield, energy yield and energy densification reach values of about 60%, 80% and 1.33 for sunflower stalks and 64%, 85% and 1.33 for sunflower oil press cake respectively. As highlighted by the results, torrefaction is more interesting for sunflower stalks than oil cake and husks due to their different original characteristics. Untreated oil press cake and husks, in fact, already show a good high heating value and, for this reason, their torrefaction should be mild to avoid an excessive ash concentration. On the contrary, for sunflower stalks the treatment is more useful and could be more severe

    The increase in the demand for energy supply during the past few decades has brought and will bring to a growth in the utilisation of renewable resources, in particular of solid biomasses. Considering the variability in the properties of biomass and the globalisation of the timber market, a chemical and physical characterisation is essential to determine the biomass quality. The specific international standards on solid biofuels (ISO 17225 series) describe proper specification and classification of wood chip and pellet, to ensure appropriate quality. Moreover, standard requires information about origin and source of the biomass, normally only to be declared by the producers. In order to fulfill the requirements for the biomass quality, the origin and the source should be assessed, even if currently is hard to determine, in particular on milled or densified biomass. Infrared spectroscopy can provide information on the biomass at the chemical level, directly linked also to its origin and source. This technique is fast and not destructive thus suitable also for online monitoring along the biofuel production chain. In this study, 60 samples belonging to 8 different species were collected and related spectra were acquired using a Fourier transform infrared (IR) spectrometer equipped with a module for solid analysis and analysed by principal component analysis. The results obtained show that the method is very efficient in the identification between coniferous and deciduous wood (99% confidence level) and good results were obtained in the recognition of coniferous/deciduous mixtures, too. Nevertheless, some clear differences have been also noted among intra-class grouping, but additional tests should be carried out. This technique can provide useful information to solid biofuel stakeholders about wood quality and origin, important especially for sustainability issues. Further work will be oriented to the development of IR methodologies for the fast measurement of other important biomass parameters (e.g., ash content, high calorific value, nitrogen content, etc.)

    Enterococcal meningitis caused by Enterococcus casseliflavus. First case report

    BACKGROUND: Enterococcal meningitis is an uncommon disease usually caused by Enterococcus faecalis and Enterococcus faecium and is associated with a high mortality rate. Enterococcus casseliflavus has been implicated in a wide variety of infections in humans, but never in meningitis. CASE PRESENTATION: A 77-year-old Italian female presented for evaluation of fever, stupor, diarrhea and vomiting of 3 days duration. There was no history of head injury nor of previous surgical procedures. She had been suffering from rheumatoid arthritis for 30 years, for which she was being treated with steroids and methotrexate. On admission, she was febrile, alert but not oriented to time and place. Her neck was stiff, and she had a positive Kernig's sign. The patient's cerebrospinal fluid was opalescent with a glucose concentration of 14 mg/dl, a protein level of 472 mg/dl, and a white cell count of 200/μL with 95% polymorphonuclear leukocytes and 5% lymphocytes. Gram staining of CSF revealed no organisms, culture yielded E. casseliflavus. The patient was successfully treated with meropenem and ampicillin-sulbactam. CONCLUSIONS: E. casseliflavus can be inserted among the etiologic agents of meningitis. Awareness of infection of central nervous system with Enterococcus species that possess an intrinsic vancomycin resistance should be increased

    GEN-O-MA project: an Italian network studying clinical course and pathogenic pathways of moyamoya disease—study protocol and preliminary results

    Background: GENetics of mOyaMoyA (GEN-O-MA) project is a multicenter observational study implemented in Italy aimed at creating a network of centers involved in moyamoya angiopathy (MA) care and research and at collecting a large series and bio-repository of MA patients, finally aimed at describing the disease phenotype and clinical course as well as at identifying biological or cellular markers for disease progression. The present paper resumes the most important study methodological issues and preliminary results. Methods: Nineteen centers are participating to the study. Patients with both bilateral and unilateral radiologically defined MA are included in the study. For each patient, detailed demographic and clinical as well as neuroimaging data are being collected. When available, biological samples (blood, DNA, CSF, middle cerebral artery samples) are being also collected for biological and cellular studies. Results: Ninety-eight patients (age of onset mean ± SD 35.5 ± 19.6 years; 68.4% females) have been collected so far. 65.3% of patients presented ischemic (50%) and haemorrhagic (15.3%) stroke. A higher female predominance concomitantly with a similar age of onset and clinical features to what was reported in previous studies on Western patients has been confirmed. Conclusion: An accurate and detailed clinical and neuroimaging classification represents the best strategy to provide the characterization of the disease phenotype and clinical course. The collection of a large number of biological samples will permit the identification of biological markers and genetic factors associated with the disease susceptibility in Italy

    Mycotoxin mixtures in food and feed: holistic, innovative, flexible risk assessment modelling approach: MYCHIF

    Mycotoxins are toxic compounds mainly produced by fungi of the genera Aspergillus, Penicillium and Fusarium. They are present, often as mixtures, in many feed and food commodities including cereals, fruits and vegetables. Their ubiquitous presence represents a major challenge to the health and well being of humans and animals. Hundreds of compounds are listed as possible mycotoxins occurring in raw and processed materials destined for human food and animal feed. In this study, mycotoxins of major toxicological relevance to humans and target animal species were investigated in a range of crops of interest (and their derived products). Extensive Literature Searches (ELSs) were undertaken for data collection on: (i) ecology and interaction with host plants of mycotoxin producing fungi, mycotoxin production, recent developments in mitigation actions of mycotoxins in crop chains (maize, small grains, rice, sorghum, grapes, spices and nuts), (ii) analytical methods for native, modified and co-occurring mycotoxins (iii) toxicity, toxicokinetics, toxicodynamics and biomarkers relevant to humans and animals (poultry, suidae (pig, wild boar), bovidae (sheep, goat, cow, buffalo), rodents (rats, mice) and others (horses, dogs), (iv) modelling approaches and key reference values for exposure, hazard and risk modelling. Comprehensive databases were created using EFSA templates and were stored in the MYCHIF platform. A range of approaches were implemented to explore the modelling of external and internal exposure as well as dose-response of mycotoxins in chicken and pigs. In vitro toxicokinetic and in vivo toxicity databases were exploited, both for single compounds and mixtures. However, large data gaps were identified particularly with regards to absence of common statistical and study designs within the literature and constitute an obstacle for the harmonisation of internal exposure and dose-response modelling. Finally, risk characterisation was also performed for humans as well as for two animal species (i.e. pigs and chicken) using available tools for the modelling of internal dose and a component-based approach for selected mycotoxins mixtures

    Muscle MRI in neutral lipid storage disease (NLSD)

    Altres ajuts: This work has been supported by Telethon Grant: GGP14066A.Neutral lipid storage disease (NLSD) is a rare inherited disorder of lipid metabolism resulting in lipid droplets accumulation in different tissues. Skeletal muscle could be affected in both two different form of disease: NLSD with myopathy (NLSD-M) and NLSD with ichthyosis (NLSD-I). We present the muscle imaging data of 12 patients from the Italian Network for NLSD: ten patients presenting NLSD-M and two patients with NLSD-I. In NLSD-M gluteus minimus, semimembranosus, soleus and gastrocnemius medialis in the lower limbs and infraspinatus in the upper limbs were the most affected muscles. Gracilis, sartorius, subscapularis, pectoralis, triceps brachii and sternocleidomastoid were spared. Muscle involvement was not homogenous and characteristic "patchy" replacement was observed in at least one muscle in all the patients. Half of the patients showed one or more STIR positive muscles. In both NLSD-I cases muscle involvement was not observed by T1-TSE sequences, but one of them showed positive STIR images in more than one muscle in the leg. Our data provides evidence that muscle imaging can identify characteristic alterations in NLSD-M, characterized by a specific pattern of muscle involvement with "patchy" areas of fatty replacement. Larger cohorts are needed to assess if a distinct pattern of muscle involvement exists also for NLSD-I
