Report of the laboratory confirmation of cases from the CDTUB Pobé, Benin
In 2009, 420 diagnostic samples (corresponding to 301 patients suspected of M. ulcerans infection) from the Centre de DĂ©pistage et de Traitement de lâUlcĂšre de Buruli of Benin were subjected to IS2404 PCR at the Centre Hospitalier Universitaire of Angers. 180 samples (corresponding to 141 patients) were confirmed positive to M. ulcerans infection by PCR
Hyperbilirubinemia and Neurodevelopmental Outcome of Very Low Birthweight Infants: Results from the LIFT Cohort
Bilirubin-related neurotoxicity is an important clinical issue in very low birthweight (VLBW) infants, and the existing literature is inconsistent.To analyze the relationship between maximal serum unconjugated bilirubin levels (SBL) and neurodevelopmental outcome at 2-year corrected age in VLBW infants.Phototherapy was initiated in all infants born before 33 weeks of gestation, according to Maisels' recommendations. Neurodevelopmental assessment at 2-year corrected age was performed in all infants that survived. SBLs collected during the first week of life were used to define three tertiles of max-SBL. The first tertile corresponded to infants with the lowest max-SBL. percentile curves of SBLs in infants with an optimal outcome). When Maisels' recommendations were applied, max SBLs were higher in 8% of infants weighing 1001â1500 g and in 15% of infants weighing less than 1001 g. Our data seems to validate Maisels' recommendations in the overall population of infants born before 33 weeks of gestation, but not in infants weighing less than 1001 g

We report here the clinical case of a Nigerian adult patient who received medical care during October 2010, at the Center for Diagnosis and Treatment of Buruli ulcer in PobĂš (Benin). He presented a massive facial tumor associated with several subcutaneous (cervical, thoracic and upper limbs) nodules, evolving since several years. Tissue samples collected at PobĂš medical center were addressed to the mycology and histology laboratories of Angers University Hospital (France), according to the medical exchange agreement between the two institutions about the diagnosis and treatment of Buruli ulcer disease. Histological examination showed a Splendore-Hoeppli phenomenon, consisting of a granulomatous reaction made of eosinophilic polynuclear cells surrounding rare, large and irregular, non-septate hyphae. A filamentous fungus was isolated by cultivation of the clinical samples, which was identified as Conidiobolus coronatus. The patient was treated orally with daily doses of ketoconazole (400Â mg per day). After 4Â months of treatment, a marked regression of the facial lesion was obtained. A first constructive facial surgery was achieved, but the patient did not attend the second step. This case report allows us to remind the mycological diagnosis of this exotic mycosis, but also to emphasize the main difficulties encountered in medical management in the developing countries
Defining and targeting high-risk populations in Buruli ulcerâAuthors' reply
We thank Jordi Landier and colleagues for their comments about our recent Article in The Lancet Global Health. 1 In their work, Landier and coworkers generalise some of our observations on Buruli ulcer in Benin to those for Cameroon, the country that has the fifth highest prevalence of Buruli ulcer worldwide. Briefly, they make use of age and sex distribution from the Cameroon national census to show that patients aged 5â14 years were twice as likely to be affected by Buruli ulcer as older individuals; and that boys were over-represented in individuals younger than 15 years, women were over-represented in patients aged 15â50 years, and that men and women were equally represented in patients older than 50 years. They advocate the use of national census references to produce incidence rates and incidence rate ratios (IRRs), which they believe to be the proper way to draw valid conclusions
On the correlation between fragility and stretching in glassforming liquids
We study the pressure and temperature dependences of the dielectric
relaxation of two molecular glassforming liquids, dibutyl phtalate and
m-toluidine. We focus on two characteristics of the slowing down of relaxation,
the fragility associated with the temperature dependence and the stretching
characterizing the relaxation function. We combine our data with data from the
literature to revisit the proposed correlation between these two quantities. We
do this in light of constraints that we suggest to put on the search for
empirical correlations among properties of glassformers. In particular, argue
that a meaningful correlation is to be looked for between stretching and
isochoric fragility, as both seem to be constant under isochronic conditions
and thereby reflect the intrinsic effect of temperature

We enrolled 30 Beninese patients with Buruli ulcers in a pilot study to evaluate efficacy of an oral chemotherapy using rifampicinâplusâclarithromycin during an 8-week period. The treatment was well tolerated, and all patients were healed by 12 months after initiation of therapy without relaps
