    Migrants in their family contexts: Application of a methodology

    The composition of immigrant families is a topic which has attracted considerable public and political attention in recent years. In the late 1980s concern was expressed over the size of some households of Pacific Island peoples in New Zealand. In the 1990s a more persistent concern has been with the incidence of what have been called ‘astronaut’ families – families where one of the partners is persistently absent overseas. The ‘astronaut’ family phenomenon has been most commonly associated with Chinese immigrants from Hong Kong and Taiwan. This report uses a novel methodology to examine the incidence of ‘astronaut’ families in New Zealand at the time of the 1991 and 1996 censuses. The methodology is described in some detail in the first section, and it is hoped that the careful attention to the procedures used to examine migrants, who have been identified in the census, in their family contexts will stimulate further research in this area. The second part of the report presents the findings of an initial exploration of 1991 and 1996 census data using the methodology outlined in the first section. It is clear from results of this inquiry that the ‘astronaut’ family phenomenon is well established amongst some components of New Zealand’s Asian community. However, it is not as widespread as media comment in 1995 and 1996, before the 1996 election, suggested. It is important to develop ways of assessing characteristics of immigrant family structures in order to counter unsubstantiated assertions which promote negative stereotypes of immigrant communities. This research, which builds on a project supported by the Marsden Fund in 1997, suggest one fruitful avenue for making more extensive use of census data on immigrants in New Zealand to provide more objective assessment of “migrants in social context”

    Storage and hydrolysis of seawater samples for inorganic carbon isotope analysis

    Preservation of seawater samples was tested for total inorganic carbon (ΣCO2), stable carbon isotope (δ13C), and radiocarbon (14C) applications using foil bags and storage by refrigeration and freezing. The aim was to preserve representative samples with minimal storage effects but without using toxic methods such as mercuric chloride poisoning. Hydrolysis of samples to CO2 was based on existing methods. Results of IAEA-C2 standard used with deionized water stored in the foil bags showed complete reaction yields, 14C results within 2σ of the consensus value, and δ13C that were internally consistent, indicating that there were no procedural effects associated with the foil bags. 14C results were statistically indistinguishable across the storage times, for frozen and refrigerated seawater samples from a coastal site, Elie Ness, Fife, UK. The scatter of ΣCO2 concentrations and δ13C was within scatter observed in other studies for lake- and seawater samples preserved by acidification or using mercuric chloride. However, both ΣCO2 and δ13C were less variable for frozen samples compared with refrigerated samples. The foil bags are lighter, safer to transport, and similar in cost to glass bottles and allow sample collection in the field and transfer to the hydrolysis vessel without exposure of the sample to atmosphere. Storage of seawater samples in the foil bags was considered a reliable, alternative method to poisoning for ΣCO2, δ13C, and 14C, and freezing the samples is recommended for storage time beyond a week

    An evaluation of the quality of statistical design and analysis of published medical research : results from a systematic survey of general orthopaedic journals

    Background: The application of statistics in reported research in trauma and orthopaedic surgery has become ever more important and complex. Despite the extensive use of statistical analysis, it is still a subject which is often not conceptually well understood, resulting in clear methodological flaws and inadequate reporting in many papers. Methods: A detailed statistical survey sampled 100 representative orthopaedic papers using a validated questionnaire that assessed the quality of the trial design and statistical analysis methods. Results: The survey found evidence of failings in study design, statistical methodology and presentation of the results. Overall, in 17% (95% confidence interval; 10–26%) of the studies investigated the conclusions were not clearly justified by the results, in 39% (30–49%) of studies a different analysis should have been undertaken and in 17% (10–26%) a different analysis could have made a difference to the overall conclusions. Conclusion: It is only by an improved dialogue between statistician, clinician, reviewer and journal editor that the failings in design methodology and analysis highlighted by this survey can be addressed

    Controlled-release of opioids for improved pain management

    The adequate treatment of pain remains one of the major medical challenges. Morphine and other opioid drugs are most commonly used to counteract moderate to severe pain, but they are also increasingly accessed by patients with chronic non-malignant pain. To achieve long-term analgesia, opioid therapy still represents the standard treatment for chronic pain alleviation. This work presents an overview of current strategies aiming at controlled opioid release. Two important, and intrinsically linked, features are discussed in detail: the used formulations (i.e. polymer systems) and the applied drug administration routes. The different administration routes and their associated advantages and limitations are described. Links between the chemical structure of commonly used opioids and suited administration modes and formulations are made. This review can potentially give insight into new opportunities for adequate relief of chronic pain, a societal burden, by means of alternative (non)opioid analgesics and may serve as inspiration for future developments in this area

    Does Public Health Policy Matter?: Explaining Variation in COVID-19 Outcomes Across the 50 States

    The Covid-19 pandemic has prompted debate about what factors cause wide variations in mortality and infection rates across the United States and raised questions about what can be done to limit the spread of future outbreaks. In the comparative international politics literature, there are four explanations that determine how well a country can contain outbreaks: leadership, state capacity, demographics, and state culture. Currently, there are no studies that show a comprehensive evaluation of what has caused variations in mortality rate among the fifty states. This study aims to examine state variation among the 50 states in the U.S. and its influence on Covid-19 infection and mortality rate. The study will utilize a mixed-method approach to determine which factors have the most impact on mortality and infection rate. Using a multivariate and case study analysis, I aim to show how the four explanations predicted the pandemic case and death rates. In the findings of this study, I found that several factors, including Republican party control, urbanization, and race, predicted a state’s Covid-19 outcomes. The purpose of this study is to urge states to repair weaknesses in pandemic response plans, address structural discrimination within the healthcare system, and facilitate national cooperation that will better equip states with the ability to contain an outbreak

    Semiautomated Multimodal Breast Image Registration

    Consideration of information from multiple modalities has been shown to have increased diagnostic power in breast imaging. As a result, new techniques such as microwave imaging continue to be developed. Interpreting these novel image modalities is a challenge, requiring comparison to established techniques such as the gold standard X-ray mammography. However, due to the highly deformable nature of breast tissues, comparison of 3D and 2D modalities is a challenge. To enable this comparison, a registration technique was developed to map features from 2D mammograms to locations in the 3D image space. This technique was developed and tested using magnetic resonance (MR) images as a reference 3D modality, as MR breast imaging is an established technique in clinical practice. The algorithm was validated using a numerical phantom then successfully tested on twenty-four image pairs. Dice's coefficient was used to measure the external goodness of fit, resulting in an excellent overall average of 0.94. Internal agreement was evaluated by examining internal features in consultation with a radiologist, and subjective assessment concludes that reasonable alignment was achieved

    Is There Visual Lateralisation of the Sun Compass in Homing Pigeons?

    Functional lateralisation in the avian visual system can be easily studied by testing monocularly occluded birds. The sun compass is a critical source of navigational information in birds, but studies of visual asymmetry have focussed on cues in a laboratory rather than a natural setting. We investigate functional lateralisation of sun compass use in the visual system of homing pigeons trained to locate food in an outdoor octagonal arena, with a coloured beacon in each sector and a view of the sun. The arena was rotated to introduce a cue conflict, and the experimental groups, a binocular treatment and two monocular treatments, were tested for their directional choice. Wefoundnosignificantdifferenceintestorientationbetweenthetreatments,withallgroupsshowing evidence of both sun compass and beacon use, suggesting no complete functional lateralisation of sun compass use within the visual system. However, reduced directional consistency of binocular vs. monocular birds may reveal a conflict between the two hemispheres in a cue conflict condition. Birds using the right hemisphere were more likely to choose the intermediate sector between the training sectorandtheshiftedtrainingbeacon,suggestingapossibleasymmetryinfavourofthelefteye/right hemisphere (LE/RH) when integrating different cues

    Host-pathogen reorganisation during host cell entry by Chlamydia trachomatis

    Chlamydia trachomatis is obligate intracellular bacterial pathogen that remains a significant public health burden worldwide. A critical early event during infection is chlamydial entry into non-phagocytic host epithelial cells. Like other Gram-negative bacteria, C. trachomatis uses a type III secretion system (T3SS) to deliver virulence effector proteins into host cells. These effectors trigger bacterial uptake and promote bacterial survival and replication within the host cell. In this review, we highlight recent cryo-electron tomography that has provided striking insights into the initial interactions between Chlamydia and its host. We describe the polarised structure of extracellular C. trachomatis elementary bodies (EBs), and the supramolecular organisation of T3SS complexes on the EB surface, in addition to the changes in host and pathogen architecture that accompany bacterial internalisation and EB encapsulation into early intracellular vacuoles. Finally, we consider the implications for further understanding the mechanism of C. trachomatis entry and how this might relate to those of other bacteria and viruses
