    A Rare Case Report of Fatal Fulminant Hepatic Failure in a Child due to Mixed vivax and falciparum Infection

    Malaria remains an overwhelming problem in the tropical developing countries, with 300 to 500 million new cases and about a million deaths per year (Mishra et al., 2003). Malaria is a potentially life-threatening disease in the tropics. Jaundice is one of the severe manifestations of falciparum malaria. Its incidence (Mishra et al., 2003). varies between 10 and 45% in different reports and is seen more in adults than in children. Jaundice may vary from mild to very severe. However, clinical signs of hepatic encephalopathy (such as liver flaps) are never seen unless there is presence of concomitant viral hepatitis (WHO, 2000). Our case is a 6-year-old female child presented with fever, jaundice, and anasarca. Peripheral smear showed trophozoites and schizonts of Plasmodium (P.) vivax and trophozoites and gametocytes of P. falciparum. Viral markers for hepatitis were negative. She developed fulminant hepatic failure and expired after 26 hours of admission

    Purpose: To investigate the co-relation of sitting-rising test scores with measures of lower extremity strength, power, and body compositions (body mass index and waist-hip ratio). Furthermore, to find out the level of physical activity in the targeted population. Methods: Participants aged between 18-35 years (n=131) of both genders were recruited in this study. Along with performing sitting-rising test, anthropometric and demographic data were collected to calculate body mass index and waist-hip ratio. Lower extremity strength was assessed using a 30-second chair stand test, and power was assessed using a triple hop test for distance. Also, the Rapid Assessment Disuse Index questionnaire was given to dividing the population into two groups depending on whether they are involved more in physical activity or not. Results: Sitting-rising test scores showed a positive correlation with lower extremity strength and power, negative correlation with body mass index, and no co-relation with waist-hip ratio. Further, 76 participants out of 131 were less physical activity and had more sedentary behavior. Conclusion: Sitting-rising from the floor in young adults is influenced by the strength and power of lower extremities and body mass index except for the waist-hip ratio. Grahani Roga is a predominant Pitta-Vata Dosha and the Manasika Bhavas can be easily be disturbed by Vata and Pitta Dosha. IBS can be considered as Agnimandyajanya Vikara. The functional dependency between normal Vata Dosha and Agni is altered in IBS. The role of Dhatus and concerned Srotas i.e., Purishavaha Srotas is also considerable. Purishavaha Srotas does majority of function with the help of Apana Vayu. So, the Dusti lies at Apana Vayu & Samana Vayu level.     Context Driven Bipolar Adjustment for Optimized Aspect Level Sentiment Analysis

Reviews on online data highly affect the user’s perception while buying a particular or related product from an online shopping site. The online review provided by a customer helps other customers to make up their decision regarding purchasing that item. Looking at the developer’s and producer’s perspective, the opinions of customers on their manufactured items is helpful in identifying deformities as well as scope for improving its quality. Equipped with all this information, the product can be developed and managed more efficiently. Along with the overall rating of the product, the feature-based rating will have a great impact on the decision-making process of the customer. In this paper, an optimized scheme of aspect level sentiment analysis is presented to analyze the online reviews of a product. Reviews ratings have been used for learning approach. Inherently biased reviews are considered to optimize the Aspect Level Sentiment Analysis. Bi-polar aspect level sentiment analysis model has been trained using multiple kernels of support vector machine to optimize the results. Lexicon based aspect level sentiment analysis is performed first and later on the basis of bipolar words adjustment, and its effect on results, aspect level sentiment analysis for efficient optimization has been performed. A Web Crawler is developed to extract data from Amazon. The results obtained outperformed traditional lexicon based Aspect Level Sentiment Analysis

    Intrusion recovery for database-backed web applications

    Warp is a system that helps users and administrators of web applications recover from intrusions such as SQL injection, cross-site scripting, and clickjacking attacks, while preserving legitimate user changes. Warp repairs from an intrusion by rolling back parts of the database to a version before the attack, and replaying subsequent legitimate actions. Warp allows administrators to retroactively patch security vulnerabilities---i.e., apply new security patches to past executions---to recover from intrusions without requiring the administrator to track down or even detect attacks. Warp's time-travel database allows fine-grained rollback of database rows, and enables repair to proceed concurrently with normal operation of a web application. Finally, Warp captures and replays user input at the level of a browser's DOM, to recover from attacks that involve a user's browser. For a web server running MediaWiki, Warp requires no application source code changes to recover from a range of common web application vulnerabilities with minimal user input at a cost of 24--27% in throughput and 2--3.2 GB/day in storage.United States.     A molecular systems approach to modelling human skin pigmentation: identifying underlying pathways and critical components

    Background: Ultraviolet radiations (UV) serve as an environmental stress for human skin, and result in melanogenesis, with the pigment melanin having protective effects against UV induced damage. This involves a dynamic and complex regulation of various biological processes that results in the expression of melanin in the outer most layers of the epidermis, where it can exert its protective effect. A comprehensive understanding of the underlying cross talk among different signalling molecules and cell types is only possible through a systems perspective. Increasing incidences of both melanoma and non-melanoma skin cancers necessitate the need to better comprehend UV mediated effects on skin pigmentation at a systems level, so as to ultimately evolve knowledge-based strategies for efficient protection and prevention of skin diseases. Methods: A network model for UV-mediated skin pigmentation in the epidermis was constructed and subjected to shortest path analysis. Virtual knock-outs were carried out to identify essential signalling components. Results: We describe a network model for UV-mediated skin pigmentation in the epidermis. The model consists of 265 components (nodes) and 429 directed interactions among them, capturing the manner in which one component influences the other and channels information. Through shortest path analysis, we identify novel signalling pathways relevant to pigmentation. Virtual knock-outs or perturbations of specific nodes in the network have led to the identification of alternate modes of signalling as well as enabled determining essential nodes in the process. Conclusions: The model presented provides a comprehensive picture of UV mediated signalling manifesting in human skin pigmentation. A systems perspective helps provide a holistic purview of interconnections and complexity in the processes leading to pigmentation. The model described here is extensive yet amenable to expansion as new data is gathered. Through this study, we provide a list of important proteins essential for pigmentation which can be further explored to better understand normal pigmentation as well as its pathologies including vitiligo and melanoma, and enable therapeutic intervention

    Nasal septal angle deviation: effect on lateral wall in nasal obstruction

    Background: Deviation of the nasal septum (DNS) refers to the convexity of the septum to one side disturbing the nasal physiology with obstructed nasal breathing leading to lateral nasal wall abnormalities and paranasal sinuses (PNS) mucosal disease. Knowledge of nasal morphological parameters plays an important role in planning successful nasal surgery. Our aim was to evaluate the angle of septal deviation (ASD) on CT scan and study its influence on the lateral nasal wall abnormalities and PNS mucosal disease.Methods: A prospective cross-sectional observational study was conducted on 130 patients with clinical evidence of DNS and chronic sinusitis. The direction and severity of DNS was recorded on CT scan along with evaluation of lateral nasal wall and sinus mucosal abnormalities.Results: Increasing ASD had statistically significant correlation with the lateral nasal wall abnormalities, most commonly, contralateral middle and inferior turbinate hypertrophy (p-value <0.0001). No significant association was found with the incidence of ipsilateral or contralateral osteomeatal complex (OMC) obstruction and sinus mucosal disease.Conclusions: The direction and severity of septal deviation has significant impact on contralateral middle and inferior turbinate hypertrophy. The analysis of these ancillary pathologies can be of great help to the surgeon in better management of patients with nasal obstruction

    Relationship between grey scale sonographic grades of fatty liver and shear wave elastographic values: an observational study

    Background: Due to the growing public health menace of metabolic syndrome, the Non-Alcoholic Fatty Liver Disease (NAFLD) has been recording a burgeoning global rise. The prognosis of NAFLD is largely depends upon its histological stage. Simple steatosis has a fairer prognosis, while Non-Alcoholic Steatohepatitis (NASH) has a poorer prognosis with tendency to progression into fibrosis and end stage liver disease. Until now, the diagnosis of varying grades of NAFLD has been dependent upon liver biopsy, which is indisputably the most reliable tool to distinguish between simple steatosis, steatohepatitis, and end stage liver disease. The sonographic grading of NAFLD is primarily based on subjective findings, the results of which may vary between different observers. Hence, there is a need to identify a reliable non-invasive objective substitute. To compare the sonographic grades of fatty liver on gray scale with liver stiffness (Kpa) values of shear wave elastography. Also, an attempt to establish objective criteria for grading of fatty liver with improved accuracy and increased specificity.Methods: Cross sectional, observational study comprising of 240 adults referred for routine abdominal sonography. All patient underwent gray scale sonography and shear wave elastography and results were analysed.Results: A significant positive correlation was found between various grades of NAFLD as assessed subjectively on grey scale sonography to the liver stiffness in kilopascal (Kpa) using shear wave elastography.Conclusions: Shear wave elastography being an objective imaging tool is a reliable modality compared to grey scale sonography in diagnosis of NAFLD and has the capacity to carry out quantitative evaluation of liver parenchyma in vivo
