    Breastfeeding: Physiological and Cultural Influences

    Within a biological perspective, breasts are indisputably for the use of nourishment of the young. This can be seen by how mammary glands are used by every other species other than humans. Humans are the only species that also looks at breasts within a cultural perspective, and that perspective is where the debate of breastfeeding occurs. There is really no debate that if given the option to breastfeed, breastfeeding is the best choice for the development of infants due to the fact that breast milk has been adapted for the human physique. In spite of the health benefits associated with breastfeeding and the economic disadvantages that accompany the use of infant formula, social influences have caused a shift in neonatal health in the United States from wet-nursing to breastfeeding to now formula feeding. The method of gathering data about this topic was primarily from previous studies and reports in the same method as a literature review. The findings of this paper support the notion that breastfeeding nourished infants best; however, cultural influences cause mothers to choose bottle-feeding more often due to factors such as an increase of women in the workforce, the sexualization of breasts, and the influence of the formula industry

    Teach Them How They Learn: Learning Styles and Information Systems Education

    The rich, interdisciplinary tradition of learning styles is markedly absent in information systems-related research. The current study applies the framework of learning styles to a common educational component of many of todayā€™s information systems curricula - object-oriented systems development - in an effort to answer the question as to whether oneā€™s learning style, when matched with a specific complementary instructional methodology, results in increased domain-specific performance. The data collected from 196 information systems majors enrolled in object-oriented systems development courses suggest that task performances increases significantly when the instructional methodology closely mirrors the studentā€™s learning style inclination

    Apoptotic changes in the myocardium in the course of experimentally-induced pleurisy

    The secreted proinflammatory interleukins IL-1, IL-6 and TNF in the course of experimentally-induced pleurisy can be the cause of pathological changes in the ultrastructure of cardiac muscle and of apoptosis. The pleurisy was induced in rats by means of carrageenin. The scraps of cardiac muscle obtained during the inflammatory reaction in the pleura were analysed by means of an electron microscope. The scraps were also stained with the TUNEL method in order to find the apoptotic foci. It was proved by the experiment that the inflammatory process affected mitochondria in the cardiomyocytes, enhanced collagen fibre synthesis and contributed to the formation of apoptotic foci in the cardiac muscle

    Uncovering Research Opportunities in the Medical Informatics Field: A Quantitative Content Analysis

    With rapid improvements in technology, the ever-pressing need to reduce healthcare costs, and continuing legislation emphasizing medical reforms, the demand for research within the healthcare/information systems interface is growing. In this study, we ascertain the prevalent themes from the extant medical informatics literature in an effort to motivate research beyond the traditional domain of health information technology research so information systems scholars can better understand where their expertise might contribute to advancements in healthcare. We used a quantitative content analysis method to systematically explore 2,188 article texts from journals in medical informatics, medicine, and MIS published over a ten-year period. Texts were analyzed using centering resonance analysis and factor analysis and the following themes emerged from the literature: Analytics; Healthcare Operations and Standards (with sub-themes: Operations, Project Management, and Information Assurance); Knowledge Transfer/Communication (with sub-themes: Extending beyond the Organization, Internal to the Organization, and Patient-Provider); Perceptions and Managing Expectations of Information Technology; Advancements in Research; and Software as a Service. In this article, we discuss these themes in greater detail and offer directions for future research

    Cryptosporidium, Enterocytozoon, and Cyclospora Infections in Pediatric and Adult Patients with Diarrhea in Tanzania.

    Cryptosporidiosis, microsporidiosis, and cyclosporiasis were studied in four groups of Tanzanian inpatients: adults with AIDS-associated diarrhea, children with chronic diarrhea (of whom 23 of 59 were positive [+] for human immunodeficiency virus [HIV]), children with acute diarrhea (of whom 15 of 55 were HIV+), and HIV control children without diarrhea. Cryptosporidium was identified in specimens from 6/86 adults, 5/59 children with chronic diarrhea (3/5, HIV+), 7/55 children with acute diarrhea (0/7, HIV+), and 0/20 control children. Among children with acute diarrhea, 7/7 with cryptosporidiosis were malnourished, compared with 10/48 without cryptosporidiosis (P < .01). Enterocytozoon was identified in specimens from 3/86 adults, 2/59 children with chronic diarrhea (1 HIV+), 0/55 children with acute diarrhea, and 4/20 control children. All four controls were underweight (P < .01). Cyclospora was identified in specimens from one adult and one child with acute diarrhea (HIV-). Thus, Cryptosporidium was the most frequent and Cyclospora the least frequent pathogen identified. Cryptosporidium and Enterocytozoon were associated with malnutrition. Asymptomatic fecal shedding of Enterocytozoon in otherwise healthy, HIV children has not been described previously

    Convergence and Perturbation Resilience of Dynamic String-Averaging Projection Methods

    We consider the convex feasibility problem (CFP) in Hilbert space and concentrate on the study of string-averaging projection (SAP) methods for the CFP, analyzing their convergence and their perturbation resilience. In the past, SAP methods were formulated with a single predetermined set of strings and a single predetermined set of weights. Here we extend the scope of the family of SAP methods to allow iteration-index-dependent variable strings and weights and term such methods dynamic string-averaging projection (DSAP) methods. The bounded perturbation resilience of DSAP methods is relevant and important for their possible use in the framework of the recently developed superiorization heuristic methodology for constrained minimization problems.Comment: Computational Optimization and Applications, accepted for publicatio

    Whey Protein Augments Leucinemia and Post-Exercise p70S6K1 Activity Compared to a Hydrolysed Collagen Blend When in Recovery From Training With Low Carbohydrate Availability

    We examined the effects of whey versus collagen protein on skeletal muscle cell signalling responses associated with mitochondrial biogenesis and protein synthesis in recovery from an acute training session completed with low carbohydrate (CHO) availability. In a repeated measures design (after adhering to a 36-h exercise-dietary intervention to standardise pre-exercise muscle glycogen), eight males completed a 75-min non-exhaustive cycling protocol and consumed 22 g of a hydrolysed collagen blend (COLLAGEN) or whey (WHEY) protein 45 min prior to exercise, 22 g during exercise and 22 g immediately post-exercise. Exercise decreased (P0.05) was observed for p53, Parkin and Beclin1 mRNA. Exercise suppressed (P<0.05) p70S6K1 activity in both conditions immediately post-exercise (ā‰ˆ 25 fmol.min-1.mg-1). Post-exercise feeding increased p70S6K1 activity at 1.5 h post-exercise (P<0.05), the magnitude of which was greater (P <0.05) in WHEY (180 Ā± 105 fmol.min-1.mg-1) versus COLLAGEN (73 Ā± 42 fmol.min-1.mg-1). We conclude that protein composition does not modulate markers of mitochondrial biogenesis when in recovery from a training session deliberately completed with low CHO availability. In contrast, whey protein augments post-exercise p70S6K activity compared with hydrolysed collagen, as likely mediated via increased leucine availability

    Capillary Assembly of Anisotropic Particles at Cylindrical Fluid-Fluid Interfaces

    The unique behavior of colloids at liquid interfaces provides exciting opportunities for engineering the assembly of colloidal particles into functional materials. The deformable nature of fluid-fluid interfaces means that we can use the interfacial curvature, in addition to particle properties, to direct self-assembly. To this end, we use a finite element method (Surface Evolver) to study the self-assembly of rod-shaped particles adsorbed at a simple curved fluid-fluid interface formed by a sessile liquid drop with cylindrical geometry. Specifically, we study the self-assembly of single and multiple rods as a function of drop curvature and particle properties such as shape (ellipsoid, cylinder, and spherocylinder), contact angle, aspect ratio, and chemical heterogeneity (homogeneous and triblock patchy). We find that the curved interface allows us to effectively control the orientation of the rods, allowing us to achieve parallel, perpendicular, or novel obliquely orientations with respect to the cylindrical drop. In addition, by tuning particle properties to achieve parallel alignment of the rods, we show that the cylindrical drop geometry favors tip-to-tip assembly of the rods, not just for cylinders, but also for ellipsoids and triblock patchy rods. Finally, for triblock patchy rods with larger contact line undulations, we can achieve strong spatial confinement of the rods transverse to the cylindrical drop due to the capillary repulsion between the contact line undulations of the particle and the pinned contact lines of the sessile drop. Our capillary assembly method allows us to manipulate the configuration of single and multiple rod-like particles and therefore offers a facile strategy for organizing such particles into useful functional materials

    Ethionamide Population Pharmacokinetic Model and Target Attainment in Multidrug-Resistant Tuberculosis

    Ethionamide (ETA), an isonicotinic acid derivative, is part of the multidrug-resistant tuberculosis (MDR-TB) regimen. The current guidelines have deprioritized ETA because it is potentially less effective than other agents. Our aim was to develop a population pharmacokinetic (PK) model and simulate ETA dosing regimens in order to assess target attainment. This study included subjects from four different sites, including healthy volunteers and patients with MDR-TB. The TB centers included were two in the United States and one in Bangladesh. Patients who received ETA and had at least one drug concentration reported were included. The population PK model was developed, regimens with a total of 1,000 to 2,250 mg daily were simulated, and target attainment using published MICs and targets of 1.0-log kill and resistance suppression was assessed with the Pmetrics R package. We included 1,167 ethionamide concentrations from 94 subjects. The final population model was a one-compartment model with first-order elimination and absorption with a lag time. The mean (standard deviation [SD]) final population parameter estimates were as follows: absorption rate constant, 1.02 (1.11) h(-1); elimination rate constant, 0.69 (0.46) h(-1); volume of distribution, 104.16 (59.87) liters; lag time, 0.43 (0.32) h. A total daily dose of 1,500 mg or more was needed for >= 90% attainment of the 1.0-log kill target at a MIC of 1 mg/liter, and 2,250 mg/day led to 80% attainment of the resistance suppression target at a MIC of 0.5 mg/liter. In conclusion, we developed a population PK model and assessed target attainment for different ETA regimens. Patients may not be able to tolerate the doses needed to achieve the pre-defined targets supporting the current recommendations for ETA deprioritization
