Psychological traits in hypertension
Wstęp Dotychczas nie wyjaśniono znaczenia cech osobowości
dla rozwoju pierwotnego nadciśnienia tętniczego.
Spośród cech osobowości największe znaczenie
przypisuje się aleksytymii, niejawnej wrogości oraz neurotyzmowi.
Wyniki badań są jednak wieloznaczne.
Celem badania była ocena wybranych cech osobowości
u osób z nadciśnieniem tętniczym.
Materiał i metody Do badania włączono 42 osoby
z nadciśnieniem tętniczym i 57 osób tworzących grupę
kontrolną. Badanych kwalifikowano spośród osób, które
zgłosiły się na badanie przesiewowe chorób układu
krążenia w podwrocławskiej wsi Boguszyce. Do grupy
z nadciśnieniem tętniczym kwalifikowano osoby, u których
w przeszłości rozpoznano nadciśnienie tętnicze
i które były leczone z tego powodu lub u których przygodne
ciśnienie tętnicze było wyższe niż 140/90 mm Hg.
Cechy osobowości oceniono za pomocą następujących
kwestionariuszy: aleksytymii Bermonda-Vorsta (BVAQ),
wrogości CookaMedleya, dostosowania społecznego
Marlowe’a-Crowne’a oraz skal neurotyzmu i kłamstwa
Kwestionariusza Osobowości Eysencka.
Wyniki Statystycznie istotne różnice stwierdzono w zakresie
wymiaru werbalizacji skali BVAQ (grupa z nadciśnieniem
tętniczym 25,76/grupa kontrolna 23,57; p =
0,032), różnicy między wymiarami reaktywności i rozpoznawania
(emocji) (-1,96/1,64; p = 0,024) oraz neurotyzmu
(14,80/12,21; p = 0,02). Po uwzględnieniu różnicy
wieku za pomocą analizy kowariancji stwierdzono, że
różnice średnich wartości wszystkich skal nie były statystycznie
istotne. Stwierdzono jednak, że wartości niektórych
pozycji skal neurotyzmu i BVAQ różniły się
znacząco w grupach. Z pozycji tych utworzono odrębne
skale, których wartości średnie różniły się w grupach,
nawet po uwzględnieniu wieku badanych. W analizie
tych skal wykazano, że dotyczą skłonności do przeżywania
negatywnych emocji oraz wglądu w emocje.
Wnioski Osoby z nadciśnieniem tętniczym charakteryzowały
się większą reaktywnością emocjonalną
oraz mniejszą umiejętnością rozpoznawania i werbalizowania
własnych emocji.Background The relationship between psychological factors
and hypertension has not been established. Such traits
as alexithymia, hostility, and neuroticism are the most
common suggested dimensions of personality that are contributable
to essential hypertension. However, the researches
have yielded inconsistent results.
The purpose of the study was the estimation of certain
psychological traits in hypertensives.
Material and methods We have studied 42 persons with
hypertension and 57 of control group. Participants were
recruited from persons who volunteered to participate in
free medical control of circulation diseases organized in
small polish village. Hypertensive group consisted of subjects
who had the diagnosis of hypertension and were
treated because of this or in who the office blood pressure
measurement was higher than 140/90 mmHg. Psychological
traits were estimated by using following questionnaires:
the Bermond-Vorst Alexithymia Questionnaire (BVAQ),
The Cook-Medley Hostility Scale, The Marlowe-Crowne
Social Desirability Scale, and two subscales from Eysenck
Personality Questionnaire (EPQ-R) - neuroticism and lie.
Results Statistically significant differences were noted in
alexithymic difficulty in verbalizing mean score
(hypertensives - 25.76/normotensives - 23.57; p = 0.032),
in the value of difference between alexithymic emotional
and cognitive dimensions (-1,96/1,64; p = 0,024) and in neuroticism score (14.8/12.21; p = 0.02). Abovementioned
differences became statistically non relevant
when age difference between groups was accounted. However,
there were some positions in neuroticism and BVAQ
scales that differed between groups. From these positions
two separate scales were constructed. Mean values of these
scales differed between groups even when age was accounted.
Analysis of these scales indicated that differences
between groups concerned tendency to experience negative
emotions and recognizing emotions.
Conclusions Hypertensives had the tendency of greater
emotional reactivity and lower ability to identify and verbalize
emotions

Background: Atrial fibrillation is the most common arrythmia in patients beyond 65 years of age. Several of those patients have
contraindications to coumadin treatment. Ninety percent of atrial thrombi related to atrial fibrillation are found in the left atrial appendage.
Aim: We present our early experience with the left atrial appendage transcatheter closure using the Amplatzer® Cardiac Plug.
Methods: Transcatheter closure was performed in general anaesthesia and under transoesophageal echocardiography control in
three patients aged 69, 73 and 61 years (two female). All had atrial fibrillation with high risk of thromboembolic events (CHADS2
score ≥ 2 points). In all of them coumadin treatment was contraindicated.
Results: The procedure time was 112, 109 and 60 min (fluoroscopy time: 25, 29 and 12,5 min). The Amplatzer® Cardiac Plugs of
24, 26 and 22 mm were implanted successfully and without complications. The day after, transthoracic echocardiogram revealed
proper position of the implant and complete occlusion of the appendage in all patients. The patients were discharged on antiplatelet
treatment.
Conclusion: Transcatheter left atrial appendage closure with the Amplatzer® Cardiac Plug may be a promissing therapeutic option
for patients with atrial fibrillation-related high risk of thromboembolic events and with contraindications to anti-vitamin K treatment
Dyslipidemias in healthy individuals and patients with cardiovascular diseases in rural population
Wstęp Choroby serca i naczyń są w Polsce główną
przyczyną zgonów. Do głównych czynników ryzyka
chorób układu krążenia zalicza się: zaburzenia lipidowe,
palenie tytoniu, nadciśnienie tętnicze, cukrzycę,
otyłość oraz czynniki psychospołeczne. Celem
pracy była ocena częstości występowania zaburzeń
gospodarki lipidowej w populacji wiejskiej.
Materiał i metody Badaniami objęto 178 osób (118
kobiet i 60 mężczyzn) w wieku 19-84 lat (śr. wieku:
53,9 ± 13,5) ze wsi Boguszyce w powiecie oleśnickim
(koło Wrocławia) na Dolnym Śląsku. Wykonywane
badania biochemiczne obejmowały oznaczenia
cholesterolu całkowitego, cholesterolu frakcji
HDL oraz trójglicerydów w surowicy. Oznaczeń dokonywano
w laboratorium Katedry i Kliniki Chorób Wewnętrznych, Zawodowych i Nadciśnienia Tętniczego
Akademii Medycznej we Wrocławiu. Stężenie
cholesterolu frakcji LDL obliczano za pomocą wzoru
Friedewalda. Badania przeprowadzono w ramach
studenckiego obozu naukowego w dniach 3-14
września 2005 roku.
Wyniki Średnie stężenie cholesterolu całkowitego
w próbie wyniosło 193,97 ± 38,59 mg/dl. Stężenie cholesterolu
całkowitego korelowało dodatnio z wiekiem
(r = 0,53, p = 0,027). Częstość występowania podwyższonego
stężenia cholesterolu całkowitego
(≥ 190 mg/dl) w badanej populacji wyniosła 51,47%.
Znacznie podwyższone stężenie tego czynnika
(≥ 320 mg/dl) stwierdzono u 5,12% badanych. Średnie
stężenie cholesterolu frakcji LDL w próbie wyniosło
112,13 ± 34,50 mg/dl. U 44,85% badanych stwierdzono
podwyższone stężenie cholesterolu frakcji LDL
(≥ 115 mg/dl). Średnie stężenie cholesterolu frakcji
HDL w próbie wyniosło 55,42 ± 14,35 mg/dl. Obniżone
stężenie cholesterolu frakcji HDL (< 46 mg/dl
u kobiet oraz < 40 mg/dl u mężczyzn) zanotowano
u 25,45% badanych. Średnie stężenie trójglicerydów
w próbie wyniosło 132,08 ± 82,08 mg/dl. Wszystkie
dyslipidemie występowały istotnie częściej u chorych
z rozpoznanymi chorobami układu krążenia.
Wnioski W badanej populacji wiejskiej częstość zaburzeń
lipidowych jest większa niż w całej populacji
Polski. Wynik ten może świadczyć o nieprawidłowym
odżywianiu się i niewłaściwym stylu życia.
Większa częstość zaburzeń lipidowych u chorych
z rozpoznanymi chorobami układu krążenia wskazuje
na niedostateczne leczenie tych schorzeń i może
świadczyć o gorszym dostępie do lekarzy pierwszego

of death in European Union countries. Many determinants
of global cardiovascular risk have been identified. Modifiable
risk factors include: arterial hypertension,
dyslipidemia, obesity, diabetes mellitus and tobacco smoking.
A group of non-modifiable factors includes: age, gender,
family history of the cardiovascular disease and formerly
diagnosed heart and/or vascular diseases. The aim
of the study was to estimate the frequency of lipid disorders
in rural population.
Material and methods The study group consisted of 178
inhabitants of the village of Boguszyce (118 women and 60
men, aged 19-84; mean age 53.9 ± 13.5) in Olesnica district
(in the neighbourhood of the city of Wroclaw) in
Lower Silesia. Studies were carried out by trained students
of medical faculty during summer scientific meetings. Serum
levels of total cholesterol, HDL cholesterol and
triglycerides were assessed.Results Mean total cholesterol concentration was 193.97
± 38.59 mg/dl. Mean cholesterol level correlated positively
with age (r = 0.53, p = 0.027). The frequency of increased
total cholesterol level (≥ 190 mg/dl) in the study population
was 51.47%. The excessive increase of cholesterol level
(≥ 320 mg/dl) was detected in 5.12% of studied individuals.
Mean LDL cholesterol level was 112.13 ± 34.50 mg/dl. In
44.85% of the study group the increased concentration of
LDL cholesterol was noted (exceeding ≥ 115 mg/dl).
Mean HDL cholesterol concentration was 55.42 ± 14.35
mg/dl. The decreased HDL concentration (< 46 mg/dl for
women and < 40 mg/dl for men) was present in 25.45% of
subjects. Mean triglycerides concentration was 132.08 ±
82.08 mg/dl. All dyslipidemias were more common in patients
with known cardiovascular disroders.
Conclusions In the studied rural population lipid disturbances
are more common than in general population of
Poland. It may suggest that village dwellers are more prone
to unhealthy lifestyle with improper diet. Higher prevalence
of dyslipidemias in subjects with known cardiovascular
diseases points out inadequate treatment and possibly

Ankle-brachial index and other risk factors of diastolic dysfunction of the left ventricle
Wstęp Dysfunkcja rozkurczowa lewej komory jest
czynnikiem ryzyka rozwoju powikłań sercowo-naczyniowych.
Celem opisywanego badania było określenie
występowania dysfunkcji rozkurczowej lewej
komory przy użyciu metod ultrasonograficznych
w badanej populacji wiejskiej w zależności od współistniejących
czynników ryzyka.
Materiał i metody Badanie przeprowadzono u 178
mieszkańców wsi Boguszyce (średni wiek 53,9 ±
13,5 r.). Funkcję rozkurczową lewej komory oceniano
za pomocą parametrów echokardiograficznych,
takich jak prędkość fali E i fali A, stosunek E/A, czas
deceleracji fali E (DcT E) i czas rozkurczu izowolumetrycznego
(IVRT). W badanej populacji oceniano
również wskaźnik masy ciała (BMI), skurczowe
i rozkurczowe ciśnienie tętnicze, parametry gospodarki
lipidowej oraz wskaźnik kostka-ramię.
Wyniki W modelu regresji logistycznej wykazano, że
niezależnymi czynnikami wystąpienia dysfunkcji rozkurczowej
lewej komory są wiek (OR: 1,11; p < 0,01),
wysoki BMI (OR: 1,02; p < 0,05), nadciśnienie tętnicze
(OR: 1,61; p < 0,05), niska wartość wskaźnika
kostka-ramię (OR: 0,52; p < 0,05) i niskie stężenia
cholesterolu frakcji HDL (OR: 0,98; p < 0,05).
Wnioski Według autorów pracy dysfunkcja rozkurczowa
lewej komory występuje częściej u osób
z wieloma czynnikami ryzyka chorób sercowo-naczyniowych.
Interesujący jest fakt ujemnej korelacji
wskaźnika kostka-ramię ze wskaźnikami dysfunkcji
rozkurczowej w badanej populacji, gdzie większość
badanych miała prawidłowe wartości tego
wskaźnika.Background Diastolic dysfunction of the left ventricle is
a risk factor of cardiovascular complications. The aim of the
study was to determine the diastolic dysfunction of the left
ventricle with the use of echocardiography in the rural
population according to coexisting risk factors.
Material and methods The study was performed in 178
people (mean age 53.9 ± 13.5), inhabitants of a village of
Boguszyce. The diastolic function of left ventricle was assessed
by echocardiography, by various parameters including
E and A waves, E/A ratio, deceleration time (DT) of
E (DTE) and isovolumetric relaxation time (IVRT). In
the studied population BMI, systolic and diastolic arterial
pressures, lipid parameters and ankle/arm index were
measured.
Results In a model of logistic regression analysis it was
revealed that independent risk factors of diastolic left ventricle
dysfunction incidence are: age (OR: 1.11; p < 0.01),
high BMI (OR: 1.02; p < 0.05), arterial hypertension (OR:
1.61; p < 0.05), low ankle–brachial index (OR: 0.52; p <
0.05) and low HDL cholesterol level (OR: 0.98; p < 0.05).
Conclusion We conclude that diastolic dysfunction of the
left ventricle is more common in patients with multiple
risk factors of cardiovascular diseases
The Role of Complement Activating Collectins and Associated Serine Proteases in Patients With Hematological Malignancies, Receiving High-Dose Chemotherapy, and Autologous Hematopoietic Stem Cell Transplantations (Auto-HSCT)
We conducted a prospective study of 312 patients (194 with multiple myeloma,
118 with lymphomas) receiving high-dose conditioning chemotherapy and autologous
hematopoietic stem cell transplantation (auto-HSCT). Polymorphisms of MBL2 and
MASP2 genes were investigated and serial measurements of serum concentrations
of mannose-binding lectin (MBL), CL-LK collectin and MASP-2 as well as activities
of MBL-MASP-1 and MBL-MASP-2 complex were made. Serum samples were
taken before conditioning chemotherapy, before HSCT and once weekly after (totally
4-5 samples); in minority of subjects also 1 and/or 3 months post transplantation.
The results were compared with data from 267 healthy controls and analyzed
in relation to clinical data to explore possible associations with cancer and with
chemotherapy-induced medical complications. We found a higher frequency of MBL
deficiency-associated genotypes (LXA/O or O/O) among multiple myeloma patients
compared with controls. It was however not associated with hospital infections or
post-HSCT recovery of leukocytes, but seemed to be associated with the most
severe infections during follow-up. Paradoxically, high MBL serum levels were a
risk factor for prolonged fever and some infections. The first possible association
of MBL2 gene 3′-untranslated region polymorphism with cancer (lymphoma) in
Caucasians was noted. Heterozygosity for MASP2 gene +359 A>G mutation was relatively frequent in lymphoma patients who experienced bacteremia during hospital
stay. The median concentration of CL-LK was higher in myeloma patients compared
with healthy subjects. Chemotherapy induced marked increases in serum MBL and
MASP-2 concentrations, prolonged for several weeks and relatively slighter decline in
CL-LK level within 1 week. Conflicting findings on the influence of MBL on infections
following chemotherapy of myeloma and lymphoma have been reported. Here we found
no evidence for an association between MBL deficiency and infection during the short
period of neutropenia following conditioning treatment before HSCT. However, we noted
a possible protective effect of MBL during follow-up, and suspected that to be fully
effective when able to act in combination with phagocytic cells after their recovery
Prevalence, risk factors and treatment of arterial hypertension in dwellers of Boguszyce village
Wstęp Nadciśnienie tętnicze jest jednym z podstawowych
czynników ryzyka chorób krążenia zwłaszcza
wśród mieszkańców wsi. Celem obecnej pracy była ocena
częstości występowania, czynników ryzyka oraz leczenia
nadciśnienia tętniczego wśród mieszkańców wsi
Boguszyce w powiecie oleśnickim na Dolnym Śląsku.Materiał i metody Badania przeprowadzono u dorosłych
mieszkańców wsi Boguszyce. Badania przeprowadzali
przeszkoleni studenci medycyny i lekarze,
odwiedzający mieszkańców wsi w ich domach. Badanie
obejmowało pomiar masy ciała, wzrostu oraz dwukrotny
pomiar ciśnienia tętniczego metodą Korotkowa.
Ponadto przeprowadzono badanie ankietowe, którego
celem była ocena wiedzy na temat nadciśnienia tętniczego
oraz współistniejących czynników ryzyka.
Wyniki Badania antropometryczne i ankietowe przeprowadzono
u 409 osób, czyli 50% dorosłych mieszkańców
Boguszyc. U pozostałych mieszkańców nie
wykonano badań z powodu nieobecności w domu
(3-krotna próba) lub braku zgody na badania. Wśród
badanych mężczyźni stanowili 40,83%, a kobiety
59,17%. Nadciśnienie tętnicze stwierdzono u 51%
mieszkańców, z czego 53% wcześniej nie miało rozpoznanego
nadciśnienia. Średni wskaźnik masy ciała
(BMI) badanej grupy wynosił 27,25 i był istotnie wyższy
u chorych z nadciśnieniem tętniczym niż u zdrowych
(30,5 vs. 24; p < 0,001). Wśród osób leczonych
68% przyjmowało przepisane leki, a 18,5% było leczonych
skutecznie. Wśród przepisywanych leków przeważały
β-adrenolityki, inhibitory konwertazy angiotensyny
i diuretyki. Zwracał uwagę brak substytucji potasem
mimo częstego stosowania leków moczopędnych.
Wnioski Wśród mieszkańców wsi Boguszyce odsetek
chorych na nadciśnienie tętnicze jest znacznie
wyższy niż średnia w Polsce. Wśród chorych na nadciśnienie
tętnicze występuje rozbieżność między subiektywną
a obiektywną oceną skuteczności leczenia.
Przyczyną niskiego odsetka skuteczności leczenia
nadciśnienia tętniczego jest między innymi
nieprzestrzeganie zaleceń lekarskich oraz styl życia
i nawyki żywieniowe mieszkańców Boguszyc.Background Arterial hypertension is the most common
cardiovascular risk factor especially in village dwellers. The
aim of the study was to estimate the prevalence and risk
factors of arterial hypertension in dwellers of the village of
Boguszyce in Oleśnica district in Lower Silesia.
Material and methods The study group consisted of dwellers
of the village of Boguszyce. Examinations were carried out
by the trained medical students and physicians visiting village
dwellers at their homes. They determined the body mass,
height, and measured blood pressure two times based on the
Korotkov method. A custom questionnaire was carried out to
determine medical history and coexisting risk factors.
Results Examinations were carried out in 409 persons (50%
of adult village population). The remaining half of village
population was not examined due to following reasons:
absence at home (3 separate visits), lack of consent. The
studied group consisted of 40.83% men and - 59.17%
women. Arterial hypertension was diagnosed in 51% of
studied population; 53% of this group was not diagnosed earlier with AH. Mean BMI in the study group was 27.25
and was significantly higher in adults with hypertension
(30.5 vs. 24; p < 0.001). 68% of adults treated for hypertension
took prescribed medications but only 18.5% were
treated succesfully. Main presribed antihypertensives were
betablockers, ACEI and diuretics. In spite of frequent use
of diuretices potassium substitution was rare.
Conclusions Arterial hypertension is more common in village
dwellers than in general polish population. There is
significant discrepancy between perception of treatment
results and objective hypertension control. The noncompliance
to physicians’ recommendations, the life style and
eating habits of the dwellers of Boguszyce, resulting in low
physical activity and high percentage of overweight are the
chief reasons for high prevalence and low effectiveness of
the treatment of hypertension
