    Healthcare Resources Use in Patients with Human Immunodeficiency Virus (HIV). Real-World Evidence From Six Italian Local Health Units

    AIM: The aim of the study was to evaluate healthcare resource use and related costs for the management of people living with Human Immunodeficiency Virus (PLWHIV) with and without comorbidities, and to compare the burden of comorbidities in PLWHIV to the general population.METHODS: An observational retrospective analysis, based on administrative and laboratory databases from 6 Italian Local Health Units (LHUs) was performed. Individuals receiving either an HIV treatment [Antiretroviral therapy (ART) – ATC code: J05A)], or with an HIV positive laboratory test result between January 1st, 2014 and December 31st, 2014 were included. The date of first ART prescription or positive test of HIV was used as the Index Date (ID). Patients enrolled were followed-up for all time available from the ID (follow-up period) and their clinical characteristics were investigated from one year prior to the ID (characterization period). Comorbidities were measured by using the Charlson Comorbidity Index; findings were compared with those of a sample of the general population with the same age and sex distribution (OsMed 2015). Healthcare resource use and related cost was evaluated during the follow-up period.RESULTS: 1,214 patients were included, 837 were PLWHIV without any comorbidities and 377 were PLWHIV with at least one comorbidity. Mean prevalence of prescriptions for treatment of comorbidities was higher in the HIV-infected population than in the Italian general population. The annual healthcare cost of managing HIV patients with comorbidities, was significantly higher than that for patients without comorbidities (€ 10,615 vs. € 8,665, p < 0.001).CONCLUSIONS: Study results showed that 30% of PLWHIV had at least one comorbidity. The cost of managing PLWHIV who have comorbidities was significantly higher than that of managing PLWHIV without comorbidities. Our data confirm that care and treatment services should be adapted to address the specific needs of people living with both HIV and comorbidities

    Mayer-Rokitansky-K\ufcster-Hauser syndrome (MRKHS) is a rare congenital disorder characterised by variable aplasia of the uterus and the upper part of the vagina in women with a normal 46,XX karyotype and normal ovarian function. It may be isolated (type I) but it is more frequently associated with other congenital defects (type II), mainly renal and vertebral defects. Exact etiology remains unknown. Clinical presentation is primary amenorrhoea with normal development of secondary sexual characteristics and normal external genitalia. Ultrasound and MRI allow accurate identification of MRKHS. Diagnosis may be delayed even in cases with known renal or vertebral malformation and causes important psychological distress. The creation of a neovagina allows sexual intercourse; fertility is obviously compromised

    International audienceThe two-particle momentum correlation functions between charm mesons (D±\mathrm{D^{*\pm}} and D±\mathrm{D}^\pm) and charged light-flavor mesons (π±\pi^{\pm} and K±^{\pm}) in all charge-combinations are measured for the first time by the ALICE Collaboration in high-multiplicity proton-proton collisions at a center-of-mass energy of s=13\sqrt{s} =13 TeV. For DK\mathrm{DK} and DK\mathrm{D^*K} pairs, the experimental results are in agreement with theoretical predictions of the residual strong interaction based on quantum chromodynamics calculations on the lattice and chiral effective field theory. In the case of Dπ\mathrm{D}\pi and Dπ\mathrm{D^*}\pi pairs, tension between the calculations including strong interactions and the measurement is observed. For all particle pairs, the data can be adequately described by Coulomb interaction only, indicating a shallow interaction between charm and light-flavor mesons. Finally, the scattering lengths governing the residual strong interaction of the Dπ\mathrm{D}\pi and Dπ\mathrm{D^*}\pi systems are determined by fitting the experimental correlation functions with a model that employs a Gaussian potential. The extracted values are small and compatible with zero

    International audienceMeasurements of the pTp_{\rm T}-dependent flow vector fluctuations in Pb-Pb collisions at sNN=5.02 TeV\sqrt{s_{_{\rm NN}}} = 5.02~\mathrm{TeV} using azimuthal correlations with the ALICE experiment at the LHC are presented. A four-particle correlation approach [1] is used to quantify the effects of flow angle and magnitude fluctuations separately. This paper extends previous studies to additional centrality intervals and provides measurements of the pTp_{\rm T}-dependent flow vector fluctuations at sNN=5.02 TeV\sqrt{s_{_{\rm NN}}} = 5.02~\mathrm{TeV} with two-particle correlations. Significant pTp_{\rm T}-dependent fluctuations of the V2\vec{V}_{2} flow vector in Pb-Pb collisions are found across different centrality ranges, with the largest fluctuations of up to \sim15% being present in the 5% most central collisions. In parallel, no evidence of significant pTp_{\rm T}-dependent fluctuations of V3\vec{V}_{3} or V4\vec{V}_{4} is found. Additionally, evidence of flow angle and magnitude fluctuations is observed with more than 5σ5\sigma significance in central collisions. These observations in Pb-Pb collisions indicate where the classical picture of hydrodynamic modeling with a common symmetry plane breaks down. This has implications for hard probes at high pTp_{\rm T}, which might be biased by pTp_{\rm T}-dependent flow angle fluctuations of at least 23% in central collisions. Given the presented results, existing theoretical models should be re-examined to improve our understanding of initial conditions, quark--gluon plasma (QGP) properties, and the dynamic evolution of the created system
