Phthalates and type 1 diabetes: is there any link?
Phthalates are a group of chemical compounds used as plasticizers in the manufacture of plastic materials. They can be present in many commonly used products. There seems to be a relationship between exposure to phthalates and the occurrence of metabolic dysfunctions, such as a decrease in glucose tolerance, oxidative stress, loss of beta cells, and a decrease in insulin synthesis. As beta cells play a key role in the onset of type 1 diabetes mellitus (T1DM), we sought to investigate the relationship between exposure to phthalates and the diagnosis of T1DM in prepubertal children. Design concentrations of phthalate metabolites were compared in the urine of a population of prepubertal children with new-onset diabetes, patients with T1DM diagnosed more than 6Â months previously, and healthy control children. Although the concentrations of DBP and DiBP metabolites were statistically identical in the new-onset diabetes, diabetes, and control groups, there was a clear trend for higher levels of DiBP metabolites in the children with new-onset diabetes. Liver dysfunction and hypoglycemia as presentation of hypopituitarism in a child
In the presence of hypoglycemia and liver dysfunction in the neonatal period and/or during the first months of life, a myriad of etiologies should be considered. We herein report the case of a child with refractory hypoglycemia and severe liver dysfunction, highlighting the importance of a multidisciplinary and rapid investigation to obtain the final diagnosis
Uso de Hormona de Crescimento em Crianças com Baixa Estatura Idiopática
A baixa estatura Ă© uma das causas mais frequentes de consulta de Pediatria e de Endocrinologia Pediátrica. SĂŁo variadas as causas que podem diminuir a velocidade de crescimento e originar uma baixa estatura. Por vezes nĂŁo Ă© possĂvel encontrar anomalias e nestas condições coloca-se o diagnĂłstico de exclusĂŁo de baixa estatura idiopática. Este grupo nosolĂłgico engloba uma variedade de situações, nele se incluindo muitas das variantes “normais” do crescimento como a baixa estatura familiar e atraso constitucional do crescimento. A hormona do crescimento produzida laboratorialmente por biotecnologia está disponĂvel em quantidades praticamente ilimitadas e as indicações para a sua utilização sĂŁo cada vez mais alargadas. Algumas crianças com baixa estatura idiopática podem beneficiar deste tipo de tratamento e assim aumentar a sua estatura final. A baixa estatura nĂŁo Ă© “per si”uma doença incapacitante e nĂŁo está provado que o seu tratamento se associe a uma melhoria da qualidade de vida. A pressĂŁo comercial, social e cientĂfica numa Ă©poca de abundância de fármacos mas de escassez de recursos econĂłmicos e financeiros, coloca questões de Ă©tica mĂ©dica e de justiça social que deve ser motivo de reflexĂŁo para adequada e ponderada actuação
