    Evaluation of Fresh Frozen Plasma Usage at the University Hospital Center Rijeka

    The aim of this investigation was to assess the rate and appropriate use of fresh frozen plasma (FFP) at the Clinical Hospital Center Rijeka in two six-month periods, at an interval of 5 years, before and after introducing clinical standards for transfusion practice and hemovigilance, and to evaluate the results of applied measures. We studied 315 patients transfused with 1341 doses of FFP in two six-month periods from September to February of 1999/2000 and 2004/2005. The first period (1999/00) of the study was retrospective and 226 patients were transfused with 928 FFP units. The second period (2004/05) was prospective and we studied 89 patients transfused with 413 units. In the first period blood bank records were retrospectively reviewed and in the second period all FFP requests, performed coagulation tests and transfusion episodes were prospectively analyzed. The number of inappropriate transfusions decreased from 39.8% to 23.6%. In most patients (85.1%), coagulation tests were made prior to FFP transfusion. The number of patients transfused with one and two FFP doses decreased, while those transfused with three or more doses increased. Most of the appropriately transfused patients were those with active bleeding due to coagulation factor deficiency and massive transfusions. The least were those requiring reversal of warfarin effect. Our results demonstrated a decrease in the number of patients treated with FFP in the second period. The introduction of clinical standards of good transfusion practice and hemovigilance showed positive effects. Considering that there was a number of inappropriately transfused patients continued education of all health personnel engaged in transfusion treatment is evidently necessary

    Examining the Presence of ABO(H) Antigens of Blood Types in the Saliva of Patients with Oral Cancer

    Number of researches dealing with the influence of the ABO blood group antigens on the development of the oral cancer have hypothesized that people who do not secrete these substances in the saliva are more prone to suffer from this disease. The objective of this research is to examine this hypothesis. In total 114 subjects were examined, half of which suffered from oral cancer, while the other half was the healthy control group. All examinees were subjected to clinical examinations and the experimental group to pathohistological examination. An analysis of the secretor status was carried out using the Wiener agglutination test. The experimental group consisted of 78,95% of secretors, while the control group consisted of 82,46% of secretors. This difference is not statistically significant. The starting hypothesis that non-secretors are more prone to the development of oral cancer was not confirmed

    Smjernice Hrvatskog druơtva za transfuzijsku medicinu za određivanje Rh(D) krvne grupe i primjenu RhD genotipizacije

    SAĆœETAK Radna skupina Hrvatskog druĆĄtva za transfuzijsku medicinu pripremila je smjernice za određivanje Rh(D) krvne grupe i primjenu RhD genotipizacije. U smjernicama je opisan klinički značaj antigena D, povijest i ograničenja seroloĆĄkog testiranja antigena D te mogućnosti RhD genotipizacije. Cilj smjernica bio je objava novih postupnika seroloĆĄkog određivanja Rh(D) krvne grupe bolesnicima, trudnicama i novorođenčadi s uputama za specijaliste transfuzijske medicine u hitnom i redovnom radu te tumačenjima nalaza namijenjenim ginekolozima, pedijatrima, neonatolozima, anesteziolozima, internistima, liječnicima obiteljske medicine te svim liječnicima koji se u svom radu susreću s bolesnicima koji primaju krvne pripravke i donose odluku o primjeni RhIG imunoprofilakse. Kao rezultat provođenja smjernica predviđeno je praćenje i periodično izvjeĆĄtavanje u slučaju sumnje na RhD imunizaciju kod osoba nositelja D-varijante. Tijekom trudnoće postoji i mogućnost neinvazivnog određivanja prijenatalnog fetalnog RhD genotipa iz majčine plazme iza 16. tjedna gestacije, kao vaĆŸnog alata u procjeni rizika razvoja hemolitičke bolesti fetusa i novorođenčeta. Radi lakĆĄeg snalaĆŸenja navedene su vrste spremnika za uzorkovanje krvi i potrebna količina uzoraka. Navedena pretraga dostupna je u Hrvatskom zavodu za transfuzijsku medicinu na zahtjev ginekologa, a preporučuje se prvenstveno RhD imuniziranim trudnicama te u slučaju donoĆĄenja odluke o ranoj prijenatalnoj anti-D imunoprofilaksi i svim Rh(D) negativnim neimuniziranim trudnicama

    Background Current management practices and outcomes in weaning from invasive mechanical ventilation are poorly understood. We aimed to describe the epidemiology, management, timings, risk for failure, and outcomes of weaning in patients requiring at least 2 days of invasive mechanical ventilation. Methods WEAN SAFE was an international, multicentre, prospective, observational cohort study done in 481 intensive care units in 50 countries. Eligible participants were older than 16 years, admitted to a participating intensive care unit, and receiving mechanical ventilation for 2 calendar days or longer. We defined weaning initiation as the first attempt to separate a patient from the ventilator, successful weaning as no reintubation or death within 7 days of extubation, and weaning eligibility criteria based on positive end-expiratory pressure, fractional concentration of oxygen in inspired air, and vasopressors. The primary outcome was the proportion of patients successfully weaned at 90 days. Key secondary outcomes included weaning duration, timing of weaning events, factors associated with weaning delay and weaning failure, and hospital outcomes. This study is registered with ClinicalTrials.gov, NCT03255109. Findings Between Oct 4, 2017, and June 25, 2018, 10 232 patients were screened for eligibility, of whom 5869 were enrolled. 4523 (77·1%) patients underwent at least one separation attempt and 3817 (65·0%) patients were successfully weaned from ventilation at day 90. 237 (4·0%) patients were transferred before any separation attempt, 153 (2·6%) were transferred after at least one separation attempt and not successfully weaned, and 1662 (28·3%) died while invasively ventilated. The median time from fulfilling weaning eligibility criteria to first separation attempt was 1 day (IQR 0–4), and 1013 (22·4%) patients had a delay in initiating first separation of 5 or more days. Of the 4523 (77·1%) patients with separation attempts, 2927 (64·7%) had a short wean (≀1 day), 457 (10·1%) had intermediate weaning (2–6 days), 433 (9·6%) required prolonged weaning (≄7 days), and 706 (15·6%) had weaning failure. Higher sedation scores were independently associated with delayed initiation of weaning. Delayed initiation of weaning and higher sedation scores were independently associated with weaning failure. 1742 (31·8%) of 5479 patients died in the intensive care unit and 2095 (38·3%) of 5465 patients died in hospital. Interpretation In critically ill patients receiving at least 2 days of invasive mechanical ventilation, only 65% were weaned at 90 days. A better understanding of factors that delay the weaning process, such as delays in weaning initiation or excessive sedation levels, might improve weaning success rates

    Geoeconomic variations in epidemiology, ventilation management, and outcomes in invasively ventilated intensive care unit patients without acute respiratory distress syndrome: a pooled analysis of four observational studies

    Background: Geoeconomic variations in epidemiology, the practice of ventilation, and outcome in invasively ventilated intensive care unit (ICU) patients without acute respiratory distress syndrome (ARDS) remain unexplored. In this analysis we aim to address these gaps using individual patient data of four large observational studies. Methods: In this pooled analysis we harmonised individual patient data from the ERICC, LUNG SAFE, PRoVENT, and PRoVENT-iMiC prospective observational studies, which were conducted from June, 2011, to December, 2018, in 534 ICUs in 54 countries. We used the 2016 World Bank classification to define two geoeconomic regions: middle-income countries (MICs) and high-income countries (HICs). ARDS was defined according to the Berlin criteria. Descriptive statistics were used to compare patients in MICs versus HICs. The primary outcome was the use of low tidal volume ventilation (LTVV) for the first 3 days of mechanical ventilation. Secondary outcomes were key ventilation parameters (tidal volume size, positive end-expiratory pressure, fraction of inspired oxygen, peak pressure, plateau pressure, driving pressure, and respiratory rate), patient characteristics, the risk for and actual development of acute respiratory distress syndrome after the first day of ventilation, duration of ventilation, ICU length of stay, and ICU mortality. Findings: Of the 7608 patients included in the original studies, this analysis included 3852 patients without ARDS, of whom 2345 were from MICs and 1507 were from HICs. Patients in MICs were younger, shorter and with a slightly lower body-mass index, more often had diabetes and active cancer, but less often chronic obstructive pulmonary disease and heart failure than patients from HICs. Sequential organ failure assessment scores were similar in MICs and HICs. Use of LTVV in MICs and HICs was comparable (42·4% vs 44·2%; absolute difference -1·69 [-9·58 to 6·11] p=0·67; data available in 3174 [82%] of 3852 patients). The median applied positive end expiratory pressure was lower in MICs than in HICs (5 [IQR 5-8] vs 6 [5-8] cm H2O; p=0·0011). ICU mortality was higher in MICs than in HICs (30·5% vs 19·9%; p=0·0004; adjusted effect 16·41% [95% CI 9·52-23·52]; p<0·0001) and was inversely associated with gross domestic product (adjusted odds ratio for a US$10 000 increase per capita 0·80 [95% CI 0·75-0·86]; p<0·0001). Interpretation: Despite similar disease severity and ventilation management, ICU mortality in patients without ARDS is higher in MICs than in HICs, with a strong association with country-level economic status
