    Impact of a COVID-19 Outbreak in an Elderly Care Home after Primary Vaccination

    [EN] Elderly care home residents are particularly vulnerable to COVID-19 due to immunesenescence, pre-existing medical conditions, and the risk of transmission from staff and visitors. This study aimed to describe the outcomes of a COVID-19 outbreak in a long-term care facility for elderly persons following the initial vaccination. A single-center, retrospective, observational design was used to analyze the variables associated with hospitalization and death rate by logistic regression. Adjusted odds ratios (aOR) and their 95% confidence intervals (CI) were calculated. Sixty-eight residents received the first dose of the COVID-19 vaccine. Despite being negative six days after vaccination, the performance of a second test 4 days later revealed 51 positives (75.0%) among residents and 18 among workers (56.3%). A total of 65 of the 68 residents (95.58%) had positive results with symptoms, whereas 34.9% required hospitalization, and 25.8% died. The best-fitting model to explain the distribution of cases reflects three points at the time of infection.. The time from vaccination to symptom onset explains the hospitalization and mortality rates since a day elapsed halves the risk of hospitalization (aOR = 0.57; CI = 0.38−0.75) and the risk of death by a quarter (aOR = 0.74; CI = 0.63−0.88). Nursing homes present an elevated risk of transmission and severity of SARS-CoV-2 infection. Although vaccination reduces the risk of hospitalization and death, extreme prevention and control measures are essential in these institutions despite the high vaccination coverage.S

    Black Hole Astrophysics in AdS Braneworlds

    We consider astrophysics of large black holes localized on the brane in the infinite Randall-Sundrum model. Using their description in terms of a conformal field theory (CFT) coupled to gravity, deduced in Ref. [1], we show that they undergo a period of rapid decay via Hawking radiation of CFT modes. For example, a black hole of mass few×M{\rm few} \times M_\odot would shed most of its mass in 104105\sim 10^4 - 10^5 years if the AdS radius is L101L \sim 10^{-1} mm, currently the upper bound from table-top experiments. Since this is within the mass range of X-ray binary systems containing a black hole, the evaporation enhanced by the hidden sector CFT modes could cause the disappearance of X-ray sources on the sky. This would be a striking signature of RS2 with a large AdS radius. Alternatively, for shorter AdS radii, the evaporation would be slower. In such cases, the persistence of X-ray binaries with black holes already implies an upper bound on the AdS radius of L \la 10^{-2} mm, an order of magnitude better than the bounds from table-top experiments. The observation of primordial black holes with a mass in the MACHO range M0.10.5MM \sim 0.1 - 0.5 M_\odot and an age comparable to the age of the universe would further strengthen the bound on the AdS radius to L \la {\rm few} \times 10^{-6} mm.Comment: 14 pages, latex, no figures v2: added reference

    The rms-flux relations in different branches in Cyg X-2

    In this paper, the rms-flux (root mean square-flux) relation along the Z-track of the bright Z-Source Cyg X-2 is analyzed using the observational data of Rossi X-ray Timing Explorer (RXTE). Three types of rms-flux relations, i.e. positive, negative, and 'arch'-like correlations are found in different branches. The rms is positively correlated with flux in normal branch (NB), but anti-correlated in the vertical horizontal branch (VHB). The rms-flux relation shows an 'arch'-like shape in the horizontal branch (HB).     International nosocomial infection control consortium (INICC) report, data summary of 36 countries, for 2004-2009

    The results of a surveillance study conducted by the International Nosocomial Infection Control Consortium (INICC) from January 2004 through December 2009 in 422 intensive care units (ICUs) of 36 countries in Latin America, Asia, Africa, and Europe are reported. During the 6-year study period, using Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN; formerly the National Nosocomial Infection Surveillance system [NNIS]) definitions for device-associated health care-associated infections, we gathered prospective data from 313,008 patients hospitalized in the consortium's ICUs for an aggregate of 2,194,897 ICU bed-days. Despite the fact that the use of devices in the developing countries' ICUs was remarkably similar to that reported in US ICUs in the CDC's NHSN, rates of device-associated nosocomial infection were significantly higher in the ICUs of the INICC hospitals; the pooled rate of central line-associated bloodstream infection in the INICC ICUs of 6.8 per 1,000 central line-days was more than 3-fold higher than the 2.0 per 1,000 central line-days reported in comparable US ICUs. The overall rate of ventilator-associated pneumonia also was far higher (15.8 vs 3.3 per 1,000 ventilator-days), as was the rate of catheter-associated urinary tract infection (6.3 vs. 3.3 per 1,000 catheter-days). Notably, the frequencies of resistance of Pseudomonas aeruginosa isolates to imipenem (47.2% vs 23.0%), Klebsiella pneumoniae isolates to ceftazidime (76.3% vs 27.1%), Escherichia coli isolates to ceftazidime (66.7% vs 8.1%), Staphylococcus aureus isolates to methicillin (84.4% vs 56.8%), were also higher in the consortium's ICUs, and the crude unadjusted excess mortalities of device-related infections ranged from 7.3% (for catheter-associated urinary tract infection) to 15.2% (for ventilator-associated pneumonia).     En el proceso de determinación de redes de intercambiadores de calor, las técnicas empleadas habitualmente exigen una primera etapa de determinación de la Máxima Energía Recuperable. Para ello, se han descrito diversas técnicas basadas en métodos termodinámicos como el procedimiento de la tabla del problema (Pinch), y métodos matemáticos que empleando el modelo de transporte en etapas, se resuelven utilizando el método Simplex para Redes o el método Out of Kilter (OKA), basados en técnicas de programación lineal. El método de la tabla del problema presenta ventajas obvias desde el punto de vista expositivo; sin embargo, de cara a la generalización del mismo con inclusión de utilidades externas múltiples, resulta de difícil adaptación. Los métodos matemáticos, por su parte, aunque de mayor complejidad en su planteamiento, permiten abordar la inclusión de utilidades múltiples con modificaciones mínimas. A fin de explorar las posibilidades de dos de las técnicas matemáticas más actuales y exponer los resultados de su aplicación, se presenta el actual trabajo

    Implementation of a program of social and emotional skills for people with myasthenia. The aim of this study was to evaluate the efficacy of a program for persons with myasthenia. The participants were 56 adults diagnosed of myasthenia. The Program was applied on line, during eleven weeks; this program works on assertiveness, communication, social skills, resilience and emotional intelligence. A single group was used with pre and post intervention. At post-treatment, different aspects related with the quality of life of the patients showed significant improvement, anxiety diminished and social skills increase. Besides, the program proved useful for both men and women at any age. These results initially endorsed the efficacy of the program employed and support psychosocial intervention with these patient
