    Endocervical glandular neoplasia associated with lobular endocervical glandular hyperplasia is HPV-independent and correlates with carbonic anhydrase-IX expression: a Gynaecological Oncology Group Study.

    BackgroundLobular endocervical glandular hyperplasia (LEGH) is a rare lesion of the uterine cervix. It has been proposed that LEGH may represent a precursor lesion to a group of mucinous adenocarcinoma with gastric phenotype (GA) that is independent of high-risk human papillomavirus (H-HPV) infection. Carbonic anhydrase-IX (CA-IX) is highly expressed in conventional glandular lesions (CGLs). However, expression of CA-IX in LEGH or GA has not been studied.MethodsIn all, 12 CGLs, 7 LEGHs, 6 LEGHs with coexisting adenocarcinoma in situ (AIS, 3) and GA (3) were identified from Japanese women with a cytological diagnosis of atypical glandular cells of undetermined significance. Immunostaining was used to detect CA-IX and p16(INK)4(a) (hereafter termed p16) protein expression in the tissues and CA-IX protein expression in the Papanicolaou smears (PSs). Polymerase chain reaction was used to detect H-HPV DNA in liquid-based cytology.ResultsOut of 12 (83%) CGLs, 10 were positive with H-HPV and high levels of CA-IX expression were seen in all (100%) cases. P16 protein expression was observed in 11 out of 12 (92%) cases. None of the LEGHs, LEGHs with AIS or GA were positive for H-HPV and only 8 out of 13 (62%) showed focal weak (1+) p16 expression. In contrast, all cases (100%) exhibited strong CA-IX protein expression.ConclusionOur study suggests that there are different molecular mechanisms of carcinogenesis resulting in CGLs vs LEGHs associated with AIS or GA. There is also a possible link between LEGHs and GAs. Furthermore, CA-IX expression may serve as a useful biomarker for the detection of GAs in the absence of H-HPV infection

    Light thresholds to prevent dredging impacts on the great barrier reef seagrass, Zostera muelleri ssp. capricorni

Coastal seagrass habitats are at risk from a range of anthropogenic activities that modify the natural light environment, including dredging activities associated with coastal and port developments. On Australia's east coast, the tropical seagrass Zostera muelleri ssp. capricorni dominates intertidal mudbanks in sheltered embayments which are also preferred locations for harbors and port facilities. Dredging to establish and maintain shipping channels in these areas can degrade water quality and diminish light conditions that are required for seagrass growth. Based on this potential conflict, we simulated in-situ light attenuation events to measure effects on Z. muelleri ssp. capricorni condition. Semi-annual in situ shading studies conducted over 3 years were used to quantify the impact of prolonged light reduction on seagrass morphometrics (biomass, percent cover, and shoot density). Experimental manipulations were complimented with an assessment of 46 months of light history and concurrent natural seagrass change at the study site in Gladstone Harbour. There was a clear light-dependent effect on seagrass morphometrics during seagrass growing seasons, but no effect during senescent periods. Significant seagrass declines occurred between 4 and 8 weeks after shading during the growing seasons with light maintained in the range of 4-5 mol photons m-2 d-1. Sensitivity to shading declined when applied in 2-week intervals (fortnightly) rather than continuous over the same period. Field observations were correlated to manipulative experiments to derive an applied threshold of 6 mol photons m-2 d-1 which formed the basis of a reactive light-based management strategy which has been successfully implemented to ensure positive ecological outcomes for seagrass during a large-scale dredging program

    Public experiences of mass casualty decontamination

    In this article, we analyze feedback from simulated casualties who took part in field exercises involving mass decontamination, to gain an understanding of how responder communication can affect peopleā€™s experiences of and compliance with decontamination. We analyzed questionnaire data gathered from 402 volunteers using the framework approach, to provide an insight into the publicā€™s experiences of decontamination and how these experiences are shaped by the actions of emergency responders. Factors that affected casualtiesā€™ experiences of the econtamination process included the need for greater practical information and better communication from responders, and the need for privacy. Results support previous findings from small-scale incidents that involved decontamination in showing that participants wanted better communication from responders during the process of decontamination, including more practical information, and that the failure of responders to communicate effectively with members of the public led to anxiety about the decontamination process. The similarity between the findings from the exercises described in this article and previous research into real incidents involving decontamination suggests that field exercises provide a useful way to examine the effect of responder communication strategies on the publicā€™s experiences of decontamination. Future exercises should examine in more detail the effect of various communication strategies on the publicā€™s experiences of decontamination. This will facilitate the development of evidence-based communication strategies intended to reduce anxiety about decontamination and increase compliance among members of the public during real-life incidents that involve mass decontamination

    Prevalence of anaemia in patients attending an outpatient clinic in Western Rift valley in Kenya during a low malaria season

    Objective: To assess the prevalence of anaemia in outpatients attending a rural health clinic in an area of seasonal malaria, during the low transmission season. Methods: Haemoglobin estimation and blood slide examination for malaria parasites were performed on 280 consecutive patients attending outpatient curative services at Entasopia Health Centre, Kajiado District, Kenya, between April-May 1996. Anaemia was definedĀ according to World Health Organisation guidelines for age, sex and pregnancy status. Results: In all groups except adult males, more than half of the patients tested had haemoglobin values below the lower reference limits, suggesting that anaemia is widely present in this population even during the low malaria season. Only 5% of patients were positive for Plasmodium fakiparum malaria. Peripheral blood film examination suggested iron deficiency as the major cause of anaemia. Conclusions: Further studies to define the underlying causes of anaemia and to develop community strategies to prevent anaemia are required. The association between fever and anaemia and the use of pallor to diagnose anaemia, are discussed.East African Medical Journal, May 1999, 251-25

    Associations between reliable changes in depression and changes in BMI, total body fatness and visceral adiposity during a 12-month weight loss trial.

    We investigated associations between changes in depression and body composition over a 12-month weight loss trial. Of the 298 adults (BMIā€‰>ā€‰27ā€‰m/kg2), 219 with complete depression and body composition data were included. A 10-item Center for Epidemiologic Studies Depression Scale measured depression; dual-energy X-ray absorptiometry measured body composition. Multinomial logistic regression predicted reliable changes in depression by BMI, body fat (BF) and visceral adiposity (VAT). Multiplicative interaction terms tested modification by sex and ethnicity. Participants with increases in body composition were less likely to experience improvements in depression (BMI: RRRā€‰=ā€‰0.79 (0.68-0.91), pā€‰<ā€‰0.01; BF: RRRā€‰=ā€‰0.97 (0.94 - 0.99), pā€‰=ā€‰0.01; VAT: RRRā€‰=ā€‰0.99 (0.98-1.00), pā€‰=ā€‰0.02), but not worsening of depression (BMI: RRRā€‰=ā€‰1.29 (0.96-1.73), pā€‰=ā€‰0.10; BF: RRRā€‰=ā€‰1.04 (0.99-1.09), pā€‰=ā€‰0.15; VAT: RRRā€‰=ā€‰1.01 (1.00-1.03), pā€‰=ā€‰0.18). Sex and ethnicity interaction terms were not significant. However, the relationship was only significant among females, among non-Latinos for BMI and BF, and among Latinos for VAT. Our study supports the association between depression and obesity and highlights the need for longitudinal studies investigating VAT and depression in diverse ethnic groups

    The effect of age on emotion processing in individuals with mood disorders and in healthy individuals

Introduction: Emotion processing is an essential part of interpersonal relationships and social interactions. Changes in emotion processing have been found in both mood disorders and in aging, however, the interaction between such factors has yet to be examined in detail. This is of interest due to the contrary nature of the changes observed in existing research - a negativity bias in mood disorders versus a positivity effect with aging. It is also unclear how changes in non-emotional cognitive function with aging and in mood disorders, interact with these biases. Methods and results: In individuals with mood disorders and in healthy control participants, we examined emotional processing and its relationship to age in detail. Data sets from two studies examining facial expression recognition were pooled. In one study, 98 currently depressed individuals (either unipolar or bipolar) were compared with 61 healthy control participants, and in the other, 100 people with bipolar disorder (in various mood states) were tested on the same facial expression recognition task. Repeated measures analysis of variance was used to examine the effects of age and mood disorder diagnosis alongside interactions between individual emotion, age, and mood disorder diagnosis. A positivity effect was associated with increasing age which was evident irrespective of the presence of mood disorder or current mood episode. Discussion: Results suggest a positivity effect occurring at a relatively early age but with no evidence of a bias toward negative emotions in mood disorder or specifically, in depressed episodes. The positivity effect in emotional processing in aging appears to occur even within people with mood disorders. Further research is needed to understand how this fits with negative biases seen in previous studies in mood disorders

    Alloactivation of naĆÆve CD4<sup>+</sup> CD8<sup>āˆ’</sup> CD25<sup>+</sup>T regulatory cells: Expression of CD8Ī± identifies potent suppressor cells that can promote transplant tolerance induction

    Therapy with alloantigen-specific CD4+ CD25+ T regulatory cells (Treg) for induction of transplant tolerance is desirable, as naĆÆve thymic Treg (tTreg) are not alloantigen-specific and are weak suppressor cells. NaĆÆve tTreg from DA rats cultured with fully allogeneic PVG stimulator cells in the presence of rIL-2 express IFN-gamma receptor (IFNGR) and IL-12 receptor beta2 (IL-12RĪ²2) and are more potent alloantigen-specific regulators that we call Ts1 cells. This study examined additional markers that could identify the activated alloantigen-specific Treg as a subpopulation within the CD4+ CD25+ Foxp3+ Treg. After culture of naĆÆve DA CD4+ CD8āˆ’ CD25+ T cells with rIL-2 and PVG alloantigen, or rIL-2 without alloantigen, CD8Ī± was expressed on 10ā€“20% and CD8Ī² on <5% of these cells. These cells expressed ifngr and Il12rb2. CD8Ī±+ cells had increased Ifngr that characterizes Ts1 cells as well was Irf4, a transcription factor induced by TCR activation. Proliferation induced by re-culture with rIL-12 and alloantigen was greater with CD4+ CD8Ī±+ CD25+ Treg consistent with the CD8Ī±+ cells expressing IL-12R. In MLC, the CD8Ī±+ fraction suppressed responses against allogeneic stimulators more than the mixed Ts1 population, whereas the CD4+ CD8āˆ’ CD25+ T cells were less potent. In an adoptive transfer assay, rIL-2 and alloantigen activated Treg suppress rejection at a ratio of 1:10 with naĆÆve effector cells, whereas alloantigen and rIL-2 activated tTreg depleted of the CD8Ī±+ cells were much less effective. This study demonstrated that expression of CD8Ī± by rIL-2 and alloantigen activation of CD4+ CD8āˆ’ CD25+ Foxp3+ T cells was a marker of activated and potent Treg that included alloantigen-specific Treg
