    Measuring the effects of the September 11 attack on New York City

    The attack on the World Trade Center had an enormous financial, as well as emotional, impact on New York City. This article measures the short-term economic effects on the city's labor force and capital stock through June 2002, the end of the recovery process at the World Trade Center site. Using a lifetime-earnings loss concept, the authors estimate that the nearly 3,000 workers killed in the attack lost 7.8billioninprospectiveincome.Moreover,theemploymentimpactinthekeyaffectedsectors−suchasfinance,airtransportation,hotels,andrestaurants−translatedintoanestimatedearningsshortfallof7.8 billion in prospective income. Moreover, the employment impact in the key affected sectors - such as finance, air transportation, hotels, and restaurants - translated into an estimated earnings shortfall of 3.6 billion to 6.4billion,whilethecostofrepairingandreplacingthedamagedphysicalcapitalstockandinfrastructuretotaledanestimated6.4 billion, while the cost of repairing and replacing the damaged physical capital stock and infrastructure totaled an estimated 21.6 billion. Accordingly, the authors determine that the total attack-related cost to New York City through June 2002 was between 33billionand33 billion and 36 billion.     Developing a Novel Web-Based Self-Management Support Intervention for Polycystic Ovary Syndrome:Mixed Methods Study With Patients and Health Care Professionals

    Background: Polycystic ovary syndrome (PCOS) represents a significant global health burden requiring urgent attention. This common chronic endocrine and cardiometabolic condition affects around 1 in 10 women and individuals assigned female at birth, with significant adverse effects on well-being, quality of life, and mental health, as well as serious and complex long-term health consequences. International guidelines for best health care practice recommend the provision of comprehensive cognitive behavioral interventions to support self-management and improve health outcomes for those living with PCOS. Web-based health interventions have the potential to meet this need in an accessible and scalable way.Objective: We aim to identify barriers to self-management and psychological well-being in women with PCOS and adapt a web-based self-management program to provide a prototype digital support intervention for them.Methods: We adapted an existing support program (HOPE) for PCOS using the antecedent target measure approach. We conducted qualitative interviews with 13 adult women living with PCOS, 3 trustees of a patients with PCOS advocacy charity, and 4 endocrinologists to identify “antecedents” (barriers) to self-management and psychological well-being. Framework analysis was used to identify potentially modifiable antecedents to be targeted by the novel intervention. At a national conference, 58 key stakeholders (patients and health professionals) voted for the antecedents they felt were most important to address. We used research evidence and relevant theory to design a prototype for the PCOS intervention.Results: Voting identified 32 potentially modifiable antecedents, relating to knowledge, understanding, emotions, motivation, and behaviors, as priorities to be targeted in the new intervention. A modular, web-based prototype HOPE PCOS intervention was developed to address these, covering six broad topic areas (instilling HOPE for PCOS; managing the stress of PCOS; feeding your mind and body well; body image, intimacy, and close relationships; staying healthy with PCOS; and keeping PCOS in its place).Conclusions: We identified barriers to self-management and psychological well-being in women with PCOS and used these to adapt a web-based self-management program, tailoring it for PCOS, which is a comprehensive group intervention combining education, empowerment, lifestyle management, peer support with cognitive behavioral tools, and goal-setting (to be delivered by peers or codelivered with health care professionals). The modular structure offers flexibility to adapt the program further as new clinical recommendations emerge. The intervention has the potential to be delivered, evaluated for feasibility, and, if effective, integrated into health care services. Self-management interventions are not designed to replace clinical care; rather, they serve as an additional source of support. The HOPE PCOS program conveys this message in its content and activities. Future research should evaluate the prototype intervention using primary outcomes such as measures of psychological well-being, self-management self-efficacy, depression, anxiety, and PCOS-related quality of life. They should also assess the intervention’s acceptability, scalability, and cost-effectiveness

    Student Perceptions Of The Value Of A University Laptop Program

    In the fall of 2000, when a mandatory laptop program was initiated at a Midwest regional university, a faculty research team began a longitudinal study to determine student perceptions of the value of the program. Value was defined as usefulness of the computer, reasonable price for the computer and the quality of the computer. A survey of 1,000 first-time full time university students were surveyed in 2000 and five years later in the fall of 2005. Significant differences were found on all three variables indicating that student perceptions of the usefulness, price, and quality of the computer increased from 2000 to 2005. Based on these findings, recommendations are offered to administrators and faculty who may be considering the adoption of a computer requirement

    Employer Perceptions Of Student Informational Interviewing Skills And Behaviors

    Employers continue to report that soft skills are critically important in obtaining employment and achieving long-term career success. Given the challenging job market for college graduates, business school faculty need to provide practical opportunities for students to develop their soft skills in professional settings. A longitudinal study was conducted at a Midwestern university’s AACSB accredited College of Business to determine the soft skills that employers believe students need to improve on to be successful. This paper shares 1) a management-oriented learning assignment that combines soft skill learning and application with professionals in work situations and 2) results of the professionals’ perceptions of the students’ soft skills. Results of this research will help faculty assess and improve their instructional methodology to better prepare students to meet employer expectations

    Maternal assault admissions are associated with increased risk of child protection involvement

    Introduction Almost half of Australian women assaulted by their partner have children in their care. Evidence suggests a link between children’s exposure to violence, and subsequent maltreatment allegations. However, this evidence is limited by small sample sizes. Linked administrative data present an opportunity to further investigate this sensitive topic. Objectives and Approach This study investigated the relationship between assaults on mothers and subsequent child maltreatment allegations. The sample included all live births in Western Australia from 1990 to 2009 (N=524,534) and their parents, with follow up to 2013. Linked administrative data on child maltreatment allegations and mothers’ assault-related hospital admissions were obtained. Multivariate Cox regression estimated the risk of maltreatment allegation following maternal assault admission. Adjusted and unadjusted hazard ratios (HR) and 95\% confidence intervals (CI) were calculated for the risk of maltreatment allegation, and time (in months) between assault admission and the first maltreatment allegation. Results One in five children whose mother had an assault admission had a subsequent maltreatment allegation, increasing to more than one in three children when restricted to assault admissions in the prenatal period. More than half of the children who had a maltreatment allegation after their mother was admitted for assault were Aboriginal. After adjusting for covariates, children whose mother had an assault admission had two-fold increased risk of having a maltreatment allegation. The risk of maltreatment allegation was greatest in young children, 5.5-year-old (SD=4.6), when restricted to maternal assault admissions in the prenatal period the children were younger at 4-year-old (SD=4.1). The time from maternal assault admission to maltreatment allegation was around 12 months longer for Aboriginal children than for non-Aboriginal children. Conclusion/Implications Children of mothers who have been assaulted are at higher risk of child maltreatment allegation. Targeted early intervention is required for families with young children, and pregnant women experiencing violence. The time to maltreatment allegation for Aboriginal children warrants community developed culturally-safe partnerships between Aboriginal communities and government services

    Training Veterans to Provide Peer Support in a Weight-Management Program: MOVE!

    IntroductionThe Veterans Health Administration (VHA) has implemented MOVE!, a weight-management program for veterans designed to address the increasing proportion of overweight and obese veterans. The objective of our study was to determine whether peer support employing motivational interviewing (MI) could positively influence lifestyle changes, thus expanding the reach of the MOVE! program. We describe the initial evaluation of the peer training program.MethodsWe developed an MI peer counselor training program for volunteer veterans, the “Buddies” program, to provide one-on-one telephone support for veterans enrolled in MOVE!. Buddies were recruited at 5 VHA sites and trained to provide peer support for the 6-month MOVE! intervention. We used a DVD to teach MI skills and followed with 2 to 3 booster sessions. We observed training, conducted pre- and posttraining surveys, and debriefed focus groups to assess training feasibility.ResultsFifty-six Buddies were trained. Results indicate positive receipt of the program (89% reported learning about peer counseling and 87% reported learning communication skills). Buddies showed a small improvement in MI self-efficacy on posttraining surveys. We also identified key challenges to learning MI and training implementation.ConclusionsMI training is feasible to implement and acceptable to volunteer Buddies. Trainers must assess how effectively volunteers learn MI skills in order to enhance its effective use in health promotion

    Checklist Model to Improve Work Practices in Small-Scale Demolition Operations with Silica Dust Exposures

    A systematic approach was developed to review, revise and adapt existing exposure control guidance used in developed countries for use in developing countries. One-page employee and multiple-page supervisor guidance sheets were adapted from existing documents using a logic framework and workers were trained to use the information to improve work practices. Interactive, hands-on training was delivered to 26 workers at five small-scale demolition projects in Maputo City, Mozambique, and evaluated. A pre-and-post walkthrough survey used by trained observers documented work practice changes. Worker feedback indicated that the training was effective and useful. Workers acquired knowledge (84% increase, p < 0.01) and applied the work practice guidance. The difference of proportions between use of work practice components before and after the intervention was statistically significant (p < 0.05). Changes in work practices following training included preplanning, use of wet methods and natural ventilation and end-of-task review. Respirable dust measurements indicated a reduction in exposure following training. Consistency in observer ratings and observations support the reliability and validity of the instruments. This approach demonstrated the short-term benefit of training in changing work practices; follow-up is required to determine the long-term impact on changes in work practices, and to evaluate the need for refresher training

    mTOR Kinase Inhibition Effectively Decreases Progression of a Subset of Neuroendocrine Tumors that Progress on Rapalog Therapy and Delays Cardiac Impairment

    Inhibition of mTOR signaling using the rapalog everolimus is an FDA-approved targeted therapy for patients with lung and gastroenteropancreatic neuroendocrine tumors (NET). However, patients eventually progress on treatment, highlighting the need for additional therapies. We focused on pancreatic NETs (pNET) and reasoned that treatment of these tumors upon progression on rapalog therapy, with an mTOR kinase inhibitor (mTORKi), such as CC-223, could overcome a number of resistance mechanisms in tumors and delay cardiac carcinoid disease. We performed preclinical studies using human pNET cells in vitro and injected them subcutaneously or orthotopically to determine tumor progression and cardiac function in mice treated with either rapamycin alone or switched to CC-223 upon progression. Detailed signaling and RNA sequencing analyses were performed on tumors that were sensitive or progressed on mTOR treatment. Approximately 57% of mice bearing pNET tumors that progressed on rapalog therapy showed a significant decrease in tumor volume upon a switch to CC-223. Moreover, mice treated with an mTORKi exhibited decreased cardiac dilation and thickening of heart valves than those treated with placebo or rapamycin alone. In conclusion, in the majority of pNETs that progress on rapalogs, it is possible to reduce disease progression using an mTORKi, such as CC-223. Moreover, CC-223 had an additional transient cardiac benefit on valvular fibrosis compared with placebo- or rapalog-treated mice. These results provide the preclinical rationale to further develop mTORKi clinically upon progression on rapalog therapy and to further test their long-term cardioprotective benefit in those NET patients prone to carcinoid syndrome

    Representational predicaments at three Hong Kong sites

    Representational predicaments arise when a job incumbent believes that attributions and images assumed by dominant authorities unfavourably ignore, or disproportionately and unfavourably emphasize, aspects of the incumbent\u27s own work and social identity. This is likely to happen when the incumbent does not have a close relationship with a dominant authority, and when power asymmetries give the former relatively little control over which aspects of their work and social identity are made visible or invisible to the latter. We draw on critical incident interviews from three organizations to illustrate a typology of six types of representational predicament: invasive spotlighting, idiosyncratic spotlighting, embedded background work, paradoxical social visibility, standardization of work processes, and standardization of work outputs. We analyse responses to representational predicaments according to whether they entailed exit, voice, loyalty, or neglect. Incumbents tended to respond with loyalty if they felt able and willing to accommodate their work behaviour and/or social identity to the dominant representations, and if there were sufficient compensatory factors, such as intrinsic rewards from the work or solidarity with colleagues. Exit or neglect appeared to reflect the belief that it was impossible to accommodate. Power asymmetries appeared to deter voice. Individual employees with a close and cordial working relationship with a member of a dominant authority group, or who were relationally networked to one, appeared not to experience representational predicaments
