    Four nurse educators went to Haiti after the 2012 earthquake to help rebuild the School of Nursing in Port-au-Prince. They were alarmed by the low level of health care in Haiti. Most people have no care, and those that do are treated by nurses who are not trained to do so. This article details the steps the nurses went through to start a family nurse practitioner program in Haiti to address the need for primary care and discusses how they created the partnerships necessary to start such an innovative program at the University of Haiti

    Osteogenic Regulatory Mechanisms Activated By Pressure In Aortic Heart Valve

    Calcific aortic valve disease (CAVD) is the most common cause of aortic valve failure and replacement in the elderly population, affecting 25% of the population over 65 years of age. Current pharmacological approaches for preventing the onset and progression of calcific aortic valve disease have not shown consistent benefits in clinical studies. Differentiation of valvular interstitial cells (VICs) into osteoblast–like cells is an integral step in the calcification process. Although clinical evidence suggests hypertension as a potential candidate contributing to the development of CAVD, the underlying molecular mechanisms that cause de-differentiation remain unclear. The present study investigates the role of elevated cyclic pressure in modulating osteoblast differentiation pathways in VICs in vitro. We used a combination of systems biology modeling and pathway-based analyses to identify novel genes and molecular mechanisms that are activated in valve tissue during exposure to elevated pressure conditions. Our results show that elevated pressure induces a gene expression pattern in valve tissue that is considerably similar to that seen in CAVD, underlining the key role of hypertension as an initiating factor in the onset of pathogenesis. In addition, our analysis revealed a set of genes that was not previously known to be regulated in valve tissue in a pressure dependent manner. Currently, the molecular mechanisms involved in CAVD and their associations with changes in local mechanical environment are poorly understood, and thus a better understanding of the cell based process mediating CAVD progression will improve our ability to develop potential medical therapies for this disease

    In this paper, Hispanic women entrepreneurs are examined through a variety of perspectives.  First, the analysis presented is an overview of miscellaneous characteristics, including financial information, age and marital status.  The analysis continues with a look at the relationships between human capital, financial capital and network structures for the Hispanic women entrepreneurs.  The study thus serves three purposes.  First, it explores in depth areas of women entrepreneurship that are not always given an extensive focus; namely human capital and network structures.  Second, it delineates how these dimensions are measured and are related to each other.  Finally, it adds to the limited body of research on Hispanic women entrepreneurs, providing additional illumination for this sector

    Impacto de la educación pre-quirúrgica impartida por el personal de enfermería a pacientes sometidos a cirugía cardiovascular

    En la Argentina, en el año 2009, las enfermedades cardiovasculares representaron la primera causa de hospitalización y muerte, con un 29%. Se observa, además, que es la primera causa de mortalidad en el grupo entre 45 y 65 años, según el Ministerio de Salud de la Nación, Dirección de Estadísticas e Información de Salud (DEIS). Si bien la medicina y las técnicas quirúrgicas han avanzado, el hecho de que el paciente se someta a una hospitalización e intervención cardíaca sin duda representa un evento de estrés y ansiedad muy intenso, haciendo más lenta y complicada la recuperación pos operatoria. Cuando se prepara psicológicamente a un paciente para una intervención quirúrgica, brindándole información, enseñando técnicas de relajación, escuchándole, atenuando sus miedos, la ansiedad disminuye, el individuo se encuentra en un estado de bienestar y control. Es en este momento cuando el personal de enfermería es de vital importancia. En él recae la responsabilidad de ayudar y educar al paciente y familiares; es quien está cerca y acompaña a transitar la etapa de la rehabilitación, el proceso del dolor, y también la muerte. La atención de enfermería se centra en torno a la valoración integral del paciente y su vigilancia continua proporcionando una asistencia de alta calidad profesional dialogando con él y su familia, para que todo quede bien en claro, sentando responsabilidades a cumplir en las etapas de la recuperación y también para que la familia pueda brindarle la atención debida en su casa. Se debe poner énfasis en estimular la confianza en el equipo asistencial y participación en el proceso de su recuperación para minimizar sus miedos, ayudarlo a tomar conciencia de ellos y así manejarlos de la manera más adecuada. Del mismo modo, incentivarlo estableciendo comunicación para esclarecer dudas y preguntas, brindando información adecuada y realista de su proceso. De esta manera el paciente tendrá seguridad y su calidad de vida mejorará siendo una persona productiva en cualquiera que fuera el área donde se desenvolvió previo a su enfermedad. Este trabajo de investigación tiene como objetivo determinar si la educación pre-quirúrgica que brinda el personal de enfermería a pacientes sometidos a cirugía cardiovascular favorece su evolución en el Servicio de Recuperación de Cirugía Cardiovascular del Hospital Central durante el período setiembre a noviembre del 2012.Fil: Carol, Andrea.     How to Build a Better Bar Exam

    As a licensing exam, the purpose of the bar exam is consumer protection–-ensuring that new lawyers have the minimum competencies required to practice law effectively. As critics point out, however, the exam, and particularly the multiple-choice question portion of the exam, has significant flaws because it assesses legal knowledge and analysis in an artificial and unrealistic context, and the closed-book format rewards the ability to memorize thousands of legal rules, a skill unrelated to law practice. This essay discusses how to improve the exam by changing its multiple-choice content and format. We use two law licensing exams to illustrate how bar examiners could utilize an open-book format and develop multiple-choice questions that assess a candidate’s ability to engage in legal reasoning and analysis without demanding unproductive memorization of so many detailed rules of law. The first example, the case file approach, is drawn from a 1983 California “Performance Test” in which test-takers received a case file and a series of multiple-choice questions testing the candidates’ ability to read, understand, and use cases to support their legal positions. The second example discusses the current licensing exam administered by The Law Society of Upper Canada (LSUC), an open-book multiple-choice exam that tests the use of doctrinal knowledge in the context of law practice. These two licensing exams demonstrate how we could re-structure the bar exam’s multiple-choice questions to measure legal analysis and reasoning skills as lawyers use those skills to represent clients. They also demonstrate that we can do a better job of testing some aspects of minimum competence, while still using a multiple-choice exam format
