    Monocyte-to-HDL Ratio (MHR) Predicts Vitamin D Deficiency in Healthy and Metabolic Women: A Cross-Sectional Study in 1048 Subjects

    Vitamin D deficiency is often linked with Metabolic Syndrome, both being more frequent with ageing and associated with an increase inflammatory state. Recently, monocytes-to-high density lipoprotein (HDL) ratio (MHR) has emerged as a powerful index to predict systemic inflammation. In this cross-sectional study, we investigated the association between circulating vitamin D level (25-OH vitamin D) and inflammatory status in a population of 1048 adult individuals. Our study reveals an inverse association between 25-OH vitamin D levels and MHR in the overall population. When the population is stratified by gender, waist circumference, and body mass index (BMI), we observed that while in men this relation is strongly significative only in condition of central obesity, in women a lifelong negative correlation exists between circulating 25-OH vitamin D and MHR and it is independent of the metabolic status. These observations underscore the relevance of circulating biomarkers such as MHR in the prediction of systemic inflammatory conditions sustained by vitamin D deficiency also in healthy and young women

    Development and Validation of an Italian Adaptation of the Psychosocial Aspects of Hereditary Cancer Questionnaire

    Individuals that attend cancer genetic counseling may experience test-related psychosocial problems that deserve clinical attention. In order to provide a reliable and valid first-line screening tool for these issues, Eijzenga and coworkers developed the Psychosocial Aspects of Hereditary Cancer (PAHC) questionnaire. The aim of this work was to develop an Italian adaptation of the PAHC (I-PACH). This prospective multicenter observational study included three stages: (1) development of a provisional version of the I-PAHC; (2) pilot studies aimed at testing item readability and revising the questionnaire; and (3) a main study aimed at testing the reliability and validity of the final version of the I-PAHC with the administration of a battery comprising measures of depression, anxiety, worry, stress, and life problems to 271 counselees from four cancer genetic clinics. Adapting the original PAHC to the Italian context involved adding two further domains and expanding the emotions domain to include positive emotions. While most of the items were found to be easy to understand and score, some required revision to improve comprehensibility; others were considered irrelevant or redundant and therefore deleted. The final version showed adequate reliability and validity. The I-PAHC provides comprehensive content coverage of cancer genetic-specific psychosocial problems, is well accepted by counselees, and can be considered a sound assessment tool for psychosocial issues related to cancer genetic counseling and risk assessment in Italy

    Se non ora, quando? è l’ultima prova narrativa e il terzo momento della trilogia dedicata da Levi al tempo della violenza nazista e della guerra: non solo per questo carattere esso assume una funzione ‘chimica’ polivalente nella produzione dello scrittore, in un complesso rapporto di analogie e di diversità con gli altri due libri, che riguardano l’intitolazione in forma di apostrofe, il genere narrativo del viaggio che si configura come anabasi, l’intento di ricostruzione storica, infine, i moduli stilistici della scrittura. Alla base del libro c’è il proposito di Levi di riscattare la memoria offesa dei tanti ebrei che avevano preso le armi, combattuto e contribuito alla sconfitta dell’esercito tedesco in un’epica impari lotta e rendere così loro l’onore della dignità e dell’orgoglio. Nel contempo il libro è anche la celebrazione della civiltà ebraica askenazita, del suo immenso patrimonio di tradizioni e di religiosità accumulati nel corso di una lunghissima storia che ha il suo segno di riconoscimento nella lingua yiddish, la cui intonazione lo scrittore si sforza di riprodurre in lingua italiana. La rievocazione della cultura ebraica si manifesta in una sorta di corpo a corpo con i suoi testi e i suoi miti religiosi con cui i vari personaggi si misurano drammaticamente, accettandone o contestandone le verità alla luce della loro condizione di esiliati e di perseguitati. Per questa condizione la narrazione dell’epos ebraico è anche l’epicedio di una civiltà irrimediabilmente perduta, affidata solo alla memoria dei sopravvissuti
