La scrittura riflessiva nella formazione infermieristica: background, esperienze e metodi
La scrittura della propria esperienza
formativa/clinica è da tempo utilizzata in ambito infermieristico
per sviluppare riflessione e dunque apprendimento. La
scrittura riflessiva (reflective writing) viene utilizzata per sostenere
lo sviluppo della competenza clinica, relazionale, etica
e della conoscenza di sé. Obiettivo. Descrivere le esperienze
riportate in letteratura sulla scrittura riflessiva, indicando
i contesti formativi e le tecniche utilizzate in ambito
infermieristico. Metodo. Analisi narrativa della letteratura internazionale
utilizzando le banche dati MedLine e Cinahl. Risultati.
La scrittura dell'esperienza viene utilizzata nella formazione
infermieristica di base, post-base e permanente per
sviluppare apprendimenti clinici oppure una più globale crescita
professionale e personale. Nel primo caso si utilizzano
brevi mandati scritti (anche a partire da scenari clinici), mentre
nel secondo diari o giornali di bordo, spesso con richieste
di focalizzazione su aspetti specifici dell'esperienza. Queste
focalizzazioni sono molto utili soprattutto per chi non è
abituato alla scrittura. Si utilizzano anche gli episodi significativi
e/o gli incidenti critici occorsi nella pratica formativa/
professionale. Molti lavori su esperienze di scrittura riflessiva
sottolineano l'importanza della sua condivisione tra
pari e/o del feedback da parte del docente/tutor. Conclusioni.
Le pratiche di reflective writing possono efficacemente supportare
studenti/infermieri nell'apprendimento dall'esperienza.
Va tuttavia attentamente considerata la predisposizione
alla scrittura e, soprattutto, va previsto un feedback o da parte
dei pari o dei docenti/tutor. Poiché questo feedback può
avere costi umani elevati, l'introduzione di attività di scrittura
riflessiva va attentamente valutata

A video densitometric analysis of viral burden and follicular dendritic cell damage in lymph nodes in the latency phase of HIV infection
BACKGROUND: The aim of this study was to collect quantitative data on damage to follicular dendritic cells (FDC) and on the structure of germinal centers (GC) in the early phase of HIV infection. METHODS: Lymph node sections from 10 HIV(+) subjects and from 5 HIV(-) control subjects were stained by immunohistochemistry for CD21, an FDC marker; gp24, to assess the HIV load; and IgM, to measure antibodies within the GC. RESULTS: The volume fraction (V(area)) and the logarithm of the inverse gray value (ArLIGV) of CD21 areas showed a highly significant decrease in HIV(+) specimens. The mean ArLIGV values ranged between 0.0916 +/- 0.01 and 0.3826 +/- 0.11 versus 0.6856 +/- 0.19 on average in controls (P < 0.001 for both). Six of 10 HIV(+) specimens were positive for gp24. Staining was limited to GC and showed a distribution pattern similar to that of CD21. CONCLUSIONS: FDC already undergo considerable damage during the latency phase of HIV infection. The pattern of CD21 indicates that, although FDC decrease in number, the staining intensity of positive cells is basically preserved. Video densitometric analysis, an approach requiring a strict standardized protocol, may help monitor disease course and evaluate response to therapy by quantifying viral burden and lymph node damage
NF-κB essential modulator (NEMO) is critical for thyroid function
The I-κB kinase (IKK) subunit NEMO/IKKγ (NEMO) is an adapter molecule that is critical for canonical activation of NF-κB, a pleiotropic transcription factor controlling immunity, differentiation, cell growth, tumorigenesis, and apoptosis. To explore the functional role of canonical NF-κB signaling in thyroid gland differentiation and function, we have generated a murine strain bearing a genetic deletion of the NEMO locus in thyroid. Here we show that thyrocyte-specific NEMO knock-out mice gradually develop hypothyroidism after birth, which leads to reduced body weight and shortened life span. Histological and molecular analysis indicate that absence of NEMO in thyrocytes results in a dramatic loss of the thyroid gland cellularity, associated with down-regulation of thyroid differentiation markers and ongoing apoptosis.
