    Responding to global climate change and political aims at the European level, the German government aims to achieve carbon neutrality by 2045. Embedded in this aim lies a necessity for converting diesel multiple units to electric multiple units, especially on regional rail passenger routes where diesel multiple units are often used. Making great use of existing electrification and longer layover times, battery electric multiple units are particularly well suited for these routes. However, identifying the best the spatial allocation of charging infrastructure to allow for their operation constitutes a complex non-linear optimization problem. This thesis develops a standardized methodological concept based in a genetic algorithm to optimize this spatial distribution with the least infrastructure cost using vehicle circulation plans, simulated vehicle power at wheel, OpenStreetMap data, a digital terrain model, a timetable dataset, and route geometries. The approach is developed and tested on a circulation plan for the German subnetwork of Pfalznetz. The methodology successfully identifies optimal spatial distributions of charging infrastructure but suffers from shortcomings relating to the quality and availability of input data, the computational efficiency of the algorithm and the approaching of local minima. Specific minor alterations to the code may significantly improve the quality of the results. The flexibility and broad applicability of the method primes it for expansion accounting for a wider range of aspects relating to charging infrastructure placement

    Giant Parathyroid Tumor:Parathyroid Adenoma versus Parathyroid Carcinoma

    Parathyroid adenoma is the most common cause of primary hyperparathyroidism (PHPT). We present the preoperative detection of a giant parathyroid adenoma (GPA) using ((99m)Tc)-sestamibi parathyroid scintigraphy in a patient presenting with severely elevated parathyroid hormone, hypercalcemia, hypophosphatemia, and vitamin D insufficiency. The patient complained of cerebral symptoms and intermittent abdominal discomfort without constipation. After surgical removal of the hyperactive parathyroid gland and D vitamin supplementation, all blood tests were normalized. The clinical and paraclinical characteristics of GPA may raise the suspicion of parathyroid carcinoma, but not absolutely in this case

    Long-term methimazole therapy in Graves' hyperthyroidism and adverse reactions: a Danish multicenter study

    PURPOSE: In this prospective multicenter study with patients newly diagnosed with Graves’ hyperthyroidism (GH), we studied the timing and characteristics of adverse drug reactions in patients treated with anti-thyroid drugs (ATD) for up to 48 months. METHODS: Patients with GH were treated with ATD until remission and hereafter with a low-dose regime to keep the patients in remission. The patients were followed with blood samples and recording of adverse events approximately every second month for the first 2 years and every third month for the following 2 years. RESULTS: We included 208 patients and the patients were treated for a median of 22 (range: 0.5–49) months. Ten percent of the patients experienced adverse drug reactions and 75% of the cases occurred during the first 6 months. After 24 months, the methimazole dose was lowered to 5 mg/day, and after this time point, no further adverse drug reactions were recorded. Skin reactions were the most prominent reaction, comprising 68% of the registered reactions, and no hepatic and bonemarrow affection was recorded. CONCLUSION: With this study, we report the frequency, timing of occurrence, and characteristics of adverse drug reactions when treating GH with the ATD drug methimazole for up to 48 months. Long-term low-dose methimazole treatment can be a cost-effective and straightforward treatment option if adverse drug reactions such as severe hepatic and bone marrow affection are kept in mind

    El Trastorno por Déficit de Atención e Hiperactividad, antes llamado hipercinesia o disfunción cerebral mínima, es uno de los trastornos mentales más comunes entre los niños. Hasta hace poco se suponía que este trastorno desaparecía en la adolescencia. Hoy sabemos que si no se realiza el diagnóstico y tratamiento indicados, el 40% de los casos se manifiestan como formas residuales. Éstas se hallan íntimamente ligadas a patologías adictivas y a otros trastornos de la impulsividad. De hecho, las 2/3 partes de los actuales abusadores de sustancias han padecido el trastorno, diagnosticado o no (Gratch, 2002). El Trastorno por Déficit de Atención e Hiperactividad es un síndrome bio-comportamental del desarrollo que posee una presentación heterogénea, es decir que la manifestación de los síntomas no es igual entre las personas que lo padecen. Frecuentemente es de origen innato y /o genético y se manifiesta en el campo de la atención (inatención), el control de impulsos (impulsividad), y la regulación del nivel de actividad (hiperactividad).
