Planar laser induced fluorescence has been used to acquire time sequence images of ground-state, neutral Si and SiO during laser ablation of an Si target in vacuum and in the presence of a background gas at a fluence of 3-4 J/cm^2. The SiO images, taken in air, strongly suggest that the observed SiO is created through reaction of silicon with oxygen at the contact front as the plume expands
Simultaneous mechanical-scan-free confocal microscopy and laser microsurgery
We demonstrate an endoscope-compatible single-fiber-based device that performs simultaneous confocal microscopy and high-precision laser microsurgery. The method is based on mapping of two-dimensional sample coordinates onto the optical spectrum and allows us to perform two-dimensional imaging and microsurgery without any mechanical movement of the probe or the sample. The technology holds promise for creating highly miniaturized endoscopes for applications such as brain tumor, pediatric, and endovascular surgeries where high-precision, small, and flexible probes are required. © 2009 Optical Society of America.published_or_final_versio
Performance of serial time-encoded amplified microscope
Serial time-encoded amplified microscopy (STEAM) is an entirely new imaging modality that enables ultrafast continuous real-time imaging with high sensitivity. By means of optical image amplification, STEAM overcomes the fundamental tradeoff between sensitivity and speed that affects virtually all optical imaging systems. Unlike the conventional microscope systems, the performance of STEAM depends not only on the lenses, but also on the properties of other components that are unique to STEAM, namely the spatial disperser, the group velocity dispersion element, and the back-end electronic digitizer. In this paper, we present an analysis that shows how these considerations affect the spatial resolution, and how they create a trade-off between the number of pixels and the frame rate of the STEAM imager. We also quantify how STEAM's optical image amplification feature improves the imaging sensitivity. These analyses not only provide valuable insight into the operation of STEAM technology but also serve as a blue print for implementation and optimization of this new imaging technology. ©2010 Optical Society of America.published_or_final_versio
Misfit strain relaxation and dislocation formation in supercritical strained silicon on virtual substrates
Relaxation of strained silicon on 20% linear graded virtual substrates was quantified using high resolution x-ray diffraction and a defect etching technique. The thickness of strained silicon was varied between 10 and 180 nm. Relaxation was observed in layers below the critical thickness but increased to only 2% relaxation in the thickest layers even with annealings up to 950 °C. Cross-sectional transmission electron microscopy revealed stacking faults present in layers thicker than 25 nm, and nucleated 90° Shockley partial dislocations forming microtwins in the thickest layer. These features are implicated in the impediment of the relaxation process
Comparing Strategies to Prevent Stroke and Ischemic Heart Disease in the Tunisian Population: Markov Modeling Approach Using a Comprehensive Sensitivity Analysis Algorithm.
Background. Mathematical models offer the potential to analyze and compare the effectiveness of very different interventions to prevent future cardiovascular disease. We developed a comprehensive Markov model to assess the impact of three interventions to reduce ischemic heart diseases (IHD) and stroke deaths: (i) improved medical treatments in acute phase, (ii) secondary prevention by increasing the uptake of statins, (iii) primary prevention using health promotion to reduce dietary salt consumption. Methods. We developed and validated a Markov model for the Tunisian population aged 35–94 years old over a 20-year time horizon. We compared the impact of specific treatments for stroke, lifestyle, and primary prevention on both IHD and stroke deaths. We then undertook extensive sensitivity analyses using both a probabilistic multivariate approach and simple linear regression (metamodeling). Results. The model forecast a dramatic mortality rise, with 111,134 IHD and stroke deaths (95% CI 106567 to 115048) predicted in 2025 in Tunisia. The salt reduction offered the potentially most powerful preventive intervention that might reduce IHD and stroke deaths by 27% (−30240 [−30580 to −29900]) compared with 1% for medical strategies and 3% for secondary prevention. The metamodeling highlighted that the initial development of a minor stroke substantially increased the subsequent probability of a fatal stroke or IHD death. Conclusions. The primary prevention of cardiovascular disease via a reduction in dietary salt consumption appeared much more effective than secondary or tertiary prevention approaches. Our simple but comprehensive model offers a potentially attractive methodological approach that might now be extended and replicated in other contexts and populations
Time trends in survival and readmission following coronary artery bypass grafting in Scotland, 1981-96: retrospective observational study
Improvements in coronary revascularisation techniques and an increase in the use of percutaneous interventions1 have led to a rise in the number of coronary artery bypass grafting operations in older patients with more severe cardiac disease and worse comorbidity and who have previously undergone revascularisation procedures. 2 3 Advances in surgical and anaesthetic techniques have prevented a worsening risk profile from being translated into an increase in perioperative deaths. 2 3 The aim of our study was to examine time trends in major outcomes up to two years after coronary artery bypass grafting
Randomised controlled trial and economic evaluation of a chest pain observation unit compared with routine care
Objectives To measure the effectiveness and cost effectiveness
of providing care in a chest pain observation unit compared
with routine care for patients with acute, undifferentiated chest
pain.
Design Cluster randomised controlled trial, with 442 days
randomised to the chest pain observation unit or routine care,
and cost effectiveness analysis from a health service costing
perspective.
Setting The emergency department at the Northern General
Hospital, Sheffield, United Kingdom.
Participants 972 patients with acute, undifferentiated chest
pain (479 attending on days when care was delivered in the
chest pain observation unit, 493 on days of routine care)
followed up until six months after initial attendance.
Main outcome measures The proportion of participants
admitted to hospital, the proportion with acute coronary
syndrome sent home inappropriately, major adverse cardiac
events over six months, health utility, hospital reattendance and
readmission, and costs per patient to the health service.
Results Use of a chest pain observation unit reduced the
proportion of patients admitted from 54% to 37% (difference
17%, odds ratio 0.50, 95% confidence interval 0.39 to 0.65,
P < 0.001) and the proportion discharged with acute coronary
syndrome from 14% to 6% (8%, –7% to 23%, P = 0.264). Rates
of cardiac event were unchanged. Care in the chest pain
observation unit was associated with improved health utility
during follow up (0.0137 quality adjusted life years gained, 95%
confidence interval 0.0030 to 0.0254, P = 0.022) and a saving of
£78 per patient (–£56 to £210, P = 0.252).
Conclusions Care in a chest pain observation unit can improve
outcomes and may reduce costs to the health service. It seems
to be more effective and more cost effective than routine care

Cardiovascular health behavior and health factor changes (1988-2008) and projections to 2020: results from the National Health and Nutrition Examination Surveys.
BackgroundThe American Heart Association's 2020 Strategic Impact Goals target a 20% relative improvement in overall cardiovascular health with the use of 4 health behavior (smoking, diet, physical activity, body mass) and 3 health factor (plasma glucose, cholesterol, blood pressure) metrics. We sought to define current trends and forward projections to 2020 in cardiovascular health.Methods and resultsWe included 35 059 cardiovascular disease-free adults (aged ≥20 years) from the National Health and Nutrition Examination Survey 1988-1994 and subsequent 2-year cycles during 1999-2008. We calculated population prevalence of poor, intermediate, and ideal health behaviors and factors and also computed a composite, individual-level Cardiovascular Health Score for all 7 metrics (poor=0 points; intermediate=1 point; ideal=2 points; total range, 0-14 points). Prevalence of current and former smoking, hypercholesterolemia, and hypertension declined, whereas prevalence of obesity and dysglycemia increased through 2008. Physical activity levels and low diet quality scores changed minimally. Projections to 2020 suggest that obesity and impaired fasting glucose/diabetes mellitus could increase to affect 43% and 77% of US men and 42% and 53% of US women, respectively. Overall, population-level cardiovascular health is projected to improve by 6% overall by 2020 if current trends continue. Individual-level Cardiovascular Health Score projections to 2020 (men=7.4 [95% confidence interval, 5.7-9.1]; women=8.8 [95% confidence interval, 7.6-9.9]) fall well below the level needed to achieve a 20% improvement (men=9.4; women=10.1).ConclusionsThe American Heart Association 2020 target of improving cardiovascular health by 20% by 2020 will not be reached if current trends continue
Susceptibility to disease (tropical theileriosis) is associated with differential expression of host genes that possess motifs recognised by a pathogen DNA binding protein
BACKGROUND: Knowledge of factors that influence the outcome of infection are crucial for determining the risk of severe disease and requires the characterisation of pathogen-host interactions that have evolved to confer variable susceptibility to infection. Cattle infected by Theileria annulata show a wide range in disease severity. Native (Bos indicus) Sahiwal cattle are tolerant to infection, whereas exotic (Bos taurus) Holstein cattle are susceptible to acute disease. METHODOLOGY/PRINCIPAL FINDINGS: We used RNA-seq to assess whether Theileria infected cell lines from Sahiwal cattle display a different transcriptome profile compared to Holstein and screened for altered expression of parasite factors that could generate differences in host cell gene expression. Significant differences (<0.1 FDR) in the expression level of a large number (2211) of bovine genes were identified, with enrichment of genes associated with Type I IFN, cholesterol biosynthesis, oncogenesis and parasite infection. A screen for parasite factors found limited evidence for differential expression. However, the number and location of DNA motifs bound by the TashAT2 factor (TA20095) were found to differ between the genomes of B. indicus vs. B. taurus, and divergent motif patterns were identified in infection-associated genes differentially expressed between Sahiwal and Holstein infected cells. CONCLUSIONS/SIGNIFICANCE: We conclude that divergent pathogen-host molecular interactions that influence chromatin architecture of the infected cell are a major determinant in the generation of gene expression differences linked to disease susceptibility
