A simplified pneumatic model for air brake of passenger trains
Braking system performance is relevant for both railway safety and network optimization. Most trains employ air brake systems; air brake systems of freight trains mostly cannot achieve a synchronous application of brake forces, which is usually customary for passenger trains. The paper generalizes a previous air brake pneumatic model to passenger trains and describes the needed modifications. Among them, the way the pressure reduces in the brake pipe is generalized. Moreover, this paper reports an analytical bi-dimensional function for calculating the nozzle diameter equivalent to the electro-pneumatic (EP) or the electronically controlled pneumatic (ECP) brake valve as a function of the wagon length and the time to vent the brake pipe locally. The numerical results of the new model are compared against several experimental tests of high-speed passenger trains of Trenitalia, namely ETR500 and ETR1000. The model is suitable to be integrated into the UIC software TrainDy, aiming to extend its computational field to passenger trains and to simulate the safety of trains during a recovery
Experimental and numerical investigation of a porous receiver equipped with Raschig Rings for CSP applications
In the context of central solar tower systems, tubular receivers are among the most appealing absorber solutions: the absorbed solar radiation is transferred from the tube external surface to the heat transfer fluid (HTF) flowing within the absorber. In the case of air as HTF, very high temperatures of the coolant can be obtained in principle, thus increasing the efficiency of the downstream thermodynamic cycle. To explore the possible applicability of a porous medium made of Raschig Rings (RRs), already successfully adopted in the heat removal from the resonant cavity of a technological device, the gyrotron, where the heat flux can go up to 20–25 MW/m2 and removed by subcooled water, a mock-up of a planar receiver equipped with RRs has been tested in a solar furnace, using air as coolant. The test results are presented here and analyzed1. Furthermore, a numerical model of the mock-up, where the RRs are modeled in detail by the Discrete Element Method, is presented and its capability to reproduce the measured data demonstrated. The model shows, for the tested configuration, an enhancement of the heat transfer of a factor of ~5 with respect to a plain channel with the same envelope, and a Performance Evaluation Criteria of 2–2.5 when the device is compared to the same receiver configuration, but without RRs
Overdiagnosis and overimaging: an ethical issue for radiological protection
Aims and objectives: This study aimed to analyse the key factors that influence the overimaging using X-ray such as self-referral, defensive medicine and duplicate imaging studies and to emphasize the ethical problem that derives from it. Materials and methods: In this study, we focused on the more frequent sources of overdiagnosis such as the total-body CT, proposed in the form of screening in both public and private sector, the choice of the most sensitive test for each pathology such as pulmonary embolism, ultrasound investigations mostly of the thyroid and of the prostate and MR examinations, especially of the musculoskeletal system. Results: The direct follow of overdiagnosis and overimaging is the increase in the risk of contrast media infusion, radiant damage, and costs in the worldwide healthcare system. The theme of the costs of overdiagnosis is strongly related to inappropriate or poorly appropriate imaging examination. Conclusions: We underline the ethical imperatives of trust and right conduct, because the major ethical problems in radiology emerge in the justification of medical exposures of patients in the practice. A close cooperation and collaboration across all the physicians responsible for patient care in requiring imaging examination is also important, balancing possible ionizing radiation disadvantages and patient benefits in terms of care

Neuroendocrine Tumors: A Comprehensive Review on Nutritional Approaches
Simple Summary Neuroendocrine neoplasms and their main subtype neuroendocrine tumors have an increasing incidence worldwide, associated with a high survival and prevalence rate. Both the tumor itself and systemic therapy can have an impact on patients' nutrition. Conversely, preliminary data suggest that malnutrition has a negative impact on the development and prognosis of neuroendocrine tumors, as does obesity. The aim of this review is to condense the latest evidence on the role of the most widely used dietary patterns, the Mediterranean diet, the ketogenic diet and intermittent fasting, in the context of neuroendocrine tumors. Nutritional plans are an integral part of the multidisciplinary treatment team of patients with neuroendocrine tumors because they improve the patient's quality of life. The nutritional approach must be tailored, based on nutritional needs and nutritionally manageable signs and/or symptoms related to drug treatment. Neuroendocrine neoplasms are a heterogeneous group of neoplasms with increasing incidence, high prevalence, and survival worldwide. About 90% of cases are well differentiated forms, the so-called neuroendocrine tumors (NETs), with slow proliferation rates and prolonged survival but frequent development of liver metastases and endocrine syndromes. Both the tumor itself and systemic therapy may have an impact on patient nutrition. Malnutrition has a negative impact on outcome in patients with NETs, as well as obesity. In addition, obesity and metabolic syndrome have been shown to be risk factors for both the development and prognosis of NET. Therefore, dietary assessment based on body composition and lifestyle modifications should be an integral part of the treatment of NET patients. Nutrition plans, properly formulated by a dietician, are an integral part of the multidisciplinary treatment team for patients with NETs because they allow an improvement in quality of life, providing a tailored approach based on nutritional needs and nutritional manageable signs and/or symptoms related to pharmacological treatment. Effects of particulate matter on genomic DNA methylation content and iNOS promoter methylation
Background: Altered patterns of gene expression mediate the effects of particulate matter (PM) on human health, but mechanisms through which PM modifies gene expression are largely undetermined.
Objectives: We aimed at identifying short- and long-term effects of PM exposure on DNA methylation, a major genomic mechanism of gene expression control, in workers in an electric furnace steel plant with well-characterized exposure to PM with aerodynamic diameters < 10 ÎĽm (PM10).
Methods: We measured global genomic DNA methylation content estimated in Alu and long interspersed nuclear element-1 (LINE-1) repeated elements, and promoter DNA methylation of iNOS (inducible nitric oxide synthase), a gene suppressed by DNA methylation and induced by PM exposure in blood leukocytes. Quantitative DNA methylation analysis was performed through bisulfite PCR pyrosequencing on blood DNA obtained from 63 workers on the first day of a work week (baseline, after 2 days off work) and after 3 days of work (postexposure). Individual PM10 exposure was between 73.4 and 1,220 ÎĽg/m3.
Results: Global methylation content estimated in Alu and LINE-1 repeated elements did not show changes in postexposure measures compared with baseline. PM10 exposure levels were negatively associated with methylation in both Alu [β = –0.19 %5-methylcytosine (%5mC); p = 0.04] and LINE-1 [β = –0.34 %5mC; p = 0.04], likely reflecting long-term PM10 effects. iNOS promoter DNA methylation was significantly lower in postexposure blood samples compared with baseline (difference = –0.61 %5mC; p = 0.02).
Conclusions: We observed changes in global and gene specific methylation that should be further characterized in future investigations on the effects of PM

Cholesteatoma vs granulation tissue: a differential diagnosis by DWI-MRI apparent diffusion coefficient
To diagnose cholesteatoma when it is not visible through tympanic perforation, imaging techniques are necessary. Recently, the combination of computed tomography and magnetic resonance imaging has proven effective to diagnose middle ear cholesteatoma. In particular, diffusion weighted images have integrated the conventional imaging for the qualitative assessment of cholesteatoma. Accordingly, the aim of this study was to obtain a quantitative analysis of cholesteatoma calculating the apparent diffusion coefficient value.
