Violence en psychiatrie: une recherche prospective au Département Universitaire de Psychiatrie Adulte (DUPA) interrogée à la lumière du modèle cindynique
Le management des risques dans les institutions psychiatriques représente aujourd'hui un challenge majeur pour les cliniciens, les administrateurs et les décideurs politiques. Il pose la question de la formation des équipes au management de la violence et de l'évaluation des patients dans la clinique quotidienne. Cet article fait le lien entre une étude, menée sur un an dans une institution psychiatrique et un modèle de management des risques, le modèle cindynique. Les résultats doivent interroger sur les représentations du phénomène de violence par les différents acteurs, l'appropriation par les équipes d'outils d'évaluation de la dangerosité et sur la communication dans l'équipe pluridisciplinaire et avec le patient
Discurso e identidade: breve caracterização linguístico-discursiva do populismo
Este artigo tem como objetivo discutir a relaçãoentre discurso e identidade no âmbito linguístico-discursivo, de modo a caracterizar o discurso político presidencial populista no contexto histórico brasileiro da década de 1950. Identificamos no populismo um objeto de estudo a ser explorado em função da escassa literatura referente à análise linguística desse fenômeno político, o qual marcou os governos latino-americanos entre as décadas de 1950 e 1960. Interessa-nos, assim, estabelecer uma aproximação teórica entre a análise retórica do discurso e o contexto histórico em que os pronunciamentos de Vargas foram realizados, de modo a contribuir para a caracterização discursivo-identitária, ainda que breve, do populismo. Para procedermos às análises, foram selecionados discursos proferidos por Getúlio Vargas no período de seu segundo mandato como presidente da República (1951-54), extraídos do livro O governo trabalhista do Brasil – volumes III e IV, reunidos e editados pela Livraria José Olympio, em 1969. Como aporte teórico sobre discurso político, recorremos aos trabalhos de Aquino (2005, 2003 e 1997) e Charaudeau (2006); sobre populismo, aos de Capelato (2001), Ferreira (2001) e Weffort (1982); e, finalmente, acerca de gêneros textuais, aos de Bakhtin (2003 [1927]) e Grillo (2006). Buscamos explicitar, ao longo do trabalho, as estratégias de seleção lexical que contribuem para a constituição identitária do populismo como fenômeno discursivo
Influencia de la metodología en los profesores que imparten la asignatura de Educación Física en el aprendizaje de la técnica del salto de longitud en los estudiantes de sexto año de educación básica
La presente investigación surge como una necesidad de los estudiantes de sexto año de educación básica que presentan dificultades en el aprendizaje de la técnica del salto de longitud, lo que la incido negativamente en el desarrollo motriz la que tiene como título: INFLUENCIA DE LA METODOLOGIA QUE UTILIZAN LOS PROFESORES QUE IMPARTEN LA ASIGNATURA DE EDUCACIÓN FÍSICA EN EL APRENDIZAJE DE LA TÉCNICA DEL SALTO DE LONGITUD EN LOS ESTUDIANTES DE SEXTO GRADO. Para la realización de la investigación la autora se formuló se formuló el siguiente objetivo. determinar la influencia de la metodología de la enseñanza que utilizan los docentes que imparten la asignatura en el aprendizaje motriz en la técnica del salto de longitud en los estudiantes de sexto grado de Educación Básica de la Escuela “Cinco de Junio” del Cantón Alfredo Baquerizo Moreno, durante el año lectivo 2014 – 2015? Para dar cumplimiento al objetivo antes mencionado se seleccionaron los siguientes métodos de investigación analítico – sintético, inductivo – deductivo, histórico – lógico , observación y test pedagógicos
para el procesamiento estadístico de la información de los datos obtenidos se utilizó el cálculo porcentual (%), lo que permitió procesar la información mediante el análisis de los resultados obtenidos en los instrumentos aplicados a los 30 estudiantes que fueron escogidos como muestra. Los resultados obtenidos en la investigación permiten afirmar que la hipótesis es aceptada, ya que como se ha evidenciado durante el desarrollo del mismo, La metodología de la enseñanza que utilizan los profesores que imparten la asignatura de Educación Física incidió negativamente en el aprendizaje motriz de los estudiantes de sexto año de básica que fueron investigado

Continuation vs Discontinuation of Renin-Angiotensin System Inhibitors Before Major Noncardiac Surgery
ImportanceBefore surgery, the best strategy for managing patients who are taking renin-angiotensin system inhibitors (RASIs) (angiotensin-converting enzyme inhibitors or angiotensin receptor blockers) is unknown. The lack of evidence leads to conflicting guidelines.ObjectiveTo evaluate whether a continuation strategy vs a discontinuation strategy of RASIs before major noncardiac surgery results in decreased complications at 28 days after surgery.Design, setting, and participantsRandomized clinical trial that included patients who were being treated with a RASI for at least 3 months and were scheduled to undergo a major noncardiac surgery between January 2018 and April 2023 at 40 hospitals in France.InterventionPatients were randomized to continue use of RASIs (n = 1107) until the day of surgery or to discontinue use of RASIs 48 hours prior to surgery (ie, they would take the last dose 3 days before surgery) (n = 1115).Main outcomes and measuresThe primary outcome was a composite of all-cause mortality and major postoperative complications within 28 days after surgery. The key secondary outcomes were episodes of hypotension during surgery, acute kidney injury, postoperative organ failure, and length of stay in the hospital and intensive care unit during the 28 days after surgery.ResultsOf the 2222 patients (mean age, 67 years [SD, 10 years]; 65% were male), 46% were being treated with angiotensin-converting enzyme inhibitors at baseline and 54% were being treated with angiotensin receptor blockers. The rate of all-cause mortality and major postoperative complications was 22% (245 of 1115 patients) in the RASI discontinuation group and 22% (247 of 1107 patients) in the RASI continuation group (risk ratio, 1.02 [95% CI, 0.87-1.19]; P = .85). Episodes of hypotension during surgery occurred in 41% of the patients in the RASI discontinuation group and in 54% of the patients in the RASI continuation group (risk ratio, 1.31 [95% CI, 1.19-1.44]). There were no other differences in the trial outcomes.Conclusions and relevanceAmong patients who underwent major noncardiac surgery, a continuation strategy of RASIs before surgery was not associated with a higher rate of postoperative complications than a discontinuation strategy.Trial registrationClinicalTrials.gov Identifier: NCT03374449
The Athena X-ray Integral Field Unit: a consolidated design for the system requirement review of the preliminary definition phase
The Athena X-ray Integral Unit (X-IFU) is the high resolution X-ray spectrometer studied since 2015 for flying in the mid-30s on the Athena space X-ray Observatory. Athena is a versatile observatory designed to address the Hot and Energetic Universe science theme, as selected in November 2013 by the Survey Science Committee. Based on a large format array of Transition Edge Sensors (TES), X-IFU aims to provide spatially resolved X-ray spectroscopy, with a spectral resolution of 2.5 eV (up to 7 keV) over a hexagonal field of view of 5 arc minutes (equivalent diameter). The X-IFU entered its System Requirement Review (SRR) in June 2022, at about the same time when ESA called for an overall X-IFU redesign (including the X-IFU cryostat and the cooling chain), due to an unanticipated cost overrun of Athena. In this paper, after illustrating the breakthrough capabilities of the X-IFU, we describe the instrument as presented at its SRR (i.e. in the course of its preliminary definition phase, so-called B1), browsing through all the subsystems and associated requirements. We then show the instrument budgets, with a particular emphasis on the anticipated budgets of some of its key performance parameters, such as the instrument efficiency, spectral resolution, energy scale knowledge, count rate capability, non X-ray background and target of opportunity efficiency. Finally, we briefly discuss the ongoing key technology demonstration activities, the calibration and the activities foreseen in the X-IFU Instrument Science Center, touch on communication and outreach activities, the consortium organisation and the life cycle assessment of X-IFU aiming at minimising the environmental footprint, associated with the development of the instrument. Thanks to the studies conducted so far on X-IFU, it is expected that along the design-to-cost exercise requested by ESA, the X-IFU will maintain flagship capabilities in spatially resolved high resolution X-ray spectroscopy, enabling most of the original X-IFU related scientific objectives of the Athena mission to be retained. The X-IFU will be provided by an international consortium led by France, The Netherlands and Italy, with ESA member state contributions from Belgium, Czech Republic, Finland, Germany, Poland, Spain, Switzerland, with additional contributions from the United States and Japan.The French contribution to X-IFU is funded by CNES, CNRS and CEA.
