The More, the Not Marry-Er: In Search of a Policy Behind Eligibility for California Domestic Partnerships
This Comment examines California Assembly Bill 25 which grants certain civic rights to state-registered domestic partners. The author seeks to provide background to the development of the current statutory scheme and examine whether any coherent policy supports the statute\u27s expansion to include greater numbers of opposite-sex couples. The author begins by examining the substantive benefits that California domestic partnerships provide and to whom those benefits are offered. She continues to explore the background of the current domestic partnership scheme in California against the backdrop of the ways that other states have sought to address the needs of nontraditional couples and families. The author then examines the legislative history of Assembly Bill 25 and the possible purposes behind expanding eligibility for domestic partnerships. The author concludes that there is no concrete policy behind such an expansion. She offers suggestions regarding the likely purpose behind this expansion and recommends that other states that wish to follow California\u27s model adopt a clear policy behind eligibility

The genetic history of the Southern Arc: a bridge between West Asia and Europe
By sequencing 727 ancient individuals from the Southern Arc (Anatolia and its neighbors in Southeastern Europe and West Asia) over 10,000 years, we contextualize its Chalcolithic period and Bronze Age (about 5000 to 1000 BCE), when extensive gene flow entangled it with the Eurasian steppe. Two streams of migration transmitted Caucasus and Anatolian/Levantine ancestry northward, and the Yamnaya pastoralists, formed on the steppe, then spread southward into the Balkans and across the Caucasus into Armenia, where they left numerous patrilineal descendants. Anatolia was transformed by intra–West Asian gene flow, with negligible impact of the later Yamnaya migrations. This contrasts with all other regions where Indo-European languages were spoken, suggesting that the homeland of the Indo-Anatolian language family was in West Asia, with only secondary dispersals of non-Anatolian Indo-Europeans from the steppe
A Statewide Prescription Monitoring Program Affects Emergency Department Prescribing Behaviors
Study objective Ohio recently instituted an online prescription monitoring program, the Ohio Automated Rx Reporting System (OARRS), to monitor controlled substance prescriptions within Ohio. This study is undertaken to identify the influence of OARRS data on clinical management of emergency department (ED) patients with painful conditions. Methods This prospective quasiexperimental study was conducted at the University of Toledo Medical Center Emergency Department during June to July 2008. Eligible participants included ED patients with painful conditions. Patients with acute injuries were excluded. After clinical evaluation, and again after presentation of OARRS data, providers answered a set of questions about anticipated pain prescription for the patient. Outcome measures included changes in opioid prescription and other potential factors that influenced opioid prescription. Results Among 179 participants, OARRS data revealed high numbers of narcotics prescriptions filled in the most recent 12 months (median 7; range 0 to 128). Numerous providers prescribed narcotics for patients (median 3 per patient; range 0 to 40). Patients had filled narcotics prescriptions at different pharmacies (mean [SD] 3.5 [4.4]). Eighteen providers are represented in the study. Four providers treated 63% (N=114) of the patients in the study. After review of the OARRS data, providers changed the clinical management in 41% (N=74) of cases. In cases of altered management, the majority (61%; N=45) resulted in fewer or no opioid medications prescribed than originally planned, whereas 39% (N=29) resulted in more opioid medication than previously planned. Conclusion The use of data from a statewide narcotic registry frequently altered prescribing behavior for management of ED patients with complaints of nontraumatic pain

Methods for addressing publication bias in school psychology journals: A descriptive review of meta-analyses from 1980 to 2019
Although meta-analyses are often used to inform practitioners and researchers, the resulting effect sizes can be artificially inflated due to publication bias. There are a number of methods to protect against, detect, and correct for publication bias. Currently, it is unknown to what extent scholars publishing meta-analyses within school psychology journals use these methods to address publication bias and whether more recently published meta-analyses more frequently utilize these methods. A historical review of every meta-analysis published to date within the most prominent school psychology journals (N = 10) revealed that 88 meta-analyses were published from 1980 to early 2019. Exactly half of them included grey literature, and 60% utilized methods to detect and correct for publication bias. The most common methods were visual analysis of a funnel plot, Orwin\u27s failsafe N, Egger\u27s regression, and the trim and fill procedure. None of these methods were used in more than 20% of the studies. About half of the studies incorporated one method, 20% incorporated two methods, 7% incorporated three methods, and none incorporated all four methods. These methods were most evident in studies published recently. Similar to other fields, the true estimates of effects from meta-analyses published in school psychology journals may not be available, and practitioners may be utilizing interventions that are, in fact, not as strong as believed. Practitioners, researchers employing meta-analysis techniques, education programs, and editors and peer reviewers in school psychology should continue to guard against publication bias using these methods
Preoperative demographics and laboratory markers may be associated with early dislocation after total hip arthroplasty
Abstract Purpose The purpose of this study was to identify modifiable medical comorbidities, laboratory markers and flaws in perioperative management that increase the risk of acute dislocation in total hip arthroplasty (THA) patients. Methods All THA with primary indications of osteoarthritis from 2007 to 2020 were queried from the National Surgical Quality Improvement Program (NSQIP) database. Demographic data, preoperative laboratory values, recorded past medical history, operative details as well as outcome and complication information were collected. The study population was divided into two cohorts: non‐dislocation and dislocation patients. Statistics were performed to compare the characteristics of both cohorts and to identify risk factors for prosthetic dislocation (α < 0.05). Results 275,107 patients underwent primary THA in 2007 to 2020, of which 1,258 (0.5%) patients experienced a prosthetic hip dislocation. Demographics between non‐dislocation and dislocation cohorts varied significantly in that dislocation patients were more likely to be female, older, with lower body mass index and a more extensive past medical history (all p < 0.05). Moreover, hypoalbuminemia and moderate/severe anemia were associated with increased risk of dislocation in a multivariate model (all p < 0.05). Finally, use of general anesthesia, longer operative time, and longer length of hospital stay correlated with greater risk of prosthetic dislocation (all p < 0.05). Conclusions Elderly female patients and patients with certain abnormal preoperative laboratory values are at risk for sustaining acute dislocations after index THA. Careful interdisciplinary planning and medical optimization should be considered in high‐risk patients as dislocations significantly increase the risk of sepsis, cerebral vascular accident, and blood transfusions on readmission
Preoperative Malnutrition and Metabolic Markers May Predict Periprosthetic Fractures in Total Hip Arthroplasty
Background: Periprosthetic fractures are a devastating complication of total hip arthroplasty (THA) and are associated with significantly higher mortality rates in the postoperative period. Given the strain that periprosthetic fractures place on the patient as well as the healthcare system, identifying and optimizing medical comorbidities is essential in reducing complications and improving outcomes. Methods: All THA with primary indications of osteoarthritis from 2007 to 2020 were queried from the National Surgical Quality Improvement Program database. Demographic data, preoperative laboratory values, medical comorbidities, hospital course, and acute complications were collected and compared between patients with and without readmission for a periprosthetic fracture. A multivariate logistic regression analysis was performed to determine associated independent risk factors for periprosthetic fractures after index THA. Results: The analysis included 275,107 patients, of which 2539 patients were readmitted for periprosthetic fractures. Patients with postoperative fractures were more likely to be older (>65 years), females, BMI >40, and increased medical comorbidities. Preoperative hypoalbuminemia, hyponatremia, and abnormal estimated glomerular filtration rates were independent risk factors for sustaining a periprosthetic fracture and readmission within 30 days. Modifiable patient-related factors of concurrent smoking and chronic steroid use at the time of index THA were also independent risk factors for periprosthetic fractures. Inpatient metrics of longer length of stay, operative time, and discharge to rehab predicted postarthroplasty fracture risk. Readmitted fracture patients subsequently had increased risks of developing a surgical site infection, urinary tract infection, and requiring blood transfusions. Conclusions: Patients with hypoalbuminemia, hyponatremia, and abnormal estimated glomerular filtration rate are at increased risk for sustaining periprosthetic fractures after THA. Preoperative optimization with close monitoring of metabolic markers and modifiable risk factors may help not only prevent acute periprosthetic fractures but also associated infection and bleeding risk with fracture readmission
