    Wearable devices for health remote monitor system

    It is feasible to see how communication and information technology have advanced at a rapid pace in today’s world. The introduction of wearable technology is one aspect contributing to this progress and has the potential to be an innovative solution to healthcare challenges since it may be utilized for illness prevention and maintenance, such as physical monitoring, as well as patient management. In order to solve some of the healthcare challenges, this paper proposes the development of an intelligent health monitoring system with alerts and continuous monitoring using wearable devices capable of collecting biometric data on human health. The concept was then proven by the development of a prototype using sensors connected to a micro-controller which transmits its information via MQTT to a Node-RED powered dashboard that handles the health metrics monitoring.     A field-extrema hysteresis loss model for high-frequency ferrimagnetic materials

    We present a new field-extrema hysteresis loss model (FHM) for high-frequency ferrimagnetic materials, along with a parameter identification procedure. The model does not involve solving an ordinary differential equation (ODE) and is asymmetric in that it works well under dc bias conditions. In the proposed model, the loss calculations are based on the extrema values of the fields. The model includes the effects of magnetic saturation as well as frequency effects. The model is comparable in accuracy to the ODE-based Jiles-Atherton model, but retains the convenience and computational efficiency of an empirical model. We demonstrate a procedure to characterize the model parameters using the Jiles-Atherton model. We compare magnetic hysteresis loss calculated by our new model with a full time-domain solution, as well as an empirical model, for a sample high-frequency ferrite. We demonstrate the use of the model, and validate the model, by calculating magnetic loss in an EI core inductor operating as the filter inductor in a buck converter. The model and identification procedure are being endorsed as a useful framework for computing magnetic loss in the context of automated magnetic device design

    Noncompleted Sexual Offenses: Internal States, Risks and Difficulties Related to Crime Commission through the Lens of Sexual Offenders

    The study aims to generate insights from sexual offenders on the influence of internal states and how they perceive risks of apprehension and difficulties in the context of noncompleted sexual offenses, that is when offenders initiated the offense but were stopped or discouraged either before or during sexual contact. Adult males incarcerated for sexually offending completed a self-report questionnaire. Regression models, including interaction effects, were estimated. Two interaction effects were found providing insights into which and how internal states, such as intoxication to alcohol, may influence perceived difficulties related to crime. Future research should promote the investigation of noncompleted sexual offenses, which could provide a real opportunity to generate new or complementary insights for better understanding and guiding prevention initiatives

    C1 inhibitor deficiency: 2014 United Kingdom consensus document

    C1 inhibitor deficiency is a rare disorder manifesting with recurrent attacks of disabling and potentially life-threatening angioedema. Here we present an updated 2014 United Kingdom consensus document for the management of C1 inhibitor-deficient patients, representing a joint venture between the United Kingdom Primary Immunodeficiency Network and Hereditary Angioedema UK. To develop the consensus, we assembled a multi-disciplinary steering group of clinicians, nurses and a patient representative. This steering group first met in 2012, developing a total of 48 recommendations across 11 themes. The statements were distributed to relevant clinicians and a representative group of patients to be scored for agreement on a Likert scale. All 48 statements achieved a high degree of consensus, indicating strong alignment of opinion. The recommendations have evolved significantly since the 2005 document, with particularly notable developments including an improved evidence base to guide dosing and indications for acute treatment, greater emphasis on home therapy for acute attacks and a strong focus on service organisation.     TOI-431/HIP 26013: A super-Earth and a sub-Neptune transiting a bright, early K dwarf, with a third RV planet

    We present the bright (Vmag = 9.12), multiplanet system TOI-431, characterized with photometry and radial velocities (RVs). We estimate the stellar rotation period to be 30.5 \ub1 0.7 d using archival photometry and RVs. Transiting Exoplanet Survey Satellite (TESS) objects of Interest (TOI)-431 b is\ua0a\ua0super-Earth with\ua0a\ua0period of 0.49 d,\ua0a\ua0radius of 1.28 \ub1 0.04 R,\ua0a\ua0mass of 3.07 \ub1 0.35 M, and\ua0a\ua0density of 8.0 \ub1 1.0 g cm-3; TOI-431 d is\ua0a\ua0sub-Neptune with\ua0a\ua0period of 12.46 d,\ua0a\ua0radius of 3.29 \ub1 0.09 R,\ua0a\ua0mass of 9.90+1.53-1.49 M, and\ua0a\ua0density of 1.36 \ub1 0.25 g cm-3. We find\ua0a\ua0third planet, TOI-431 c,\ua0in\ua0the High Accuracy Radial velocity Planet Searcher RV data, but it is not seen to transit\ua0in\ua0the TESS light curves. It has an Msin i of 2.83+0.41-0.34 M, and\ua0a\ua0period of 4.85 d. TOI-431 d likely has an extended atmosphere and is one of the most well-suited TESS discoveries for atmospheric characterization, while the super-Earth TOI-431 b may be\ua0a\ua0stripped core. These planets straddle the radius gap, presenting an interesting case-study for atmospheric evolution, and TOI-431 b is\ua0a\ua0prime TESS discovery for the study of rocky planet phase curves

    DYNAMIC EVALUATION OF A PINNED ANCHORING SYSTEM FOR NEW YORK STATE’S TEMPORARY CONCRETE BARRIERS

    Temporary concrete barrier (TCB) systems are utilized in many circumstances, including for placement adjacent to vertical dropoffs. Free-standing TCB systems are known to have relatively large deflections when impacted, which may be undesirable when dealing with limited space behind the barrier (as seen on a bridge deck) or limited lane width in front of the barrier system. In order to allow TCB systems to be used in space-restricted locations, a variety of TCB stiffening options have been tested, including beam stiffening and pinning the barriers to the pavement. These pavement-pinning procedures have been considered time-consuming and may pose undue risk to work-zone personnel who are anchoring the barrier on the traffic-side face. Thus, a means of reducing TCB deflections while reducing risk to workers was deemed necessary. The primary research objective was to evaluate the potential for pinning alternate barrier sections on the back-side toe of the New York State’s New Jersey-shape TCBs and evaluate the barrier system according to the Test Level 3 (TL-3) criteria set forth in MASH. The research study included one 2270P full-scale vehicle crash test with a Dodge Quad Cab pickup truck. Four 151⁄2-in. (394-mm) long, vertical steel pins were placed through holes on the back-side toe of alternating barrier sections and inserted into drilled holes within the rigid concrete surface. Following the successful redirection of the pickup truck, the safety performance of the pinned anchoring system was determined to be acceptable according to the TL-3 evaluation criteria specified in MASH using the 2270P vehicle. However, it should be noted that significant barrier deflections were observed during the crash test and may be greater than those desired for work areas with restricted space

    Blood Flow Restriction Training After Patellar INStability (BRAINS Trial)

    Background Patellar instability is a common and understudied condition that disproportionally affects athletes and military personnel. The rate of post-traumatic osteoarthritis that develops following a patellar dislocation can be up to 50% of individuals 5–15 years after injury. Conservative treatment is the standard of care for patellar instability however, there are no evidence-informed rehabilitation guidelines in the scientific literature. The purpose of this study is to assess the effectiveness of blood-flow restriction training (BFRT) for patellar instability. Our hypotheses are that this strategy will improve patient-reported outcomes and accelerate restoration of symmetric strength and knee biomechanics necessary to safely return to activity. Methods/Design This is a parallel-group, superiority, randomized, double-blinded, placebo-controlled clinical trial at the University of Kentucky, sports medicine clinic that aims to recruit 78 patients with acute patellar dislocations randomly allocated into two groups: (1) sham BFRT and (2) BFRT. Both groups will receive the current standard of care physical therapy 3 times per week for up to 9 weeks. Physical therapy sessions will consist of typical standard of care treatment followed by BFRT or sham BFRT. Primary outcomes include the Norwich Patellar Instability Scale, quadriceps strength, and imaging and biochemical biomarkers of cartilage degradation. Discussion The current standard of care for non-operative treatment of patellar instability is highly variable does not adequately address the mechanisms necessary to restore lower extremity function and protect the long-term health of articular cartilage following injury. This proposed novel intervention strategy uses an easily implementable therapy to evaluate if BFRT significantly improves patient-reported outcomes, function, and joint health over the first year of recovery.
