    Early osseointegration driven by the surface chemistry and wettability of dental implants

    Objective The objective of this study was to investigate the impact of two different commercially available dental implants on osseointegration. The surfaces were sandblasting and acid etching (Group 1) and sandblasting and acid etching, then maintained in an isotonic solution of 0.9% sodium chloride (Group 2). Material and Methods X-ray photoelectron spectroscopy (XPS) was employed for surface chemistry analysis. Surface morphology and topography was investigated by scanning electron microscopy (SEM) and confocal microscopy (CM), respectively. Contact angle analysis (CAA) was employed for wetting evaluation. Bone-implant-contact (BIC) and bone area fraction occupied (BAFO) analysis were performed on thin sections (30 μm) 14 and 28 days after the installation of 10 implants from each group (n=20) in rabbits' tibias. Statistical analysis was performed by ANOVA at the 95% level of significance considering implantation time and implant surface as independent variables. Results Group 2 showed 3-fold less carbon on the surface and a markedly enhanced hydrophilicity compared to Group 1 but a similar surface roughness (p>;0.05). BIC and BAFO levels in Group 2 at 14 days were similar to those in Group 1 at 28 days. After 28 days of installation, BIC and BAFO measurements of Group 2 were approximately 1.5-fold greater than in Group 1 (

    Alveolar bone repair with strontium- containing nanostructured carbonated hydroxyapatite

    Objective: This study aimed to evaluate bone repair in rat dental sockets after implanting nanostructured carbonated hydroxyapatite/sodium alginate (CHA) and nanostructured carbonated hydroxyapatite/sodium alginate containing 5% strontium microspheres (SrCHA) as bone substitute materials. Methods: Twenty male Wistar rats were randomly divided into two experimental groups: CHA and SrCHA (n=5/period/group). After one and 6 weeks of extraction of the right maxillary central incisor and biomaterial implantation, 5 μm bone blocks were obtained for histomorphometric evaluation. The parameters evaluated were remaining biomaterial, loose connective tissue and newly formed bone in a standard area. Statistical analysis was performed by Mann-Withney and and Wilcoxon tests at 95% level of significance. Results: The histomorphometric results showed that the microspheres showed similar fragmentation and bio-absorbation (p>;0.05). We observed the formation of new bones in both groups during the same experimental periods; however, the new bone formation differed significantly between the weeks 1 and 6 (p=0.0039) in both groups. Conclusion: The CHA and SrCHA biomaterials were biocompatible, osteoconductive and bioabsorbable, indicating their great potential for clinical use as bone substitutes

    Evaluation of strontium-containing hydroxyapatite as bone substitute in sheep tibiae.

    With the advancement of research in biomaterials, it has been suggested that the best osteoconductivity of hydroxyapatite would be achieved if its crystal were closer to the structure, size and morphology of biological apatite, that is why nano-hydroxyapatite (nano-HA) is of great importance. current interest. Strontium ions are known to reduce bone resorption, induce osteoblastic activity and stimulate bone formation. The aim of this study was to evaluate biocompatibility and osteoconduction in surgical defects filled with nano-hydroxyapatite microspheres containing 1% strontium (nano-SrHA), stoichiometric nano-HA microspheres (nano-HA) compared to the clot (control) . Four Santa Inês sheep, weighing an average of 32 kg, were anesthetized and submitted to three 2 mm diameter perforations in the medial face of the tibia. The surgical defects were filled with blood clot, microspheres of Sr-HA 1% and microspheres of HA. After 30 days the samples were drawn (6 mm), decalcified, processed for inclusion in paraffin and stained with hematoxylin and eosin (HE) for histological evaluation with light microscopy. All groups revealed bone neoformation from the periphery to the center of the defect, with the nano-SrHA group being less intense among those studied. Presence of a discrete mononuclear inflammatory infiltrate in all experimental groups. Giant foreign body cells were only observed in the HA group. Areas of bone neoformation were observed in close contact with both biomaterials.     Prevalência do uso de protetor bucal por atletas dos principais clubes de futebol do estado do Rio de Janeiro

    Esportes de contato apresentam cada vez mais competividade e, com isso, há um aumento no número de lesões, em geral. Lesões orofaciais estão dentro desse aumento, principalmente traumatismos bucais e dentários. O protetor bucal é um aparato que diminui e/ou evita tais danos faciais. Dessa forma, o presente estudo objetivou verificar a prevalência e a importância do uso do protetor bucal nos atletas profissionais dos principais clubes de futebol do Estado do Rio de Janeiro. A amostra foi composta de 57 jogadores de futebol profissional entre 18 e 36 anos. A avaliação se deu através de questionário individual para cada atleta. Os resultados mostraram que 82,4% dos jogadores apresentaram lesões bucais. Além disso, 94,7% disseram ter conhecimento sobre protetores bucais e 76,7% acharem seu uso necessário, porém apenas 12,2% responderam possuir algum protetor bucal. Conclui-se que apesar do conhecimento da importância do uso de proteção bucal, os atletas de futebol ainda não fazem uso de tal aparato de proteção.Contact sports have increasingly competitiveness and, therefore, there is na increase in the number of injuries in general. Orofacial injuries are within of this increase, mainly buccal and dental trauma. The mouthguard is an apparatus that reduces and/or prevents such facial injuries. Thus, the present study aimed to determine the prevalence and the importance of using the mouthguard in professional athletes of the top soccer clubs of Rio de Janeiro State. The sample consisted of 57 professional soccer players between 18 and 36 years. The evaluation was made through individual questionnaire for each athlete. The results showed that 82.4 % of subjects showed oral lesions. In addition, 94.7 % said they had knowledge of mouthguards and 76.7 % deem their use necessary, but only 12.2 % said having some mouth guard. In conclusion, despite the knowledge of the importance of using oral protection, soccer players still do not use such protection apparatus

    Cytocompatibility and biocompatibility of nanostructured carbonated hydroxyapatite spheres for bone repair

    Objective The aim of this study was to investigate the in vitro and in vivo biological responses to nanostructured carbonated hydroxyapatite/calcium alginate (CHA) microspheres used for alveolar bone repair, compared to sintered hydroxyapatite (HA). Material and Methods The maxillary central incisors of 45 Wistar rats were extracted, and the dental sockets were filled with HA, CHA, and blood clot (control group) (n=5/period/group). After 7, 21 and 42 days, the samples of bone with the biomaterials were obtained for histological and histomorphometric analysis, and the plasma levels of RANKL and OPG were determined via immunoassay. Statistical analysis was performed by Two-Way ANOVA with post-hoc Tukey test at 95% level of significance. Results The CHA and HA microspheres were cytocompatible with both human and murine cells on an in vitro assay. Histological analysis showed the time-dependent increase of newly formed bone in control group characterized by an intense osteoblast activity. In HA and CHA groups, the presence of a slight granulation reaction around the spheres was observed after seven days, which was reduced by the 42nd day. A considerable amount of newly formed bone was observed surrounding the CHA spheres and the biomaterials particles at 42-day time point compared with HA. Histomorphometric analysis showed a significant increase of newly formed bone in CHA group compared with HA after 21 and 42 days from surgery, moreover, CHA showed almost 2-fold greater biosorption than HA at 42 days (two-way ANOVA,

    18 Years of Medication-Related Osteonecrosis of the Jaw (MRONJ) Research: Where Are We Now?—An Umbrella Review

Many studies have highlighted that ONJ is a severe condition, which is very challenging to manage, especially in individuals with oncologic disease. The aim of this umbrella review is to analyze all available interventional and non-interventional systematic reviews published on medication-related osteonecrosis of the jaw (MRONJ) and summarize this evidence. Material and methods: A multi-database search (PubMed, MEDLINE, EMBASE and CINAHL) was performed to identify related multi-language papers published from January 2003 until June 2021. An additional manual search was also performed in systematic review registries (PROSPERO, INPLASY, JBI and OFS) to identify possible missing reviews. Data were extracted from relevant papers and analyzed according to the outcomes selected in this review. Results: The search generated 25 systematic reviews eligible for the analysis. The total number of patients included in the analysis was 80,840. Of the reviews, 64% (n = 16) were non-interventional and 36% (n = 9) were interventional. Study designs included case series 20.50% (n = 140), retrospective cohort studies 12.30% (n = 84) and case reports 12.20% (n = 83). It was unclear what study design was used for 277 studies included in the 25 systematic reviews. Conclusions: The data reviewed confirmed that the knowledge underpinning MRONJ in the last 20 years is still based on weak evidence. This umbrella review highlighted a widespread low-level quality of studies and many poorly designed reviews
