INR Telemonitoring: Efficacy and Safety of a Telemonitoring Program in 453 Patients
Introdução: A análise do valor de INR em doentes hipocoagulados traz grande sobrecarga aos profissionais de saúde, despesas excessivas ao Serviço Nacional de Saúde (SNS) e perda de qualidade de vida dos doentes que se vêem obrigados a deslocações
hospitalares frequentes. Não deveria constituir surpresa que a tecnologia esteja na vanguarda dos cuidados de saúde no século XXI e na área da anticoagulação têm sido desenvolvidos projectos de auto-monitorização do INR através do telefone, telemóvel ou internet.
O objectivo deste estudo foi analisar a eficácia e segurança de um sistema de telemonitorização de INR implementado no nosso hospital em 2006.
Material e Métodos: Estudo observacional, prospectivo dos 453 doentes incluÃdos no sistema de telemonitorização de INR desde
2006 até final Novembro 2010. A comunicação entre o doente e os profissionais de saúde é feita através de mensagens do telemóvel
num sistema codificado e estandardizado que inclui informação sobre a manutenção ou alteração de terapêutica sempre que necessário e a data da próxima avaliação. Sempre que necessário o doente pode emitir um pedido de ajuda através de um código para
o efeito. Na população estudada avaliaram-se os seguintes parâmetros: desistência do projecto de telemonitorização, necessidade de
modificação da dose do anticoagulante, pedidos de esclarecimento por parte do doente, complicações hemorrágicas e internamento por INR > 10.
Resultados: Da população estudada 53% eram do sexo feminino, idade média = 57 +/- 16. A percentagem dos valores de INR dentro do intervalo terapêutico obtida foi de 83%. Não se registaram desistências do projecto de telemonitorização. A percentagem de doentes
com complicações hemorrágicas minor e major durante o follow-up foi de 0,4% e 0,2% respectivamente.
Conclusões: O sistema de telemonitorização mostra-se seguro e eficaz no controlo à distância de análise de INR, permitindo controlo eficaz de INR com baixa prevalência de hemorragias major ou minor
Dynamic Role-Based Access Control for Decentralized Applications
Access control management is an integral part of maintaining the security of
an application. Although there has been significant work in the field of cloud
access control mechanisms, however, with the advent of Distributed Ledger
Technology (DLT), on-chain access control management frameworks hardly exist.
Existing access control management mechanisms are tightly coupled with the
business logic, resulting in governance issues, non-coherent with existing
Identity Management Solutions, low security, and compromised usability. We
propose a novel framework to implement dynamic role-based access control for
decentralized applications (dApps). The framework allows for managing access
control on a dApp, which is completely decoupled from the business application
and integrates seamlessly with any dApps. The smart contract architecture
allows for the independent management of business logic and execution of access
control policies. It also facilitates secure, low cost, and a high degree of
flexibility of access control management. The proposed framework promotes
decentralized governance of access control policies and efficient smart
contract upgrades. We also provide quantitative and qualitative metrics for the
efficacy and efficiency of the framework. Any Turing complete smart contract
programming language is an excellent fit to implement the framework. We expect
this framework to benefit enterprise and non-enterprise dApps and provide
greater access control flexibility and effective integration with traditional

Paediatric obsessive-compulsive disorder and depressive symptoms: clinical correlates and CBT treatment outcomes.
Depression frequently co-occurs with paediatric obsessive-compulsive disorder (OCD), yet the clinical correlates and impact of depression on CBT outcomes remain unclear. The prevalence and clinical correlates of depression were examined in a paediatric specialist OCD-clinic sample (N = 295; Mean = 15 [7 - 18] years, 42 % female), using both dimensional (Beck Depression Inventory-youth; n = 261) and diagnostic (Development and Wellbeing Assessment; n = 127) measures of depression. The impact of depressive symptoms and suspected disorders on post-treatment OCD severity was examined in a sub-sample who received CBT, with or without SSRI medication (N = 100). Fifty-one per-cent of patients reported moderately or extremely elevated depressive symptoms and 26 % (95 % CI: 18 - 34) met criteria for a suspected depressive disorder. Depressive symptoms and depressive disorders were associated with worse OCD symptom severity and global functioning prior to CBT. Individuals with depression were more likely to be female, have had a psychiatric inpatient admission and less likely to be attending school (ps < 0.01). OCD and depressive symptom severity significantly decreased after CBT. Depressive symptoms and depressive disorders predicted worse post-treatment OCD severity (βs = 0.19 and 0.26, ps < 0.05) but became non-significant when controlling for pre-treatment OCD severity (βs = 0.05 and 0.13, ns). Depression is common in paediatric OCD and is associated with more severe OCD and poorer functioning. However, depression severity decreases over the course of CBT for OCD and is not independently associated with worse outcomes, supporting the recommendation for treatment as usual in the presence of depressive symptoms
The stb Operon Balances the Requirements for Vegetative Stability and Conjugative Transfer of Plasmid R388
The conjugative plasmid R388 and a number of other plasmids carry an operon, stbABC, adjacent to the origin of conjugative transfer. We investigated the role of the stbA, stbB, and stbC genes. Deletion of stbA affected both conjugation and stability. It led to a 50-fold increase in R388 transfer frequency, as well as to high plasmid loss. In contrast, deletion of stbB abolished conjugation but provoked no change in plasmid stability. Deletion of stbC showed no effect, neither in conjugation nor in stability. Deletion of the entire stb operon had no effect on conjugation, which remained as in the wild-type plasmid, but led to a plasmid loss phenotype similar to that of the R388ΔstbA mutant. We concluded that StbA is required for plasmid stability and that StbA and StbB control conjugation. We next observed the intracellular positioning of R388 DNA molecules and showed that they localize as discrete foci evenly distributed in live Escherichia coli cells. Plasmid instability of the R388ΔΔstbA mutant correlated with aberrant localization of the plasmid DNA molecules as clusters, either at one cell pole, at both poles, or at the cell center. In contrast, plasmid molecules in the R388ΔΔstbB mutant were mostly excluded from the cell poles. Thus, results indicate that defects in both plasmid maintenance and transfer are a consequence of variations in the intracellular positioning of plasmid DNA. We propose that StbA and StbB constitute an atypical plasmid stabilization system that reconciles two modes of plasmid R388 physiology: a maintenance mode (replication and segregation) and a propagation mode (conjugation). The holographic principle
There is strong evidence that the area of any surface limits the information
content of adjacent spacetime regions, at 10^(69) bits per square meter. We
review the developments that have led to the recognition of this entropy bound,
placing special emphasis on the quantum properties of black holes. The
construction of light-sheets, which associate relevant spacetime regions to any
given surface, is discussed in detail. We explain how the bound is tested and
demonstrate its validity in a wide range of examples.
A universal relation between geometry and information is thus uncovered. It
has yet to be explained. The holographic principle asserts that its origin must
lie in the number of fundamental degrees of freedom involved in a unified
description of spacetime and matter. It must be manifest in an underlying
quantum theory of gravity. We survey some successes and challenges in

Comparison of 8 weeks standard treatment (rifampicin plus clarithromycin) vs. 4 weeks standard plus amoxicillin/clavulanate treatment [RC8 vs. RCA4] to shorten Buruli ulcer disease therapy (the BLMs4BU trial): study protocol for a randomized controlled multi-centre trial in Benin
Background
Buruli ulcer (BU) is a neglected tropical disease caused by Mycobacterium ulcerans that affects skin, soft tissues, and bones, causing long-term morbidity, stigma, and disability. The recommended treatment for BU requires 8 weeks of daily rifampicin and clarithromycin together with wound care, physiotherapy, and sometimes tissue grafting and surgery. Recovery can take up to 1 year, and it may pose an unbearable financial burden to the household.
Recent in vitro studies demonstrated that beta-lactams combined with rifampicin and clarithromycin are synergistic against M. ulcerans. Consequently, inclusion of amoxicillin/clavulanate in a triple oral therapy may potentially improve and shorten the healing process.
The BLMs4BU trial aims to assess whether co-administration of amoxicillin/clavulanate with rifampicin and clarithromycin could reduce BU treatment from 8 to 4 weeks.
Methods
We propose a randomized, controlled, open-label, parallel-group, non-inferiority phase II, multi-centre trial in Benin with participants stratified according to BU category lesions and randomized to two oral regimens: (i) Standard: rifampicin plus clarithromycin therapy for 8 weeks; and (ii) Investigational: standard plus amoxicillin/clavulanate for 4 weeks. The primary efficacy outcome will be lesion healing without recurrence and without excision surgery 12 months after start of treatment (i.e. cure rate). Seventy clinically diagnosed BU patients will be recruited per arm. Patients will be followed up over 12 months and managed according to standard clinical care procedures. Decision for excision surgery will be delayed to 14 weeks after start of treatment. Two sub-studies will also be performed: a pharmacokinetic and a microbiology study.
Discussion
If successful, this study will create a new paradigm for BU treatment, which could inform World Health Organization policy and practice. A shortened, highly effective, all-oral regimen will improve care of BU patients and will lead to a decrease in hospitalization-related expenses and indirect and social costs and improve treatment adherence. This trial may also provide information on treatment shortening strategies for other mycobacterial infections (tuberculosis, leprosy, or non-tuberculous mycobacteria infections).

Comparison of Similarity Coefficients used for Cluster Analysis with Amplified Fragment Length Polymorphism Markers in the Silkworm, Bombyx mori
Establishing accurate genetic similarity and dissimilarity between individuals is an essential and decisive point for clustering and analyzing inter and intra population diversity because different similarity and dissimilarity indices may yield contradictory outcomes. We assessed the variations caused by three commonly used similarity coefficients including Jaccard, Sorensen-Dice and Simple matching in the clustering and ordination of seven Iranian native silkworm, Bombyx mori L. (Lepidoptera: Bombycidae), strains analyzed by amplified fragment length polymorphism markers. Comparisons among the similarity coefficients were made using the Spearman correlation analysis, dendrogram evaluation (visual inspection and consensus fork index - CIC), projection efficiency in a two-dimensional space, and groups formed by the Tocher optimization procedure. The results demonstrated that for almost all methodologies, the Jaccard and Sorensen-Dice coefficients revealed extremely close results, because both of them exclude negative co-occurrences. Due to the fact that there is no guarantee that the DNA regions with negative cooccurrences between two strains are indeed identical, the use of coefficients such as Jaccard and Sorensen-Dice that do not include negative co-occurrences was imperative for closely related organisms
