Screening methods have become increasingly important due to the growing number of patients suspected of having obstructive sleep apnea (OSA) being referred to sleep clinics. The Lausanne NoSAS (Neck circumference, Obesity, Snoring, Age, Sex) score test is a simple, efficient, and easily employed tool enabling identification of individuals at risk for the disease. Walking clinic in ambulatory surgery: a patient based concept: a Portuguese pioneer project
Walking Clinic is an innovative, efficient and easily reproducible concept adapted to ambulatory surgery. It consists of a preoperative single day work-up, with a surgeon, an anesthetist and a nurse. The aim of this study was to evaluate patient satisfaction and its determinants. Methods: A survey was applied to 171 patients (101 of the Walking Clinic group and 70 not engaged in this new concept). Patient satisfaction was assessed evaluating five major questionnaire items: secretariat (quality of the information and support given), physical space (overall comfort and cleanliness), nurses and medical staff (willingness and expertise), and patients (waiting time until pre-operative consults and exams, waiting time until being scheduled for surgery, surgery day waiting time and postoperative pain control). Furthermore, overall assessment of the received treatment, and probability of patient recommending or returning to our ambulatory unit were also analyzed
Large phenotype jumps in biomolecular evolution
By defining the phenotype of a biopolymer by its active three-dimensional
shape, and its genotype by its primary sequence, we propose a model that
predicts and characterizes the statistical distribution of a population of
biopolymers with a specific phenotype, that originated from a given genotypic
sequence by a single mutational event. Depending on the ratio g0 that
characterizes the spread of potential energies of the mutated population with
respect to temperature, three different statistical regimes have been
identified. We suggest that biopolymers found in nature are in a critical
regime with g0 in the range 1-6, corresponding to a broad, but not too broad,
phenotypic distribution resembling a truncated Levy flight. Thus the biopolymer
phenotype can be considerably modified in just a few mutations. The proposed
model is in good agreement with the experimental distribution of activities

Discriminating among Earth composition models using geo-antineutrinos
It has been estimated that the entire Earth generates heat corresponding to
about 40 TW (equivalent to 10,000 nuclear power plants) which is considered to
originate mainly from the radioactive decay of elements like U, Th and K,
deposited in the crust and mantle of the Earth. Radioactivity of these elements
produce not only heat but also antineutrinos (called geo-antineutrinos) which
can be observed by terrestrial detectors. We investigate the possibility of
discriminating among Earth composition models predicting different total
radiogenic heat generation, by observing such geo-antineutrinos at Kamioka and
Gran Sasso, assuming KamLAND and Borexino (type) detectors, respectively, at
these places. By simulating the future geo-antineutrino data as well as reactor
antineutrino background contributions, we try to establish to which extent we
can discriminate among Earth composition models for given exposures (in units
of kt yr) at these two sites on our planet. We use also information on
neutrino mixing parameters coming from solar neutrino data as well as KamLAND
reactor antineutrino data, in order to estimate the number of geo-antineutrino

Benchmark Parameters for CMB Polarization Experiments
The recently detected polarization of the cosmic microwave background (CMB)
holds the potential for revealing the physics of inflation and gravitationally
mapping the large-scale structure of the universe, if so called B-mode signals
below 10^{-7}, or tenths of a uK, can be reliably detected. We provide a
language for describing systematic effects which distort the observed CMB
temperature and polarization fields and so contaminate the B-modes. We identify
7 types of effects, described by 11 distortion fields, and show their
association with known instrumental systematics such as common mode and
differential gain fluctuations, line cross-coupling, pointing errors, and
differential polarized beam effects. Because of aliasing from the small-scale
structure in the CMB, even uncorrelated fluctuations in these effects can
affect the large-scale B modes relevant to gravitational waves. Many of these
problems are greatly reduced by having an instrumental beam that resolves the
primary anisotropies (FWHM << 10'). To reach the ultimate goal of an
inflationary energy scale of 3 \times 10^{15} GeV, polarization distortion
fluctuations must be controlled at the 10^{-2}-10^{-3} level and temperature
leakage to the 10^{-4}-10^{-3} level depending on effect. For example pointing
errors must be controlled to 1.5'' rms for arcminute scale beams or a percent
of the Gaussian beam width for larger beams; low spatial frequency differential
gain fluctuations or line cross-coupling must be eliminated at the level of

Gamma-Ray Bursts: The Underlying Model
A pedagogical derivation is presented of the ``fireball'' model of gamma-ray
bursts, according to which the observable effects are due to the dissipation of
the kinetic energy of a relativistically expanding wind, a ``fireball.'' The
main open questions are emphasized, and key afterglow observations, that
provide support for this model, are briefly discussed. The relativistic outflow
is, most likely, driven by the accretion of a fraction of a solar mass onto a
newly born (few) solar mass black hole. The observed radiation is produced once
the plasma has expanded to a scale much larger than that of the underlying
``engine,'' and is therefore largely independent of the details of the
progenitor, whose gravitational collapse leads to fireball formation. Several
progenitor scenarios, and the prospects for discrimination among them using
future observations, are discussed. The production in gamma- ray burst
fireballs of high energy protons and neutrinos, and the implications of burst
neutrino detection by kilometer-scale telescopes under construction, are

Surgical site infection following surgery for hand trauma: a systematic review and meta-analysis
Surgical site infection is the most common healthcare-associated infection. Surgical site infection after surgery for hand trauma is associated with increased antibiotic prescribing, re-operation, hospital readmission and delayed rehabilitation, and in severe cases may lead to amputation. As the risk of surgical site infection after surgery for hand trauma remains unclear, we performed a systematic review and meta-analysis of all primary studies of hand trauma surgery, including randomized controlled trials, cohort studies, case-control studies and case series. A total of 8836 abstracts were screened, and 201 full studies with 315,618 patients included. The meta-analysis showed a 10% risk of surgical site infection in randomized control trials, with an overall risk of 5% when all studies were included. These summary statistics can be used clinically for informed consent and shared decision making, and for power calculations for future clinical trials of antimicrobial interventions in hand trauma
Decoherence in trapped ions due to polarization of the residual background gas
We investigate the mechanism of damping and heating of trapped ions
associated with the polarization of the residual background gas induced by the
oscillating ions themselves. Reasoning by analogy with the physics of surface
electrons in liquid helium, we demonstrate that the decay of Rabi oscillations
observed in experiments on 9Be+ can be attributed to the polarization phenomena
investigated here. The measured sensitivity of the damping of Rabi oscillations
with respect to the vibrational quantum number of a trapped ion is also
