Cosmology with redshift surveys of radio sources
We use the K-z relation for radio galaxies to illustrate why it has proved
difficult to obtain definitive cosmological results from studies based entirely
on catalogues of the brightest radio sources, e.g. 3C. To improve on this
situation we have been undertaking redshift surveys of complete samples drawn
from the fainter 6C and 7C radio catalogues. We describe these surveys, and
illustrate the new studies they are allowing. We also discuss our `filtered' 6C
redshift surveys: these have led to the discovery of a radio galaxy at z=4.4,
and are sensitive to similar objects at higher redshift provided the space
density of these objects is not declining too rapidly with z. There is
currently no direct evidence for a sharp decline in the space density of radio
galaxies for z > 4, a result only barely consistent with the observed decline

The Complete Star Formation History of the Universe
The determination of the star-formation history of the Universe is a key goal
of modern cosmology, as it is crucial to our understanding of how structure in
the Universe forms and evolves. A picture has built up over recent years,
piece-by-piece, by observing young stars in distant galaxies at different times
in the past.
These studies indicated that the stellar birthrate peaked some 8 billion
years ago, and then declined by a factor of around ten to its present value.
Here we report on a new study which obtains the complete star formation history
by analysing the fossil record of the stellar populations of 96545 nearby
galaxies. Broadly, our results support those derived from high-redshift
galaxies elsewhere in the Universe. We find, however, that the peak of star
formation was more recent - around 5 billion years ago. Our study also shows
that the bigger the stellar mass of the galaxy, the earlier the stars were
formed. This striking result indicates a very different formation history for

Vigorous star formation hidden by dust in a galaxy at
Near-infrared surveys have revealed a substantial population of enigmatic
faint galaxies with extremely red optical-to-near-infrared colours and with a
sky surface density comparable to that of faint quasars. There are two
scenarios for these extreme colours: (i) these distant galaxies have formed
virtually all their stars at very high redshifts and, due to the absence of
recently formed stars, the colours are extremely red and (ii) these distant
galaxies contain large amounts of dust, severely reddening the rest-frame
UV--optical spectrum. HR10 () is considered the archetype of the
extremely red galaxies. Here we report the detection of the continuum emission
from HR10 at 850m and at 1250m, demonstrating that HR10 is a very
dusty galaxy undergoing a major episode of star formation. Our result provides
a clear example of a high-redshift galaxy where the star formation rate
inferred from the ultraviolet luminosity would be underestimated by a factor up
to 1000, and shows that great caution should be used to infer the global star

Bilateral anterior dislocation of the shoulders at the start of a backstroke competition
Bilateral anterior dislocation of the shoulders is very rare. A 20-year-old man presented with bilateral anterior shoulder dislocation as a result of a diving incident. He complained of pain and restriction of movement in both shoulders with abducted and externally rotated arms. Radiographs revealed that the shoulders were dislocated. The patient was treated with closed reduction and was able to resume swimming 3 months later. To our knowledge, this is the first bilateral anterior dislocation of the shoulders during a backstroke swimming competition that was caused by this mechanism of injury. The rarity of this lesion and its uncommon mechanism prompted us to relate this observation
Early star-forming galaxies and the reionization of the Universe
Star forming galaxies represent a valuable tracer of cosmic history. Recent
observational progress with Hubble Space Telescope has led to the discovery and
study of the earliest-known galaxies corresponding to a period when the
Universe was only ~800 million years old. Intense ultraviolet radiation from
these early galaxies probably induced a major event in cosmic history: the
reionization of intergalactic hydrogen. New techniques are being developed to
understand the properties of these most distant galaxies and determine their

Satellite Observations of Separator Line Geometry of Three-Dimensional Magnetic Reconnection
Detection of a separator line that connects magnetic nulls and the
determination of the dynamics and plasma environment of such a structure can
improve our understanding of the three-dimensional (3D) magnetic reconnection
process. However, this type of field and particle configuration has not been
directly observed in space plasmas. Here we report the identification of a pair
of nulls, the null-null line that connects them, and associated fans and spines
in the magnetotail of Earth using data from the four Cluster spacecraft. With
di and de designating the ion and electron inertial lengths, respectively, the
separation between the nulls is found to be ~0.7di and an associated
oscillation is identified as a lower hybrid wave with wavelength ~ de. This in
situ evidence of the full 3D reconnection geometry and associated dynamics
provides an important step toward to establishing an observational framework of

Old Galaxies in the Young Universe
More than half of all stars in the local Universe are found in massive
spheroidal galaxies, which are characterized by old stellar populations with
little or no current star formation. In present models, such galaxies appear
rather late as the culmination of a hierarchical merging process, in which
larger galaxies are assembled through mergers of smaller precursor galaxies.
But observations have not yet established how, or even when, the massive
spheroidals formed, nor if their seemingly sudden appearance when the Universe
was about half its present age (at redshift z \approx 1) results from a real
evolutionary effect (such as a peak of mergers) or from the observational
difficulty of identifying them at earlier epochs. Here we report the
spectroscopic and morphological identification of four old, fully assembled,
massive (>10^{11} solar masses) spheroidal galaxies at 1.6<z<1.9, the most
distant such objects currently known. The existence of such systems when the
Universe was only one-quarter of its present age, shows that the build-up of
massive early-type galaxies was much faster in the early Universe than has been

Effectiveness and cost-effectiveness of a patient-initiated botulinum toxin treatment model for blepharospasm and hemifacial spasm: a study protocol for a randomised controlled trial
Background
Blepharospasm and hemifacial spasm are debilitating conditions that significantly impact on patient quality of life. Cyclical treatment with botulinum toxin injections offers temporary relief, but the duration of treatment efficacy is variable. The standard model of patient care defines routine fixed-time based scheduled treatment cycles which may lead to unnecessarily frequent treatment for some patients and experience of distressing symptoms in others, if symptoms return before the scheduled follow-up period.
Methods/Design
A randomised controlled trial will compare a patient-initiated model of care, where patients determine botulinum toxin treatment timing, to the standard model of care in which care is scheduled by the clinical team. A sample of 266 patients with blepharospasm or hemifacial spasm will be recruited from Moorfields Eye Hospital (MEH), London. The trial will be accompanied by a mixed methods evaluation of acceptability of the new service. Patients who meet eligibility criteria will be assessed at baseline and those in the intervention group will be provided instructions on how to book their own treatment appointments. Patients in both groups will be followed up 3 and 9 months into the trial and all patients will be returned to usual care after 9 months to meet safety protocols. Primary outcome measures include disease severity (questionnaire), functional disability (questionnaire) and patient satisfaction with care (questionnaire). Secondary outcomes include disease-specific quality of life (questionnaire), mood (questionnaire), illness and treatment perceptions (questionnaire and semi-structured interviews), economic impact (questionnaire) and acceptability (questionnaire and semi-structured interviews).
Discussion
This trial will assess the effectiveness and cost-effectiveness of a patient-led care model for botulinum toxin therapy. If the new model is shown to be effective in reducing distress and disability in these populations and is found to be acceptable to patients, whilst being cost-effective, this will have significant implications for service organisation across the NHS.

Carrion Availability in Space and Time
Introduction
Availability of carrion to scavengers is a central issue in carrion ecology and management, and is crucial for understanding the evolution of scavenging behaviour. Compared to live animals, their carcasses are relatively unpredictable in space and time in natural conditions, with a few exceptions (see below, especially Sect. “Carrion Exchange at the Terrestrial-Aquatic Interface”). Carrion is also an ephemeral food resource due to the action of a plethora of consumers, from microorganisms to large vertebrates, as well as to desiccation (i.e., loss of water content; DeVault et al. 2003; Beasley et al. 2012; Barton et al. 2013; Moleón et al. 2014). With a focus on vertebrate carcasses, here we give an overview of (a) the causes that produce carrion, (b) the rate of carrion production, (c) the factors affecting carrion quality, and (d) the distribution of carrion in space and time, both in terrestrial and aquatic environments (including their interface). In this chapter, we will focus on naturally produced carrion, whereas non-natural causes of animal mortality are described in chapter “Human-Mediated Carrion: Effects on Ecological Processes”. However, throughout this chapter we also refer to extensive livestock carrion, because in the absence of strong restrictions such as those imposed in the European Community after the bovine spongiform encephalopathy crisis (Donázar et al. 2009; Margalida et al. 2010), the spatiotemporal availability of carrion of extensive livestock and wild ungulates is similar
Knowledge synthesis of benefits and adverse effects of measles vaccination: the Lasbela balance sheet
<p>Abstract</p> <p>Background</p> <p>In preparation for a cluster-randomized controlled trial of a community intervention to increase the demand for measles vaccination in Lasbela district of Pakistan, a balance sheet summarized published evidence on benefits and possible adverse effects of measles vaccination.</p> <p>Methods</p> <p>The balance sheet listed: 1) major health conditions associated with measles; 2) the risk among the unvaccinated who contract measles; 3) the risk among the vaccinated; 4) the risk difference between vaccinated and unvaccinated; and 5) the likely net gain from vaccination for each condition.</p> <p>Results</p> <p>Two models revealed very different projections of net gain from measles vaccine. A Lasbela-specific combination of low period prevalence of measles among the unvaccinated, medium vaccination coverage and low vaccine efficacy rate, as revealed by the baseline survey, resulted in less-than-expected gains attributable to vaccination. Modelled on estimates where the vaccine had greater efficacy, the gains from vaccination would be more substantial.</p> <p>Conclusion</p> <p>Specific local conditions probably explain the low rates among the unvaccinated while the high vaccine failure rate is likely due to weaknesses in the vaccination delivery system. Community perception of these realities may have had some role in household decisions about whether to vaccinate, although the major discouraging factor was inadequate access. The balance sheet may be useful as a communication tool in other circumstances, applied to up-to-date local evidence.</p
