O Banco Mundial E O Sistema Único De Saúde Brasileiro No Início Do Século Xxi

This essay has the aim of updating discussions on the political perspective and the role played by the World Bank in the development of public health policies in Brazil, seeking to identify continuities and changes in the way this institution acts and suggest hypotheses about action strategies in this new century. To do this, we analyzed a 2007 and a 2013 document published by that institution, and gather data on projects funded by the World Bank from 2000 to 2015 in Brazil, with emphasis on the healthcare industry. We concluded that the traditional mechanisms of action have not changed from those used in the 1980s and 1990s, as well as the guiding principles; what we observed is that the World Bank’s actions have shifted from the national level to the state and municipal level. We indicate the need for specific studies of the agreements between the Bank and subnational governments, since the Brazilian federative model and the national health system itself allow implementing decentralized management mechanisms that can alter the setting of the Unified Health System. © 2016, UNIV SAOPAULO. Apoio Matricial No Sus Campinas: Análise Da Consolidação De Uma Prática Interprofissional Na Rede De Saúde

This study aims to characterize the teams and the inter-professional work process of Matrix Support developed and practiced in primary healthcare provided by the Brazilian Unified Health System (SUS) in Campinas, São Paulo State, Brazil. This is an exploratory descriptive study involving a questionnaire that was applied to 232 professionals who practice Matrix Support for primary healthcare. For analysis, the data were grouped into four categories: Identification of the professional; Work links to the Campinas SUS; Organization of the Matrix Support work; and the Support practice. The study indicates that the methodology of support for inter-professional work has achieved an important degree of consolidation in the municipality, in spite of the restricted investment. The reduced working time dedicated to support, and the large number of teams supported by each Matrix Support team were identified as the principal points of fragility in the work process. In turn, strong points that emerged were the multiplicity of tools used, the possibility of shared construction of work guidelines, and the flexibility in the composition of the support teams. Both the fragilities and the potentialities found can offer inputs for reflection and full creation of Matrix Support in other contexts. © 2016, Associacao Brasileira de Pos - Graduacao em Saude Coletiva. O cuidado em saúde mental pelos agentes comunitários de saúde: o que aprendem em seu cotidiano de trabalho?

Este trabalho visa descrever o conhecimento em saúde mental construído pelo agente comunitário de saúde, concomitante à produção de cuidado em saúde mental. Trata-se de pesquisa qualitativa, cujo referencial teórico é o Construcionismo Social. Os dados foram produzidos em grupos focais, analisados e elencados nas seguintes categorias temáticas: “A gente precisa orientar as famílias”, que agrupa os entidos relacionados à construção do cuidado às famílias que convivem com o sofrimento mental; “Só de você parar e ouvir...”, onde se descrevem os repertórios relacionados ao uso de tecnologias relacionais de cuidado; “Nós sabemos disso, porque nós andamos ali”, que delineia as estratégias para construção de saberes e, finalmente, a categoria que descreve os sentidos do medo do louco: “A gente tem medo daquilo que a gente vê”. Conclui-se que os conhecimentos construídos no cotidiano de trabalho dos agentes comunitários de saúde, quando refletidos e sistematizados, são potentes para a produção de práticas de cuidado condizentes com o paradigma psicossocial de atenção a pessoas e famílias em sofrimento mental
