    Apicotomy: a root apical fracture for surgical treatment of impacted upper canines

    Impacted canines, due to systemic or local factors, represent a frequent problem in most populations. Surgical intervention usually involves exposure for spontaneous eruption, exposure for orthodontic traction or extraction. The author presents the apicotomy technique, which has been successfully used during the past twenty years for conservative intervention in cases of impacted upper canines with dilaceration or apical root-ankylosis.     Variability in depression prevalence in early rheumatoid arthritis: a comparison of the CES-D and HAD-D Scales

    <p>Abstract</p> <p>Background</p> <p>Depression is common in rheumatoid arthritis (RA), however reported prevalence varies considerably. Two frequently used instruments to identify depression are the Center for Epidemiological Studies Depression (CES-D) scale, and the Hospital Anxiety and Depression Scale (HADS). The objectives of this study were to test if the CES-D and HADS-D (a) satisfy current modern psychometric standards for unidimensional measurement in an early RA sample; (b) measure the same construct (i.e. depression); and (c) identify similar levels of depression.</p> <p>Methods</p> <p>Data from the two scales completed by patients with early RA were fitted to the Rasch measurement model to show that (a) each scale satisfies the criteria of fit to the model, including strict unidimensionality; (b) that the scales can be co-calibrated onto a single underlying continuum of depression and to (c) examine the location of the cut points on the underlying continuum as indication of the prevalence of depression.</p> <p>Results</p> <p>Ninety-two patients with early RA (62% female; mean age = 56.3, SD = 13.7) gave 141 sets of paired CES-D and HAD-D data. Fit of the data from the CES-D was found to be poor, and the scale had to be reduced to 13 items to satisfy Rasch measurement criteria whereas the HADS-D met model expectations from the outset. The 20 items combined (CES-D13 and HADS-D) satisfied Rasch model expectations. The CES-D gave a much higher prevalence of depression than the HADS-D.</p> <p>Conclusion</p> <p>The CES-D in its present form is unsuitable for use in patients with early RA, and needs to be reduced to a 13-item scale. The HADS-D is valid for early RA and the two scales measure the same underlying construct but their cut points lead to different estimates of the level of depression. Revised cut points on the CES-D13 provide comparative prevalence rates.</p

    Characterization of geological materials using ion and photon beams.

    Geological specimens are often complex materials that require different analytical methods for their characterization. The parameters of interest may include the chemical composition of major, minor and trace elements. The chemical compounds incorporated in the minerals, the crystal structure and isotopic composition need to be considered. Specimens may be highly heterogeneous thus necessitating analytical methods capable of measurements on small sample volumes with high spatial resolution and sensitivity. Much essential information on geological materials can be obtained by using ion or photon beams. In this chapter we describe the principal analytical techniques based on particle accelerators, showing some applications that are hardly possible with conventional methods. In particular, the following techniques will be discussed: (1) Synchrotron radiation (SR) induced X-ray emission (SRIXE) and particle-induced X-ray emission (PEE) and other ion beam techniques for trace element analysis. (2) Accelerator mass spectrometry (AMS) for ultra sensitive analysis of stable nuclides and long-lived radionuclides. In most of the cases also the possibilities of elemental and isotopic analysis with high resolution will be discussed

    Mechanisms, impact and management of pain in rheumatoid arthritis

People with rheumatoid arthritis (RA) identify pain as their most important symptom, one that often persists despite optimal control of inflammatory disease. RA pain arises from multiple mechanisms, involving inflammation, peripheral and central pain processing and, with disease progression, structural change within the joint. Consequently, RA pain has a wide range of characteristics-constant or intermittent, localized or widespread-and is often associated with psychological distress and fatigue. Dominant pain mechanisms in an individual are identified by critical evaluation of clinical symptoms and signs, and by laboratory and imaging tests. Understanding these mechanisms is essential for effective management, although evidence from preclinical models should be interpreted with caution. A range of pharmacological analgesic and immunomodulatory agents, psychological interventions and surgery may help manage RA pain. Pain contributes importantly to the clinical assessment of inflammatory disease activity, and noninflammatory components of RA pain should be considered when gauging eligibility for or response to biologic agents. Further randomized controlled trials are required to determine the optimal usage of analgesics in RA, and novel agents with greater efficacy and lower propensity for adverse events are urgently needed. Meanwhile, targeted use of existing treatments could reduce pain in people with RA
