    Efficiency of spinal anesthesia versus general anesthesia for lumbar spinal surgery: a retrospective analysis of 544 patients.

    BACKGROUND: Previous studies have shown varying results in selected outcomes when directly comparing spinal anesthesia to general in lumbar surgery. Some studies have shown reduced surgical time, postoperative pain, time in the postanesthesia care unit (PACU), incidence of urinary retention, postoperative nausea, and more favorable cost-effectiveness with spinal anesthesia. Despite these results, the current literature has also shown contradictory results in between-group comparisons. MATERIALS AND METHODS: A retrospective analysis was performed by querying the electronic medical record database for surgeries performed by a single surgeon between 2007 and 2011 using procedural codes 63030 for diskectomy and 63047 for laminectomy: 544 lumbar laminectomy and diskectomy surgeries were identified, with 183 undergoing general anesthesia and 361 undergoing spinal anesthesia (SA). Linear and multivariate regression analyses were performed to identify differences in blood loss, operative time, time from entering the operating room (OR) until incision, time from bandage placement to exiting the OR, total anesthesia time, PACU time, and total hospital stay. Secondary outcomes of interest included incidence of postoperative spinal hematoma and death, incidence of paraparesis, plegia, post-dural puncture headache, and paresthesia, among the SA patients. RESULTS: SA was associated with significantly lower operative time, blood loss, total anesthesia time, time from entering the OR until incision, time from bandage placement until exiting the OR, and total duration of hospital stay, but a longer stay in the PACU. The SA group experienced one spinal hematoma, which was evacuated without any long-term neurological deficits, and neither group experienced a death. The SA group had no episodes of paraparesis or plegia, post-dural puncture headaches, or episodes of persistent postoperative paresthesia or weakness. CONCLUSION: SA is effective for use in patients undergoing elective lumbar laminectomy and/or diskectomy spinal surgery, and was shown to be the more expedient anesthetic choice in the perioperative setting

    Phenomenology of Mirror Fermions in the Littlest Higgs Model with T-Parity

    Little Higgs models are an interesting alternative to explain electroweak symmetry breaking without fine-tuning. Supplemented with a discrete symmetry (T-parity) constraints from electroweak precision data are naturally evaded and also a viable dark matter candidate is obtained. T-parity implies the existence of new (mirror) fermions in addition to the heavy gauge bosons of the little Higgs models. In this paper we consider the effects of the mirror fermions on the phenomenology of the littlest Higgs model with T-parity at the LHC. We study the most promising production channels and decay chains for the new particles. We find that the mirror fermions have a large impact on the magnitude of signal rates and on the new physics signatures. Realistic background estimates are given.Comment: 13 p

    Phenomenology of neutral heavy leptons

    We continue our previous work on the flavour-conserving leptonic decays of the Z boson with neutral heavy leptons (NHL's) in the loops by considering box, vertex, and self-energy diagrams for the muon decay. By inclusion of these loops (they contribute to the input parameter M_W) we can probe the full parameter space spanned by the so-called flavour-conserving mixing parameters ee_(mix), \mu\mu_(mix), \tau\tau_(mix) in a superstring-inspired model of neutrino mass. We compare the results of our analysis with the existing work in this field and conclude that flavour-conserving decays have certain advantages over traditionally considered flavour-violating ones.Comment: submitted to Phys.     Single-cell RNA-sequencing of herpes simplex virus 1-infected cells connects NRF2 activation to an antiviral program

    Herpesvirus infection initiates a range of perturbations in the host cell, which remain poorly understood at the level of individual cells. Here, we quantify the transcriptome of single human primary fibroblasts during the first hours of lytic infection with HSV-1. By applying a generalizable analysis scheme, we define a precise temporal order of early viral gene expression and propose a set-wise emergence of viral genes. We identify host cell genes and pathways relevant for infection by combining three different computational approaches: gene and pathway overdispersion analysis, prediction of cell-state transition probabilities, as well as future cell states. One transcriptional program, which correlates with increased resistance to infection, implicates the transcription factor NRF2. Consequently, Bardoxolone methyl and Sulforaphane, two known NRF2 agonists, impair virus production, suggesting that NRF2 activation restricts viral infection. Our study provides insights into early stages of HSV-1 infection and serves as a general blueprint for the investigation of heterogeneous cell states in virus infection

    The magnetic dipole moments of the light tensor mesons f2f_2, a2a_2 and strange K20(1430)K_2^{\ast 0} (1430) tensor meson are calculated in the framework of the light cone QCD sum rules. It is observed that the values of the magnetic dipole moment for the charged tensor particles are considerably different from zero.     Migration without interbreeding: Evolutionary history of a highly selfing Mediterranean grass inferred from whole genomes

    Wild plant populations show extensive genetic subdivision and are far from the ideal of panmixia which permeates population genetic theory. Understanding the spatial and temporal scale of population structure is therefore fundamental for empirical population genetics –and of interest in itself, as it yields insights into the history and biology of a species. In this study we extend the genomic resources for the wild Mediterranean grass Brachypodium distachyon to investigate the scale of population structure and its underlying history at whole-genome resolution. A total of 86 accessions were sampled at local and regional scales in Italy and France, which closes a conspicuous gap in the collection for this model organism. The analysis of 196 accessions, spanning the Mediterranean from Spain to Iraq, suggests that the interplay of high selfing and seed dispersal rates has shaped genetic structure in B. distachyon. At the continental scale, the evolution in B. distachyon is characterized by the independent expansion of three lineages during the Upper Pleistocene. Today, these lineages may occur on the same meadow yet do not interbreed. At the regional scale, dispersal and selfing interact and maintain high genotypic diversity, thus challenging the textbook notion that selfing in finite populations implies reduced diversity. Our study extends the population genomic resources for B. distachyon and suggests that an important use of this wild plant model is to investigate how selfing and dispersal, two processes typically studied separately, interact in colonizing plant species

    We point out that the pure ``tree'' decays Bc±Ds±DB_c^\pm\to D^\pm_s D are particularly well suited to extract the CKM angle γ\gamma through amplitude relations. In contrast to conceptually similar strategies using B±K±DB^\pm\to K^\pm D or BdK0DB_d\to K^{\ast0} D decays, the advantage of the BcB_c approach is that the corresponding triangles have three sides of comparable length and do not involve small amplitudes. Decays of the type Bc±D±DB_c^\pm\to D^\pm D -- the UU-spin counterparts of Bc±Ds±DB_c^\pm\to D^\pm_s D -- can be added to the analysis, as well as channels, where the Ds±D^\pm_s- and D±D^\pm-mesons are replaced by higher resonances.Comment: 9 pages, LaTeX, 3 figures, reference adde
