    Tocilizumab is widely being used to treat COVID-19. Although reducing systemic inflammation, it also increases the risk for secondary infections as a result of the immunosuppression produced. We report the case of a 69-year-old patient admitted to the ICU with severe respiratory distress caused by COVID-19 pneumonia who developed pulmonary aspergillosis. On the basis of these findings, we suggest early testing for pulmonary aspergillosis in COVID-19 patients treated with tocilizumab

    Validação factorial do 'Spanish Burnout Inventory' em polícias portugueses.

    This study was designed to assess the factor structure of the Spanish Burnout Inventory in a sample of 245 Portuguese police officers working in Lisbon. Using exploratory factor analysis (EFA), 4 factors were extracted, reproducing the original model: Enthusiasm toward the job (5 items), Psychological exhaustion (4 items), Indolence (6 items), and Guilt (5 items). However, item 20 -designed to assess Guilt- was grouped with both items of Indolence and Psychological exhaustion. The results of the study support the factor validity of the SBI and the adequate internal consistency of its dimensions, with Cronbach Alpha higher than .70. The results suggest that SBI is an adequate instrument to assess burnout in Portuguese police officers

    On the Angular Resolution of the AGILE gamma-ray imaging detector

    We present a study of the Angular Resolution of the AGILE gamma-ray imaging detector (GRID) that is operational in space since April 2007. The AGILE instrument is made of an array of 12 planes each equipped with a Tungsten converter and Silicon micros trip detectors and is sensitive in the energy range 50 MeV - 10 GeV. Among the space instruments devoted to gamma-ray astrophysics, AGILE uniquely exploits an analog readout system with dedicated electronics coupled with Silicon detectors. We show the results of Monte Carlo simulations carried out to reproduce the gamma-ray detection by the GRID, and we compare them to in-flight data. We use the Crab (pulsar + Nebula) system for discussion of real data performance, since its E^{-2} energy spectrum is representative of the majority of gamma-ray sources. For Crab-like spectrum sources, the GRID angular resolution (FWHM of ~4deg at 100 MeV; ~0.8deg at 1 GeV; ~0.9deg integrating the full energy band from 100 MeV to tens of GeV) is stable across a large field of view, being characterized by a flat response up to 30deg off-axis. A comparison of the angular resolution obtained by the two operational gamma-ray instruments, AGILE-GRID and Fermi-LAT, is interesting in view of future gamma-ray missions, that are currently under study. The two instruments exploit different detector configurations affecting the angular resolution: the former being optimized in the readout and track reconstruction especially in the low-energy band, the latter in terms of converter thickness and power consumption. We show that, despite these differences, the angular resolution of both instruments is very similar between 100 MeV and a few GeV.Comment: 19 pages, 8 figures, accepted for publication in Ap

    Prevention of Ventricular Arrhythmias With Sarcoplasmic Reticulum Ca2+ ATPase Pump Overexpression in a Porcine Model of Ischemia Reperfusion

    Background— Ventricular arrhythmias are life-threatening complications of heart failure and myocardial ischemia. Increased diastolic Ca2+ overload occurring in ischemia leads to afterdepolarizations and aftercontractions that are responsible for cellular electric instability. We inquired whether sarcoplasmic reticulum Ca2+ ATPase pump (SERCA2a) overexpression could reduce ischemic ventricular arrhythmias by modulating Ca2+ overload.Methods and Results— SERCA2a overexpression in pig hearts was achieved by intracoronary gene delivery of adenovirus in the 3 main coronary arteries. Homogeneous distribution of the gene was obtained through the left ventricle. After gene delivery, the left anterior descending coronary artery was occluded for 30 minutes to induce myocardial ischemia followed by reperfusion. We compared this model with a model of permanent coronary artery occlusion. Twenty-four–hour ECG Holter recordings showed that SERCA2a overexpression significantly reduced the number of episodes of ventricular tachycardia after reperfusion, whereas no significant difference was found in the occurrence of sustained or nonsustained ventricular tachycardia and ventricular fibrillation in pigs undergoing permanent occlusion. Conclusions— We show that Ca2+ cycling modulation using SERCA2a overexpression reduces ventricular arrhythmias after ischemia-reperfusion. Strategies that modulate postischemic Ca2+ overload may have clinical promise for the treatment of ventricular arrhythmias

    Brazilian sedimentary zeolite use in agriculture.

    Zeolites are hydrated crystalline aluminosilicate minerals of alkaline and alkaline earth metals, structured in rigid three dimension nets, organized by AlO4 and SiO4 tetrahedral and are of natural occurrence. This report describes the characterization and application of the Brazilian zeolitic sedimentary rock as a release fertilizer and soil conditioner. The characterization of the head samples showed that it is composed of the zeolite stilbite intermixed with a smectitic clay mineral, and quartz. A low-cost quartz separation technique was established. Enrichment of concentrated natural zeolite was carried out: zeolite + KNO3, zeolite + K2HPO4 and zeolite + H3PO4 + apatite and the concentrated zeolite. These materials were tested with Rangpur lime rootstock and an experiment was also carried out with successive crops grown on the same substrate: lettuce, tomato, rice, and Andropogon grass. The results indicated that N, P and K enriched zeolite was an adequate slow-release source of nutrients to plants. Other green house and field experiments with concentrated zeolite applied with urea showed reduction of losses of ammonia by volatilization and improved in N use efficiency by maize. Concentrated zeolite also increased water retention and the available water capacity of a sand soil
