It is unlikely that they alone will cause a significant potential to underestimate fecal contamination using MUG as the primary criterion for that assessment.     Ixekizumab Efficacy and Safety with and without Concomitant Conventional Disease-modifying Antirheumatic Drugs (cDMARDs) in Biologic DMARD (bDMARD)-naive Patients with Active Psoriatic Arthritis (PsA): Results from SPIRIT-P1

    Objective: To evaluate the efficacy and safety of ixekizumab alone or with concomitant conventional disease-modifying antirheumatic drugs (cDMARDs) versus placebo in patients with active psoriatic arthritis (PsA) as part of a SPIRIT-P1 subgroup analysis (NCT01695239). Methods: Patients were stratified by cDMARD use (concomitant cDMARDs use (including methotrexate) or none (past or naive use)) and randomly assigned to treatment groups (ixekizumab 80 mg every 4 weeks (IXEQ4W) or every 2 weeks (IXEQ2W) or placebo). Efficacy was evaluated versus placebo at week 24 by the American College of Rheumatology criteria (ACR20/50/70), modified total Sharp score and Health Assessment Questionnaire-Disability Index (HAQ-DI). Safety was assessed according to cDMARD status. Results: Regardless of concomitant cDMARD usage, ACR20, ACR50 and ACR70 response rates were significantly higher versus placebo with IXEQ4W and IXEQ2W. The proportion of patients achieving HAQ-DI minimal clinically important difference was significantly higher versus placebo with IXEQ4W with concomitant cDMARD use and IXEQ2W, regardless of concomitant cDMARD use. Treatment-emergent adverse events (AE) were more frequent versus placebo for either ixekizumab-dosing regimen, regardless of concomitant cDMARD use. Serious AEs were not higher versus placebo, regardless of concomitant cDMARD use. Conclusion: Ixekizumab treatment improved measures of disease activity and physical function in patients with active PsA relative to placebo, when used with or without concomitant cDMARD therapy

    Smartphone Mobile Application to Enhance Diagnosis of Skin Cancer: A Guide for the Rural Practitioner

    Primary care physicians occupy a vital position to impact many devastating conditions, especially those dependent upon early diagnosis, such as skin cancer. Skin cancer is the most common cancer in the United States and despite improvements in skin cancer therapy, patients with a delay in diagnosis and advanced disease continue to have a grave prognosis. Due to a variety of barriers, advanced stages of skin cancer are more prominent in rural populations. In order to improve early diagnosis four things are paramount: increased patient participation in prevention methods, establishment of screening guidelines, increased diagnostic accuracy of malignant lesions, and easier access to dermatologists. Recent expansion in smartphone mobile application technology offers simple ways for rural practitioners to address these problems. More than 100,000 health related applications are currently available, with over 200 covering dermatology. This review will evaluate the newest and most useful of those applications offered to enhance the prevention and early diagnosis of skin cancer, particularly in the rural population

    Quaking Aspen in the Residential-Wildland Interface: Elk Herbivory Hinders Forest Conservation

    Quaking aspen (Populus tremuloides Michx.) forests are experiencing numerous impediments across North America. In the West, recent drought, fire suppression, insects, diseases, climate trends, inappropriate management, and ungulate herbivory are impacting these high biodiversity forests. Additionally, ecological tension zones are sometimes created where the above factors intermingle with jurisdictional boundaries. The public-private land interface may result in stress to natural areas where game species find refuge and plentiful forage at the expense of ecosystem function. We examined putative herbivore impacts to aspen forests at Wolf Creek Ranch (WCR), a large residential landscape in northern Utah. Forty-three ha-1 monitoring plots were established to measure a range of attributes summarizing location description, tree and vegetation condition, and herbivore presence. Additionally, we tested the ability of a stand-level visual rating system to represent more detailed field measures. Elk (Cervus elaphus L.) herbivory is currently having a strong effect on aspen in the study area, reducing many locations to single-layer aspen forests dominated by aging canopy trees. Regeneration (\u3c 2 m stems) is experiencing moderate-to-high browse and recruitment (2 - 6 m stems) are below replacement levels on approximately half of WCR\u27s aspen forests. The condition rating system represented significant trends in forest cover, canopy height, stand aspect, regeneration, recruitment, and tree mortality. Ordination of all plot and forest data found a strong negative relationship between elk presence and recruitment success. We make recommendations for addressing difficult herbivore-aspen interactions where publicly managed wildlife a present barriers to conservation of privately owned forest reserves
