    Artificial Intelligence systems are increasingly playing an increasingly important role in our daily lives. As their importance in our everyday lives grows, it is fundamental that the internal mechanisms that guide these algorithms are as clear as possible. It is not by chance that the recent General Data Protection Regulation (GDPR) emphasized the users' right to explanation when people face artificial intelligencebased technologies. Unfortunately, the current research tends to go in the opposite direction, since most of the approaches try to maximize the effectiveness of the models (e.g., recommendation accuracy) at the expense of the explainability and the transparency. The main research questions which arise from this scenario is straightforward: how can we deal with such a dichotomy between the need for effective adaptive systems and the right to transparency and interpretability? Several research lines are triggered by this question: building transparent intelligent systems, analyzing the impact of opaque algorithms on final users, studying the role of explanation strategies, investigating how to provide users with more control in the behavior of intelligent systems. XAI.it, the Italian workshop on Explainable AI, tries to address these research lines and aims to provide a forum for the Italian community to discuss problems, challenges and innovative approaches in the various sub-fields of XAI

    Gynecological surgical interventions (conservative and non-conservative): Clinical psychological variables and symptomatology pertaining to perioperative period

    Objective: After considering great inter-individual variability of subjective experience and clinical course in reference to conservative and non-conservative gynecological surgical interventions, an attempt was made to evaluate potential role of several clinical and psychological variables with respect to perioperative symptomatology course and illness behaviour. Materials and methods: The sample consists of 58 women (mean age 41.4 ± 8.7) undergoing gynecological surgical interventions (conservative and non-conservative ones) for benign pathologies. The anamnestic and clinical data (psychological anamnesis, clinical history, indications, methods and typology of the intervention) were collected using a specifically designed summary form. For the evaluation of pre-and post-operative symptomatology course and illness behaviour, the following psychological tests were respectively used: The Symptom Questionnaire (SQ), with 3 planned administrations (respectively 15 days before the intervention, a day before the intervention and at discharge) and the Illness Behavior Questionnaire (IBQ), completed before discharge. Non-parametric tests (Mann-Whitney and Kruskal-Wallis) were used to compare performances on independent samples. Results: Results show that perioperative course and illness behaviour in the cases of gynecological surgical interventions for benign pathologies depend on clinical variables, that is, typology and methods of the intervention, clinical history, psychopathological anamnesis. Conclusion: Findings suggest the importance of clinical-anamnestic inquiry oriented towards the evaluation of variables that emerged as risk factors, with the goal of planning personalized support interventions for preventing and/or reducing distress and impact on psychophysical wellbeing arising after gynaecological surgical interventions

    A longitudinal assessment of chronic care pathways in real-life: self-care and outcomes of chronic heart failure patients in Tuscany

    Background: Worldwide healthcare systems face challenges in assessing and monitoring chronic care pathways and, even more, the value generated for patients. Patient-reported outcomes measures (PROMs) represent a valid Real-World Evidence (RWE) source to fully assess health systems’ performance in managing chronic care pathways. Methods: The originality of the study consists in the chance of adopting PROMs, as a longitudinal assessment tool for continuous monitoring of patients’ adherence to therapies and self-care behavior recommendations in clinical practice and as a chance to provide policy makers insights to improve chronic pathways adopting a patient perspective. The focus was on PROMs of patients with chronic heart failure (CHF) collected in the Gabriele Monasterio Tuscan Foundation (FTGM), a tertiary referral CHF centre in Pisa, Italy. During the hospital stay, CHF patients were enrolled and received a link (via SMS or email) to access to the first questionnaire. Follow-up questionnaires were sent 1, 7 and 12 months after the index hospitalisation. Professionals invited 200 patients to participate to PROMs surveys. 174 answers were digitally collected at baseline from 2018 to 2020 and analysed. Quantitative and qualitative analyses were conducted, using Chi2, t-tests and regression models together with narrative evidence from free text responses. Results: Both quantitative and qualitative results showed FTGM patients declared to strongly adhere to the pharmacological therapy across the entire pathway, while seemed less careful to adhere to self-care behavior recommendations (e.g., physical activity). CHF patients that performed adequate Self-Care Maintenance registered outcome improvements. Respondents declared to be supported by family members in managing their adherence. Conclusions: The features of such PROMs collection model are relevant for researchers, policymakers and for managers to implement interventions aimed at improving pathway adherence dimensions. Among those, behavioral economics interventions could be implemented to increase physical activity among CHF patients since proven successful in Tuscany. Strategies to increase territorial care and support patients’ caregivers in their daily support to patients’ adherence should be further explored. Systematic PROMs collection would allow to monitor changes in the whole pathway organization. This study brings opportunities for extending such monitoring systems to other organizations to allow for reliable benchmarking opportunities

    Reconstructive nature of temporal memory for movie scenes

    Remembering when events took place is a key component of episodic memory. Using a sensitive behavioral measure, the present study investigates whether spontaneous event segmentation and script-based prior knowledge affect memory for the time of movie scenes. In three experiments, different groups of participants were asked to indicate when short video clips extracted from a previously encoded movie occurred on a horizontal timeline that represented the video duration. When participants encoded the entire movie, they were more precise at judging the temporal occurrence of clips extracted from the beginning and the end of the film compared to its middle part, but also at judging clips that were closer to event boundaries. Removing the final part of the movie from the encoding session resulted in a systematic bias in memory for time. Specifically, participants increasingly underestimated the time of occurrence of the video clips as a function of their proximity to the missing part of the movie. An additional experiment indicated that such an underestimation effect generalizes to different audio-visual material and does not necessarily reflect poor temporal memory. By showing that memories are moved in time to make room for missing information, the present study demonstrates that narrative time can be adapted to fit a standard template regardless of what has been effectively encoded, in line with reconstructive theories of memory

    Tight Regulation of Mechanotransducer Proteins Distinguishes the Response of Adult Multipotent Mesenchymal Cells on PBCE-Derivative Polymer Films with Different Hydrophilicity and Stiffness

    : Mechanotransduction is a molecular process by which cells translate physical stimuli exerted by the external environment into biochemical pathways to orchestrate the cellular shape and function. Even with the advancements in the field, the molecular events leading to the signal cascade are still unclear. The current biotechnology of tissue engineering offers the opportunity to study in vitro the effect of the physical stimuli exerted by biomaterial on stem cells and the mechanotransduction pathway involved in the process. Here, we cultured multipotent human mesenchymal/stromal cells (hMSCs) isolated from bone marrow (hBM-MSCs) and adipose tissue (hASCs) on films of poly(butylene 1,4-cyclohexane dicarboxylate) (PBCE) and a PBCE-based copolymer containing 50 mol% of butylene diglycolate co-units (BDG50), to intentionally tune the surface hydrophilicity and the stiffness (PBCE = 560 Mpa; BDG50 = 94 MPa). We demonstrated the activated distinctive mechanotransduction pathways, resulting in the acquisition of an elongated shape in hBM-MSCs on the BDG50 film and in maintaining the canonical morphology on the PBCE film. Notably, hASCs acquired a new, elongated morphology on both the PBCE and BDG50 films. We found that these events were mainly due to the differences in the expression of Cofilin1, Vimentin, Filamin A, and Talin, which established highly sensitive machinery by which, rather than hASCs, hBM-MSCs distinguished PBCE from BDG50 films

    COVID-19 vaccinations: An overview of the Italian national health system's online communication from a citizen perspective

    COVID-19 vaccine hesitancy is still widespread. During the pandemic, the internet has been the preferred channel for health-related information, especially for less-educated citizens who tend to be the most hesitant about vaccination. A well-structured web communication strategy could help both to overcome vaccine hesitancy and to ensure equity in healthcare service access. This study investigated how the various regional and local health authorities in Italy used their institutional websites to inform users about COVID-19 vaccinations between March and April 2021. We browsed 129 institutional websites, checking the availability, quality and quantity, actionability and readability of information using a literature-based common grid. Descriptive statistics and statistical tests were performed. The online public dissemination of COVID-19 vaccination information in Italy was fragmented, both across and within regions. The side effects of vaccinations, were often not reported on the websites, thus missing an opportunity to enhance vaccination uptake. More focus should also be placed on readability, since readability indexes showed that they were difficult to understand. Our research revealed that several actions could be implemented to enhance online communication on COVID-19 vaccination. For instance, simplifying texts can make them more understandable and the information reported actionable
