    Patient perspectives and experiences of remote consultations in people receiving kidney care: A scoping review

    Background: The COVID-19 pandemic resulted in a rapid and sometimes chaotic change in how clinical care was delivered for people living with kidney disease, with increased reliance on digital technologies and the introduction of remote services. Objective: To conduct a scoping review of studies about peoples’ experiences and perspectives in receipt of remote consultations for kidney care. Methods: Using Arksey and O’Malley’s (2005) framework, three databases (CINAHL, MEDLINE, and PsycINFO) were searched simultaneously on EBSCO The search included studies published in English from August 2010 to August 2021. Results: Eight cross-sectional studies met the scoping review criteria (two quantitative, two mixed-method, and four qualitative). Four themes were identified: overall satisfaction with remote services, benefits to patients (convenience, involvement in care, and patient safety), barriers to remote consultations (technical difficulties, digital literacy, loss of interpersonal communication, existing patient-practitioner relationship, and access to technology), and patients’ concerns (need for physical examination, privacy, and confidentiality). Conclusion: Remote consultations confer multiple advantages to patients; therefore, remote consultations should be offered as an option to patients living with kidney disease beyond the COVID-19 pandemic. However, there are several barriers to remote consultation that need to be addressed and understood before implementing remote care long-term. Future research should examine the impact of remote consultations on people living with kidney disease from under-served groups to identify barriers and ensure their suitability and accessibility to the wider population for a more patient-centred approach to kidney care

    Background: People of Black, Asian and minority ethnic (BAME) heritage have a higher-than-average incidence of, and mortality from hypertension and stroke. Therefore, it is important to identify new settings for engaging people at risk of high blood pressure (BP). Aim: This feasibility study aimed to evaluate if barbers in a London borough can support and educate men of BAME heritage to manage their BP. Following UK Medical Research Council guidance, the RE-AIM (reach, effectiveness, adoption, implementation, maintenance) framework was used to guide study objectives and feasibility outcomes. Methods: We collaborated with 8 barbers who were part of an existing BAME barber network. Barbers were trained online (1.5 h) and face-to-face (3 h) to provide BP healthcare advice and take customers BP readings. Qualitative field notes were collected to assess how best to recruit and train barbers, and to understand how to maintain motivation and retention of barbers. BP readings were recorded between June 2021 and March 2022. Results: Both online and face-to-face training were effective, however, greater focus on how to start conversations about BP with clients was needed. We found that motivation, incentivization and regular contact with barbers were important for recruitment, retention, and sustained BP measurement. Obtaining BP readings was challenging due to client concerns about recording their data and the impracticalities of recording results. We captured 236 BP recordings, of which 39 (16.53%) were over 140/90 mmHg; of these, 5 were over 180/100 mmHg. Conclusion: The combined data showed that educating barbers to take BP readings and deliver healthcare advice about BP is a viable intervention for rollout in a large-scale study. It has demonstrated the need to identify strategies to motivate barbers for sustained recruitment and retention, as well as further efforts to build trust among customers for long-term BP surveillance

    Spectral line shape of resonant four-wave mixing induced by broad-bandwidth lasers

    We present a theoretical and experimental study of the line shape of resonant four-wave mixing induced by broad-bandwidth laser radiation that revises the theory of Meacher, Smith, Ewart, and Cooper (MSEC) [Phys. Rev. A 46, 2718 (1992)]. We adopt the same method as MSEC but correct for an invalid integral used to average over the distribution of atomic velocities. The revised theory predicts a Voigt line shape composed of a homogeneous, Lorentzian component, defined by the collisional rate Γ, and an inhomogeneous, Doppler component, which is a squared Gaussian. The width of the inhomogeneous component is reduced by a factor of √2 compared to the simple Doppler width predicted by MSEC. In the limit of dominant Doppler broadening, the width of the homogeneous component is predicted to be 4Γ, whereas in the limit of dominant homogeneous broadening, the predicted width is 2Γ. An experimental measurement is reported of the line shape of the four-wave-mixing signal using a broad-bandwidth, "modeless", laser resonant with the Q1 (6) line of the A2 Σ - X2 Π(0,0) system of the hydroxyl radical. The measured widths of the Voigt components were found to be consistent with the predictions of the revised theory

    Genetic Evaluation of the Nine Component Features of Hip Score in UK Labrador Retrievers

    The aim of this study was to explore the genetic relationship between the nine component traits comprising the British Veterinary Association (BVA) total hip score in UK registered Labrador Retrievers. Data consisted of 11,928 single records of trait scores of dogs aged between one and four years (365–1459 days) old, from radiographs evaluated between 2000 and 2007. Pedigree information was provided by the UK Kennel Club. The distribution of trait scores showed only small numbers of dogs with visible malformation in the six traits that were scored according to the severity of osteoarthritis. Linear mixed models were fitted using ASREML. Estimates of heritability ranged from 0.15 to 0.38, and litter effects from 0.04 to 0.10. Genetic correlations between all nine traits were extremely high ranging from 0.71 to 1.0, implying considerable genetic similarity. The decomposition demonstrated that aggregate scores of only the 3 traits indicative of laxity in one year old dogs was predictive of the phenotype of the remaining six scored on osteoarthritic severity in dogs at 4+ years old. The application of selection index methodology in selecting against hip dysplasia using the trait scores was explored and potential improvements in accuracy (directly related to response to selection) of over 10% are reported compared to the current total hip score. This study demonstrates that traits descriptive of joint laxity are valuable early-age predictors of osteoarthritis and shows that there is scope for improvement in the way data from the UK hip score scheme are used for selection against hip dysplasia in Labradors. This was verified via use of selection indices, which identified substantial increases in accuracy, not only via optimum coefficients, but also through an easily applicable aggregate of scores of just two or three traits only compared with the current total hip score
