An exploration of knowledge and understanding – the eighth flow
The argument for understanding Lean construction as a socio-technical field is growing and the need to better consider the role of human beings within construction systems is becoming the dominant factor in project success. Many current attributes of lean already focus on people and on human engagement approaches but the field of lean construction addresses project environments that are often complex and highly variable. The authors argue that the successful delivery of these projects relies on the creation of a common understanding of the project objectives within the diverse value systems of project participants and wider society. Additionally, many of the new ways of working that lean thinking brings already support the creation of a common understanding and could be harnessed to better effect.
Based on a literature review and supported by case study examples the authors explore the nature of knowledge and understanding and position them within an eight flow model for construction production. The findings indicate a need to reconsider the development of a common understand for each project due to the tacit nature of experiential knowledge held within the project team and the specificity and complexity of the project environment. As a result effort is required to generate and maintain a common understanding throughout the project duration. The continued attention and action required to maintain this common understanding elevates it to a flow of equal status to those identified in Koskela’s flow production model thus increasing the number of flows to eight. A significant lean construction case study is revisited and examined to identify interventions undertaken to achieve this generation and management of common understanding thus demonstrating that this development already exists, albeit intuitively, as an element of “lean thinking”
Systematic review of SGLT2 receptor inhibitors in dual or triple therapy in type 2 diabetes
Background Despite the number of medications for type 2 diabetes, many people with the condition do not achieve good glycaemic control. Some existing glucose-lowering agents have adverse effects such as weight gain or hypoglycaemia. Type 2 diabetes tends to be a progressive disease, and most patients require treatment with combinations of glucose-lowering agents. The sodium glucose co-transporter 2 (SGLT2) receptor inhibitors are a new class of glucose-lowering agents.
Objective To assess the clinical effectiveness and safety of the SGLT2 receptor inhibitors in dual or triple therapy in type 2 diabetes.
Data sources MEDLINE, Embase, Cochrane Library (all sections); Science Citation Index; trial registries; conference abstracts; drug regulatory authorities; bibliographies of retrieved papers.
Inclusion criteria Randomised controlled trials of SGLT2 receptor inhibitors compared with placebo or active comparator in type 2 diabetes in dual or combination therapy.
Methods Systematic review. Quality assessment used the Cochrane risk of bias score.
Results Seven trials, published in full, assessed dapagliflozin and one assessed canagliflozin. Trial quality appeared good. Dapagliflozin 10 mg reduced HbA1c by −0.54% (weighted mean differences (WMD), 95% CI −0.67 to −0.40) compared to placebo, but there was no difference compared to glipizide. Canagliflozin reduced HbA1c slightly more than sitagliptin (up to −0.21% vs sitagliptin). Both dapagliflozin and canagliflozin led to weight loss (dapagliflozin WMD −1.81 kg (95% CI −2.04 to −1.57), canagliflozin up to −2.3 kg compared to placebo).
Limitations Long-term trial extensions suggested that effects were maintained over time. Data on canagliflozin are currently available from only one paper. Costs of the drugs are not known so cost-effectiveness cannot be assessed. More data on safety are needed, with the Food and Drug Administration having concerns about breast and bladder cancers.
Conclusions Dapagliflozin appears effective in reducing HbA1c and weight in type 2 diabetes, although more safety data are needed

The Evolution of X-ray Bursts in the "Bursting Pulsar" GRO J1744-28
GRO J1744-28, commonly known as the `Bursting Pulsar', is a low mass X-ray
binary containing a neutron star and an evolved giant star. This system,
together with the Rapid Burster (MXB 1730-33), are the only two systems that
display the so-called Type II X-ray bursts. These type of bursts, which last
for 10s of seconds, are thought to be caused by viscous instabilities in the
disk; however the Type II bursts seen in GRO J1744-28 are qualitatively very
different from those seen in the archetypal Type II bursting source the Rapid
Burster. To understand these differences and to create a framework for future
study, we perform a study of all X-ray observations of all 3 known outbursts of
the Bursting Pulsar which contained Type II bursts, including a population
study of all Type II X-ray bursts seen by RXTE. We find that the bursts from
this source are best described in four distinct phenomena or `classes' and that
the characteristics of the bursts evolve in a predictable way. We compare our
results with what is known for the Rapid Burster and put out results in the

Effect of Total Quality Management on Industrial Performance in Nigeria: An Empirical Investigation.
The study examined effect of Total Quality Management on Industrial Performance in Nigeria. Longitudinal design was employed which spanned the period of 1996 to 2008. Secondary data set of quality control cost, expenditure of salary on labour and the value of industrial output that served as surrogates of Total Quality Management and Industrial Performance respectively were collected from Pal Brewery in Anambra State. The data were analysed using Ordinary Least Square (OSL) method of multiple regression to determine the effect of the predictor variables on the dependent variable. The study found that cost of quality control had non significant positive effect on industrial performance while expenditure of salary on labour had a non significant negative effect on industrial performance. It was concluded that Total Quality Management substantially enhanced industrial performance in the Brewery subsector of the Nigerian economy. The study recommended that firms should initiate and implement company-wide and customer-centred Total Quality Management programmes to attain and sustain customer satisfaction and high performance. A huntsman’s bite: envenoming from the Malaysian ‘Black & Gold’, Thelcticopis sp. spider (Arachnida: Araneae: Sparassidae: Sparianthinae)
Most spider species in Malaysia are considered harmless and spider bites are rarely reported. This is partly due to the mild effects from the bite and victims do not seek medical treatment in the hospital. To date, there are few well-documented cases of medically significant bites from indigenous spider species in Malaysia. Here, we report the presentation, clinical management and outcome of two patients following a bite by the yet to be described ‘Black & Gold’ Huntsman spider from the genus Thelcticopis in Malaysia. The first case involved a 42-year-old man who was bitten on the tip of the distal phalanx of his right middle finger and presented with severe pain and local swelling. He was treated symptomatically and was discharged well. The second case involved a 57-year-old woman, who was bitten on the proximal phalanx of her right little finger and presented with severe throbbing pain and progressive swelling. She was treated symptomatically and was discharged after 24 hours observation in the medical ward. However, she noted moderate neck and generalized joints pains especially affecting the hips and knees, one day prior to discharge. She was treated with oral analgesia and symptoms resolved within two days

Calibration strategies for use of the nanoDot OSLD in CT applications.
Aluminum oxide based optically stimulated luminescent dosimeters (OSLD) have been recognized as a useful dosimeter for measuring CT dose, particularly for patient dose measurements. Despite the increasing use of this dosimeter, appropriate dosimeter calibration techniques have not been established in the literature; while the manufacturer offers a calibration procedure, it is known to have relatively large uncertainties. The purpose of this work was to evaluate two clinical approaches for calibrating these dosimeters for CT applications, and to determine the uncertainty associated with measurements using these techniques. Three unique calibration procedures were used to calculate dose for a range of CT conditions using a commercially available OSLD and reader. The three calibration procedures included calibration (a) using the vendor-provided method, (b) relative to a 120 kVp CT spectrum in air, and (c) relative to a megavoltage beam (implemented with 60 Co). The dose measured using each of these approaches was compared to dose measured using a calibrated farmer-type ion chamber. Finally, the uncertainty in the dose measured using each approach was determined. For the CT and megavoltage calibration methods, the dose measured using the OSLD nanoDot was within 5% of the dose measured using an ion chamber for a wide range of different CT scan parameters (80-140 kVp, and with measurements at a range of positions). When calibrated using the vendor-recommended protocol, the OSLD measured doses were on average 15.5% lower than ion chamber doses. Two clinical calibration techniques have been evaluated and are presented in this work as alternatives to the vendor-provided calibration approach. These techniques provide high precision for OSLD-based measurements in a CT environment
Natural History of the Peregrine Falcon in the Keewatin District of the Northwest Territories
A dense, productive population of peregrine falcons (Falco peregrinus) was studied for five years on a 450 sq km study area located along the northwest coast of Hudson Bay. The mean internest distance of 3.3 km represents the highest nesting density on record for the species in the Arctic. Morphometric and plumage characteristics of adults in the population suggest they are intermediate between F.p. tundrius from farther north and F.p. anatum from boreal regions to the south and west. The migratory pathway used by this population of birds is similar to that used by peregrines from Alaska, northern Canada, and Greenland. Both timing of arrival on the breeding grounds and clutch initiation are influenced strongly by spring weather patterns. Nineteen species of birds and three species of mammals were used as prey; however, in most years the bulk of the diet consisted of six species of birds and one mammal. A dramatic increase in the density of territorial peregrines in a year of high microtine rodent abundance, coupled with changes in various measures of reproductive performance, suggested that use of lemmings by falcons can be significant in some years. Suggestions that peregrines lay fewer eggs in the arctic part of their range were not supported. Broods hatched asynchronously, with the last-hatched young dying in about one-half of all broods of four. Surviving last-hatched young in broods of four grew at rates similar to older nest mates. The Jefferson Lab Frozen Spin Target
A frozen spin polarized target, constructed at Jefferson Lab for use inside a
large acceptance spectrometer, is described. The target has been utilized for
photoproduction measurements with polarized tagged photons of both longitudinal
and circular polarization. Protons in TEMPO-doped butanol were dynamically
polarized to approximately 90% outside the spectrometer at 5 T and 200--300 mK.
Photoproduction data were acquired with the target inside the spectrometer at a
frozen-spin temperature of approximately 30 mK with the polarization maintained
by a thin, superconducting coil installed inside the target cryostat. A 0.56 T
solenoid was used for longitudinal target polarization and a 0.50 T dipole for
transverse polarization. Spin-lattice relaxation times as high as 4000 hours
were observed. We also report polarization results for deuterated propanediol
