    A Profile and Analysis of Repeat Bankruptcy Petitioners in the District of Utah 1984-2004

    The purpose of this study was to describe the incidence of repeat filers in Utah and estimate the extent that repeat filers may be abusing the bankruptcy system. This study sought to develop a profile of repeat filers . Demographic and financial variables were examined to determine their association with abuser/nonabuser status. In this study, abuse of bankruptcy was characterized only by the timing and number of filings over 20 years. Debtors with three filings in a 2-year period or less and debtors with four or more total filings were classified as abusers. Nonabusers were defined as debtors who filed only once as well as debtors who had two or three scattered filings over the 20-year period. About I I% of the total sample appeared to be abusing the bankruptcy system by filing repeatedly. The majority (76.2%) of the I 997 cases filed by abusers were dismissed while only 23.8% received a discharge of their unsecured debts. Only five (2.9%) of the I 71 abusers who filed chapter I 3 in I 997 completed their payment plan and received a discharge of their debts. The logistic regression model found chapter (7 versus 13), filing status, unsecured debt, and monthly income to be the most significant variables in estimating abuse. Males and females filing alone were nearly 50% less likely than joint filers to be abusers. Chapter 13 debtors are nearly five times as likely to be abusers when compared to chapter 7 debtors. Filers who had unsecured debt levels above the median were less likely to be abusers, and filers who had incomes above the median were almost twice as likely to be abusers. Realistic repayment plans that pay careful attention to construction of budgets and a financial counselor to work with debtors who miss payments is one approach to combating abuse by repeat filing. Judges may need to discipline attorneys who file cases repeatedly. Perhaps a new Code is not what we need to combat abusers; instead, closer monitoring of cases by trustees, more responsible attorneys, and more responsible lending are needed

    Marine litter education boosts children’s understanding and self-reported actions

    Marine litter is a significant environmental problem inherently linked to individuals’ purchasing, use and disposal behaviour. This research examined 176 British schoolchildren’s (aged 8–13 years) baseline marine litter understanding and self-reported actions, and tested the impact of an educational intervention. All children participated in the educational intervention and completed a pre- and post-intervention questionnaire. At baseline, children were quite concerned about marine litter and recognised some of the causes and impacts of the problem. Children also reported taking a number of actions to help solve the problem. After the intervention, children were significantly more concerned, had a better understanding of the causes and negative impacts, and reported engaging in more actions to reduce the potential causes of marine litter. Understanding the perceptions and behaviours of children is crucial as they represent current and future actors and a potentially important source of social influence among their peers, parents and community

    The c- Jun N-terminal kinase JNK participates in cytokine- and isolation stress-induced rat pancreatic islet apoptosis

    Aims/hypothesis: The protocols used for the preparation of human pancreatic islets immediately induce a sustained and massive activation of the c-Jun-N-terminal kinase (JNK). JNK, which participates in apoptosis of insulin-secreting cells, is activated by mechanical stresses, as well as by exposure to pro-inflammatory cytokines. Here, we investigated whether the delivery of a protease-resistant JNK inhibitory peptide (D-JNKI) through a protein transduction system during pancreatic digestion might impair JNK signalling throughout the transplantation procedure. Methods: Rat pancreases were treated with D-JNKI through the pancreatic duct and cells then isolated by enzymatic digestion. Protein extracts were prepared to determine JNK activity by kinase assays and total RNA was extracted to measure gene expressions by a Light-Cycler technique. Cell apoptosis rate was determined by terminal deoxynucleotidyl transferase dUTP nick end labelling (TUNEL) assay and by scoring cells displaying pycnotic nuclei. Results: Our data establish that the peptide transduction system used here efficiently transfects islets, allowing for stable in vivo (up to 2days) transfection of human islets transplanted under the kidney capsule. Further, D-JNKI decreases intracellular JNK signalling during isolation and following cytokine exposure in both human and rat islets, as measured by kinase assays and reduced c-fos expression; D-JNKI also confers protection against apoptosis induced during the rat islet preparation and subsequent to IL-1β exposure. Conclusions/interpretation: JNK signalling participates in islet isolation- and IL-1β-induced apoptosis in rat islets. Furthermore, the system we used might be more generally applicable for the persistent blockage (several days) of pro-apoptotic pathways in the transplanted islets; this days-long protection might potentially be an absolute prerequisite to help transplanted islets better survive the first wave of the non-specific inflammatory attac

    Proyek konstruksi sangat rentan terhadap kecelakaan kerja. Keselamatan dan kesehatan kerja bertujuan untuk menciptakan kondisi yang mendukung Kenyamanan kerja bagi tenaga kerja, sehingga dapat memperkecil angka kecelakaan dan penyakit kerja pada suatu proyek konstruksi. Pemerintah telah mengeluarkan undang-undang dan berbagai peraturan menyangkut keselamatan dan kesehatan kerja (K3). Tetapi semua USAha pemerintah tidak akan berhasil tanpa adanya respon dari Perusahaan-Perusahaan yang bergerak dibidang konstruksi. Wawancara serta observasi telah dilakukan terhadap pekerja, pimpinan proyek dan safety officer pada proyek the Lagoon Tamansari untuk meneliti apakah dalam pelaksanaan proyek tersebut sudah menerapkan Sistem Manajemen Keselamatan dan Kesehatan Kerja (K3). Dasar penerapan SMK3 disesuaikan dengan standar Internasional yaitu OHSAS 18001:2007. Hasil penelitian menunjukkan bahwa penerapan aspek hukum terhadap K3 pada proyek ini sudah baik karena setiap elemen K3 telah dilaksanakan sesuai dengan peraturan Perundangan-undangan yang dikeluarkan oleh pemerintah maupun peraturan dan sangsi yang dikeluarkan oleh Perusahaan, dalam hal ini PT.Wika Gedung. Hal ini berpengaruh baik kepada tenaga kerja maupun Perusahaan karena dapat menghindari kecelakaan dan penyakit kerja serta pelanggaran kerja dalam proyek the Lagoon Tamansari. Dengan adanya peraturan Perundangan undangan tersebut maka telah lengkap landasan hukum untuk melaksanakan Keselamatan dan Kesehatan Kerja (K3). Untuk mengetahui penerapan aspek hukum dan juga manajemen K3, PT. Wika Gedung mengadakan Inspeksi K3, Audit K3 dan Tindakan Perbaikan dan Pencegahan. Hal ini dilakukan agar Perusahaan dapat mengidentifikasi kondisi yang beresiko agar bisa dilakukan tindakan perbaikan dan juga mengevaluasi pelaksanaan manajemen K3 apakah sudah berjalan baik atau tidak. Sehingga dapat dikatakan PT. Wika Gedung sudah berhasil menerapkan Peraturan Perundangan undangan K3

    Routine Screening of Adolescents for Trichomonas vaginalis in a Juvenile Detention Center

    Background: Detained and incarcerated adolescents are at increased risk of sexually transmitted infections (STI), but limited information is currently available regarding the prevalence of Trichomonas vaginalis (TV) in detained youth.Methods: A total of 144 detainees (75 males and 69 females) in the Franklin County Juvenile Detention Facility (FCJDF) in Columbus, Ohio, consented to STI screening between May 2016 and June 2017. Participants were screened for TV in addition to Neisseria gonorrhoeae (NG) and Chlamydia trachomatis (CT) using urine nucleic acid amplification tests (NAAT).Results: Among detained youth who consented to testing, TV was identified in 7 of 69 (10.1%) females and 0 of 75 (0%) males (P <0.01). Neisseria gonorrhoeae was identified in 12 of 69 (17.4%) females and 3 of 75 (4.0%) males (P = 0.01), and CT was identified in 16 of 69 (23.2%) female and 7 of 75 (9.3%) male detainees (P = 0.04). Among females, positive TV screen was associated with prior history of STI.Conclusion: Our data support routine screening of female detainees at FCJDF, based on our finding of 10% posi-tivity among females who underwent testing

    JNK3 is abundant in insulin-secreting cells and protects against cytokine-induced apoptosis

    Aims/hypothesis: In insulin-secreting cells, activation of the c-Jun NH2-terminal kinase (JNK) pathway triggers apoptosis. Whereas JNK1 and JNK2 are ubiquitously produced, JNK3 has been described exclusively in neurons. This report aims to characterise the expression and role in apoptosis of the three JNK isoforms in insulin-secreting cells exposed to cytokines. Methods: Sections of human and mouse pancreases were used for immunohistochemistry studies with isoform-specific anti-JNK antibodies. Human, pig, mouse and rat pancreatic islets were isolated by enzymatic digestion and RNA or protein extracts were prepared. RNA and protein levels were determined by quantitative RT-PCR and western blotting respectively, using JNK-isoform-specific primers and isoform-specific antibodies; activities of the three JNK isoforms were determined by kinase assays following quantitative immunoprecipitation/depletion of JNK3. JNK silencing was performed with small interfering RNAs and apoptotic rates were determined in INS-1E cells by scoring cells displaying pycnotic nuclei. Results: JNK3 and JNK2 mRNAs are the predominant isoforms expressed in human pancreatic islets. JNK3 is nuclear while JNK2 is also cytoplasmic. In INS-1E cells, JNK3 knockdown increases c-Jun levels and caspase-3 cleavage and sensitises cells to cytokine-induced apoptosis; in contrast, JNK1 or JNK2 knockdown is protective. Conclusions/interpretation: In insulin-secreting cells, JNK3 plays an active role in preserving pancreatic beta cell mass from cytokine attacks. The specific localisation of JNK3 in the nucleus, its recruitment by cytokines, and its effects on key transcription factors such as c-Jun, indicate that JNK3 is certainly an important player in the transcriptional control of genes expressed in insulin-secreting cell

    Furosemide stimulation of parathormone in humans: role of the calcium-sensing receptor and the renin-angiotensin system.

    Interactions between sodium and calcium regulating systems are poorly characterized but clinically important. Parathyroid hormone (PTH) levels are increased shortly after furosemide treatment by an unknown mechanism, and this effect is blunted by the previous administration of a calcimimetic in animal studies. Here, we explored further the possible underlying mechanisms of this observation in a randomized crossover placebo-controlled study performed in 18 human males. Volunteers took either cinacalcet (60 mg) or placebo and received a 20 mg furosemide injection 3 h later. Plasma samples were collected at 15-min intervals and analyzed for intact PTH, calcium, sodium, potassium, magnesium, phosphate, plasma renin activity (PRA), and aldosterone up to 6 h after furosemide injection. Urinary electrolyte excretion was also monitored. Subjects under placebo presented a sharp increase in PTH levels after furosemide injection. In the presence of cinacalcet, PTH levels were suppressed and marginal increase of PTH was observed. No significant changes in electrolytes and urinary excretion were identified that could explain the furosemide-induced increase in PTH levels. PRA and aldosterone were stimulated by furosemide injection but were not affected by previous cinacalcet ingestion. Expression of NKCC1, but not NKCC2, was found in parathyroid tissue. In conclusion, our results indicate that furosemide acutely stimulates PTH secretion in the absence of any detectable electrolyte changes in healthy adults. A possible direct effect of furosemide on parathyroid gland needs further studies

    SLC2A9 (GLUT9) mediates urate reabsorption in the mouse kidney.

    Uric acid (UA) is a metabolite of purine degradation and is involved in gout flairs and kidney stones formation. GLUT9 (SLC2A9) was previously shown to be a urate transporter in vitro. In vivo, humans carrying GLUT9 loss-of-function mutations have familial renal hypouricemia type 2, a condition characterized by hypouricemia, UA renal wasting associated with kidney stones, and an increased propensity to acute renal failure during strenuous exercise. Mice carrying a deletion of GLUT9 in the whole body are hyperuricemic and display a severe nephropathy due to intratubular uric acid precipitation. However, the precise role of GLUT9 in the kidney remains poorly characterized. We developed a mouse model in which GLUT9 was deleted specifically along the whole nephron in a tetracycline-inducible manner (subsequently called kidney-inducible KO or kiKO). The urate/creatinine ratio was increased as early as 4 days after induction of the KO and no GLUT9 protein was visible on kidney extracts. kiKO mice are morphologically identical to their wild-type littermates and had no spontaneous kidney stones. Twenty-four-hour urine collection revealed a major increase of urate urinary excretion rate and of the fractional excretion of urate, with no difference in urate concentration in the plasma. Polyuria was observed, but kiKO mice were still able to concentrate urine after water restriction. KiKO mice displayed lower blood pressure accompanied by an increased heart rate. Overall, these results indicate that GLUT9 is a crucial player in renal handling of urate in vivo and a putative target for uricosuric drugs
