Debugging Woven Code

The ability to debug woven programs is critical to the adoption of Aspect Oriented Programming (AOP). Nevertheless, many AOP systems lack adequate support for debugging, making it difficult to diagnose faults and understand the program's structure and control flow. We discuss why debugging aspect behavior is hard and how harvesting results from related research on debugging optimized code can make the problem more tractable. We also specify general debugging criteria that we feel all AOP systems should support. We present a novel solution to the problem of debugging aspect-enabled programs. Our Wicca system is the first dynamic AOP system to support full source-level debugging of woven code. It introduces a new weaving strategy that combines source weaving with online byte-code patching. Changes to the aspect rules, or base or aspect source code are rewoven and recompiled on-the-fly. We present the results of an experiment that show how these features provide the programmer with a powerful interactive debugging experience with relatively little overhead

Nighttime resident supervision and education: results of a national survey of internal medicine residency program directors

Over the past several years, the Accreditation Council for Graduate Medical Education (ACGME) has issued new restrictions on resident duty hours while calling for increased supervision to ensure patient safety. To meet these requirements, some hospitals have hired overnight in-house hospitalist physicians, also called nocturnists, while others have continued a traditional model wherein a resident in-house can access a supervisor at home by phone as needed. Increased myocardial blood flow during acute exposure to simulated altitudes

Background: Although only poor data exist on changes in myocardial blood flow (MBF) under acute hypoxia, patients with known coronary artery disease are advised not to exceed a moderate altitude exposure of about 2000 m above sea level. Methods and Results: We measured MBF with positron emission tomography using O-15-labeled water in 8 healthy human volunteers (aged 26 ± 3 years [mean ± SD]) at baseline (450 m above sea level, Zurich, Switzerland) and during acute hypoxic hypoxemia induced by inhalation of 2 hypoxic gas mixtures corresponding to altitudes of 2000 and 4500 m. MBF remained unchanged at 2000 m (increase of 10%, not significant) but increased significantly at 4500 m (62%, P <.001), exceeding the relative increase in rate pressure product. Conclusions: Our results may explain why exposure to an altitude of 2000 m (corresponding to the cabin pressure in most airplanes during flight) is clinically well tolerated, even by patients with reduced coronary flow reserve, such as those with coronary artery disease. However, at an altitude of 4500 m, MBF increases significantly, supporting the recommendation that patients with impaired flow reserve avoid exposure to higher altitude

Survey of overnight academic hospitalist supervision of trainees

In 2003, Accreditation Council for Graduate Medical Education (ACGME) announced the first in a series of guidelines related to the residency training. The most recent recommendations include explicit recommendations regarding the provision of on‐site clinical supervision for trainees of internal medicine. To meet these standards, many internal medicine residency programs turned to hospitalist programs to fill that need. However, much is unknown about the current relationships between hospitalist and residency programs, specifically with regard to supervisory roles and supervision policies. We aimed to describe how academic hospitalists currently supervise housestaff during the on‐call, or overnight, period and hospitalist program leader their perceptions of how these new policies would impact trainee‐hospitalist interactions. Exploring Glucose Dysregulation in Migraine: Insights from Continuous Glucose Monitoring

Despite being associated with hypoglycemia for nearly a century, a definitive relationship between migraines and glucose dysregulation remains elusive. Accumulating evidence suggests that migraines are in part due to a metabolic mismatch between cerebral demand and available energy. Research analyzing plasma glucose levels and migraine activity may further elucidate this interface between metabolic dysregulation and migraine pathophysiology and may potentially open avenues for therapeutic interventions targeting holistic metabolism for migraine management

Development of a Protocol for Obtaining Biological Samples for Genetic Testing from Remote Individuals

Pharmacogenomic sequencing allows individuals to learn more about how they will respond to certain medications but requires shipping of a biological sample. One complication of sending biological samples to remote laboratories is stability. Blood generally yields sufficient quantities of high-quality DNA but requires a clinic visit. Saliva and buccal swabs are routinely used for DNA extractions, but the DNA quality is notoriously low due to the presence of bacteria in the mouth. Additionally, elderly individuals have difficulty producing enough saliva for testing, and the tubes contain several milliliters of liquid and shipping requires special considerations. Dried blood spot cards, which serve as an alternative to saliva and buccal swabs, yield high-quality DNA and ship easily, but may produce a lower yield. This project aims to determine which biological sample methods can reasonably be obtained from remote individuals
