    Carers\u27 preferences for the delivery of therapy services for people with disability in rural Australia: Evidence from a discrete choice experiment

    Background: The implementation of the Australian National Disability Insurance Scheme (NDIS) is expected to generate a responsive, person-centred system that will empower people with disability to choose the services and support they receive. However little attention has been paid to examine how users of the NDIS will choose and spend their individual budgets. This study aimed to determine quantitatively the relative importance that carers of people with a disability living in rural Australia place on different therapy service delivery characteristics. Methods: A stated preference discrete choice experiment (DCE) was incorporated into a survey of carers of people with disability living in rural Australia. Carers chose between therapy delivery services differing in attributes such as travel time to receive therapy, sector providing the service (i.e. Government, not-for-profit, private), out-of-pocket costs, person who delivers the therapy (therapist or other staff) and waiting time. Results: A total of 133 carers completed the DCE. The majority of respondents cared for a child with a disability (84%), the average age of the person they cared for was 17 years (SD 14.25). Participants expressed strong preferences for a short waiting time (0-3 months) to receive therapy services; services delivered by a therapist, no out of pocket cost and travelling up to 4 hours to receive a therapy session (p Conclusion: Carers of people with a disability in rural Australia exhibited strongest preferences for short waiting times (0-3 months). Therapy services that are delivered by therapy assistants or support workers will require careful introduction to achieve uptake and acceptability

    Factors affecting retention of allied health professionals working with people with disability in rural New South Wales, Australia: discrete choice experiment questionnaire development

    Objective: This paper describes the development of a discrete choice experiment (DCE) questionnaire to identify the factors (attributes) that allied health professionals (AHPs) working with people with disability identify as important to encouraging them to remain practising in rural areas. Methods: Focus groups and semi-structured interviews were conducted with 97 purposively selected service providers working with people with disability in rural New South Wales, Australia. Focus groups and interviews were digitally recorded, transcribed, and analysed using a modified grounded theory approach involving thematic analysis and constant comparison. Results: Six attributes that may influence AHPs working with people with disability in rural areas to continue to do so were inductively identified: travel arrangements, work flexibility, professional support, professional development, remuneration, and autonomy of practice. The qualitative research information was combined with a policy review to define these retention factors and ensure that they are amenable to policy changes. Conclusion: The use of various qualitative research methods allowed the development of a policy-relevant DCE questionnaire that was grounded in the experience of the target population (AHPs)

    Should I stay or should I go? Exploring the job preferences of allied health professionals working with people with disability in rural Australia

    Introduction: The uneven distribution of allied health professionals (AHPs) in rural and remote Australia and other countries is well documented. In Australia, like elsewhere, service delivery to rural and remote communities is complicated because relatively small numbers of clients are dispersed over large geographic areas. This uneven distribution of AHPs impacts significantly on the provision of services particularly in areas of special need such as mental health, aged care and disability services. Objective: This study aimed to determine the relative importance that AHPs (physiotherapists, occupational therapists, speech pathologists and psychologists – “therapists”) living in a rural area of Australia and working with people with disability, place on different job characteristics and how these may affect their retention. Methods: A cross-sectional survey was conducted using an online questionnaire distributed to AHPs working with people with disability in a rural area of Australia over a 3-month period. Information was sought about various aspects of the AHPs’ current job, and their workforce preferences were explored using a best–worst scaling discrete choice experiment (BWSDCE). Conditional logistic and latent class regression models were used to determine AHPs’ relative preferences for six different job attributes. Results: One hundred ninety-nine AHPs completed the survey; response rate was 51 %. Of those, 165 completed the BWSDCE task. For this group of AHPs, “high autonomy of practice” is the most valued attribute level, followed by “travel BWSDCE arrangements: one or less nights away per month”, “travel arrangements: two or three nights away per month” and “adequate access to professional development”. On the other hand, the least valued attribute levels were “travel arrangements: four or more nights per month”, “limited autonomy of practice” and “minimal access to professional development”. Except for “some job flexibility”, all other attributes had a statistical influence on AHPs’ job preference. Preferences differed according to age, marital status and having dependent children. Conclusions: This study allowed the identification of factors that contribute to AHPs’ employment decisions about staying and working in a rural area. This information can improve job designs in rural areas to increase retention

    Case-Analysis for Rippling and Inductive Proof

    Rippling is a heuristic used to guide rewriting and is typically used for inductive theorem proving. We introduce a method to support case-analysis within rippling. Like earlier work, this allows goals containing if-statements to be proved automatically. The new contribution is that our method also supports case-analysis on datatypes. By locating the case-analysis as a step within rippling we also maintain the termination. The work has been implemented in IsaPlanner and used to extend the existing inductive proof method. We evaluate this extended prover on a large set of examples from Isabelle’s theory library and from the inductive theorem proving literature. We find that this leads to a significant improvement in the coverage of inductive theorem proving. The main limitations of the extended prover are identified, highlight the need for advances in the treatment of assumptions during rippling and when conjecturing lemmas

    The development of reading tests for use in a regularly spelled language.

    Data are presented on the development of tests of reading skill in rural Tanzanian primary school pupils. Instruction in these schools is in Kiswahili, a regularly spelt language. Using a translation of a standard reading test, children could read aloud all words once they had learnt sound-letter correspondences, regardless of comprehension. In addition, children can appear to pass traditional comprehension tasks by decoding only some of the words. Three graded tests were developed which allow testing of children who either have only some letter knowledge, can read single words, or are proficient readers. The tests require children to both decode and understand the reading material in order to achieve high scores. The tests correlated well with scores on other educational achievement tests, and showed age and school grade differences. It is suggested that these tests are useful measures of reading development in a regularly spelt language. Adaptation to English and validation against standardised instruments is planned

    Low Levels of after School-Hours Social Interaction and Physical Activity of 5-7 Year Olds

    As part of a cluster randomized controlled trial (Bundy et al., 2017), the after school-hours activities of 5-7 year old children were recorded by parents and other adult carers on four consecutive weekdays between 3:30pm-7:00pm. Records of time use showed most time was spent indoors in activities involving low levels of physical activity. The most-frequently-recorded activity was screen time, accounting for approximately one quarter of all activities. Higher levels of physical activity were reported when children were outdoors (19.5% of time) and/or with peers (9.58%). If an adult was present, highest activity levels of children occurred when the child was with a teacher/carer (6.07%). Although concern is frequently raised about children spending too much time in organized activities, these structured forms of out-of-school choices accounted for only 8.09% of recordings. The results of the current study strengthen the evidence base, supporting the need to optimise out-ofschool times with more developmentally important social and physical contexts. We discuss ways in which context may support or inhibit opportunities physical activity in after school-hours activities

    AEGIS: Extinction and Star Formation Tracers from Line Emission

    Strong nebular emission lines are a sensitive probe of star formation and extinction in galaxies, and the [O II] line detects star forming populations out to z>1. However, star formation rates from emission lines depend on calibration of extinction and the [O II]/H-alpha line ratio, and separating star formation from AGN emission. We use calibrated line luminosities from the DEEP2 survey and Palomar K magnitudes to show that the behavior of emission line ratios depends on galaxy magnitude and color. For galaxies on the blue side of the color bimodality, the vast majority show emission signatures of star formation, and there are strong correlations of extinction and [O II]/H-alpha with restframe H magnitude. The conversion of [O II] to extinction-corrected H-alpha and thus to star formation rate has a significant slope with M_H, 0.23 dex/mag. Red galaxies with emission lines have a much higher scatter in their line ratios, and more than half show AGN signatures. We use 24 micron fluxes from Spitzer/MIPS to demonstrate the differing populations probed by nebular emission and by mid-IR luminosity. Although extinction is correlated with luminosity, 98% of IR-luminous galaxies at z~1 are still detected in the [O II] line. Mid-IR detected galaxies are mostly bright and intermediate color, while fainter, bluer galaxies with high [O II] luminosity are rarely detected at 24 microns.Comment: 4 pages, 3 figures.     On the Evolution of the Velocity-Mass-Size Relations of Disk-Dominated Galaxies over the Past 10 Billion Years

    We study the evolution of the scaling relations between maximum circular velocity, stellar mass and optical half-light radius of star-forming disk-dominated galaxies in the context of LCDM-based galaxy formation models. Using data from the literature combined with new data from the DEEP2 and AEGIS surveys we show that there is a consistent observational and theoretical picture for the evolution of these scaling relations from z\sim 2 to z=0. The evolution of the observed stellar scaling relations is weaker than that of the virial scaling relations of dark matter haloes, which can be reproduced, both qualitatively and quantitatively, with a simple, cosmologically-motivated model for disk evolution inside growing NFW dark matter haloes. In this model optical half-light radii are smaller, both at fixed stellar mass and maximum circular velocity, at higher redshifts. This model also predicts that the scaling relations between baryonic quantities evolve even more weakly than the corresponding stellar relations. We emphasize, though, that this weak evolution does not imply that individual galaxies evolve weakly. On the contrary, individual galaxies grow strongly in mass, size and velocity, but in such a way that they move largely along the scaling relations. Finally, recent observations have claimed surprisingly large sizes for a number of star-forming disk galaxies at z \sim 2, which has caused some authors to suggest that high redshift disk galaxies have abnormally high spin parameters. However, we argue that the disk scale lengths in question have been systematically overestimated by a factor \sim 2, and that there is an offset of a factor \sim 1.4 between H\alpha sizes and optical sizes.     Extremely Red Objects in the Field of QSO 1213-0017: A Galaxy Concentration at z=1.31

    We have discovered a concentration of extremely red objects (EROs; R-K>6) in the field of the z=2.69 quasar QSO 1213-0017 (UM 485), which is significantly overabundant compared to the field ERO surface density. The optical/near-IR colors of the EROs and numerous other red galaxies in this field are consistent with elliptical galaxies at z=1-2. HST optical images for a subset of galaxies show regular morphologies, most of them being disky or diffuse and without any obvious evidence for interactions. Ground-based IR images show similar morphologies, indicating any dust reddening in these objects is spatially uniform. Optical spectroscopy with the W. M. Keck Telescope has found that four of the red galaxies lie at =1.31, and a fifth lies in the foreground at z=1.20. Of the =1.31 galaxies, one is a reddened AGN while the remaining three have rest-frame UV absorption-line spectra characteristic of old (few Gyr) stellar populations, similar to the old red galaxy LBDS 53W091 at z=1.55. Including the MgII absorber seen in the QSO spectrum, we find five galaxies at =1.31 spread over 1.5 h_50^{-1} Mpc on the sky. These results suggest we have discovered a coherent structure of old galaxies at high-redshift, possibly associated with a massive galaxy cluster.Comment: 37 pages including 11 Postscript figures.     KwaZulu-Natal coastal erosion events of 2006/2007 and 2011: A predictive tool?

    Severe coastal erosion occurred along the KwaZulu-Natal coastline between mid-May and November 2011. Analysis of this erosion event and comparison with previous coastal erosion events in 2006/2007 offered the opportunity to extend the understanding of the time and place of coastal erosion strikes. The swells that drove the erosion hotspots of the 2011 erosion season were relatively low (significant wave heights were between 2 m and 4.5 m) but of long duration. Although swell height was important, swell-propagation direction and particularly swell duration played a dominant role in driving the 2011 erosion event. Two erosion hotspot types were noted: sandy beaches underlain by shallow bedrock and thick sandy beaches. The former are triggered by high swells (as in March 2007) and austral winter erosion events (such as in 2006, 2007 and 2011). The latter become evident later in the austral winter erosion cycle. Both types were associated with subtidal shore-normal channels seaward of megacusps, themselves linked to megarip current heads. This 2011 coastal erosion event occurred during a year in which the lunar perigee sub-harmonic cycle (a ±4.4-year cycle) peaked, a pattern which appears to have recurred on the KwaZulu-Natal coast. If this pattern proves true, severe coastal erosion may be expected in 2015. Evidence indicates that coastal erosion is driven by the lunar nodal cycle peak but that adjacent lunar perigee sub-harmonic peaks can also cause severe coastal erosion. Knowing where and when coastal erosion may occur is vital for coastal managers and planners
