    The Undergraduate preparation of dentists: Confidence levels of final year dental students at the School of Dentistry in Cardiff

    Objective To investigate the self-reported confidence and preparedness of final year undergraduate students in undertaking a range of clinical procedures. Methods A questionnaire was distributed to final year dental students at Cardiff University, six months prior to graduation. Respondents rated their confidence in undertaking 39 clinical procedures using a 5-point scale (1 = can undertake on own with confidence, 5 = unable to undertake). Students also responded yes/no to experiencing four difficulties and to three statements about general preparedness. Results 71% (N = 51) responded of which 55% (N = 28) were female. Over half reported being 'anxious that the supervisor was not helping enough' (57%) and 'relying heavily on supervisor for help' (53%). Eighty percent 'felt unprepared for the clinical work presented' and gender differences were most notable here (male: 65% N = 33; females: 93% N = 47). Mean confidence scores were calculated for each clinical procedure (1 = lowest; 5 = highest). Confidence was highest in performing 'simple scale' and 'fissure sealant' (mean-score = 5). Lowest scores were reported for 'surgical extractions involving a flap (mean-score = 2.28)', 'simple surgical procedures' (mean-score = 2.58) and the 'design/fit/adjustment of orthodontic appliances' (mean-score = 2.88). Conclusions As expected complex procedures that were least practised scored the lowest in overall mean confidence. Gender differences were noted in self-reported confidence for carrying out treatment unsupervised and feeling unprepared for clinical work

    Triton's surface age and impactor population revisited in light of Kuiper Belt fluxes: Evidence for small Kuiper Belt objects and recent geological activity

    Neptune's largest satellite, Triton, is one of the most fascinating and enigmatic bodies in the solar system. Among its numerous interesting traits, Triton appears to have far fewer craters than would be expected if its surface was primordial. Here we combine the best available crater count data for Triton with improved estimates of impact rates by including the Kuiper Belt as a source of impactors. We find that the population of impactors creating the smallest observed craters on Triton must be sub-km in scale, and that this small-impactor population can be best fit by a differential power-law size index near -3. Such results provide interesting, indirect probes of the unseen small body population of the Kuiper Belt. Based on the modern, Kuiper Belt and Oort Cloud impactor flux estimates, we also recalculate estimated ages for several regions of Triton's surface imaged by Voyager 2, and find that Triton was probably active on a time scale no greater than 0.1-0.3 Gyr ago (indicating Triton was still active after some 90% to 98% of the age of the solar system), and perhaps even more recently. The time-averaged volumetric resurfacing rate on Triton implied by these results, 0.01 km3^3 yr−1^{-1} or more, is likely second only to Io and Europa in the outer solar system, and is within an order of magnitude of estimates for Venus and for the Earth's intraplate zones. This finding indicates that Triton likely remains a highly geologically active world at present, some 4.5 Gyr after its formation.     Clinical skills of a new foundation dentist: the expectations of dental foundation educational supervisors

    Aim To investigate dental foundation (DF) educational supervisors' (ES) expectations of the level of clinical ability expected of a foundation dentist (FD) on entry into foundation training. Methods Following ethical approval, a pre-piloted online questionnaire was circulated to ESs across England and Northern Ireland (n = 959), requiring respondents to rate their expectations of their FDs' ability at the beginning of the training year, in relation to undertaking 104 clinical skills. A five-point scale and descriptors were provided, ranging from 'on own with confidence' to 'unable to undertake'. Respondents were invited to comment further. Data were imported into IBM SPSS (v20) and descriptive analysis was undertaken. Expectations were assigned values and a mean, 'skill rank score' was generated for each skill. A ranking score closer to five indicated high expectation; closer to one indicated low expectation. Results A total of 510 (53%) questionnaires were completed. The study highlighted a range of expectations which were used to identify 'upper-level' (core) skills expected of a new FD. The majority of expectations were in line with UG curriculum guidance; however, certain skills were identified as having overly high expectations and these may need to be modified to align with current guidance. Conclusions Understanding the expectations ESs have of a new FD is useful for both dental schools and those involved in DF training. Findings add to the existing evidence base and should generate discussion for those within the education continuum to enhance the successful development of the FD

    Dark matter halo concentrations in the Wilkinson Microwave Anisotropy Probe year 5 cosmology

    We use a combination of three large N-body simulations to investigate the dependence of dark matter halo concentrations on halo mass and redshift in the WMAP year 5 cosmology. The median relation between concentration and mass is adequately described by a power-law for halo masses in the range 10^11 - 10^15 Msol/h and redshifts z < 2, regardless of whether the halo density profiles are fit using NFW or Einasto profiles. Compared with recent analyses of the Millennium Simulation, which uses a value of sigma_8 that is higher than allowed by WMAP5, z = 0 halo concentrations are reduced by factors ranging from 23 per cent at 10^11 Msol/h to 16 per cent at 10^14 Msol/h.     Small molecule induced reactivation of mutant p53 in cancer cells

    The p53 cancer mutant Y220C is an excellent paradigm for rescuing the function of conformationally unstable p53 mutants because it has a unique surface crevice that can be targeted by small-molecule stabilizers. Here, we have identified a compound, PK7088, which is active in vitro: PK7088 bound to the mutant with a dissociation constant of 140 μM and raised its melting temperature, and we have determined the binding mode of a close structural analogue by X-ray crystallography. We showed that PK7088 is biologically active in cancer cells carrying the Y220C mutant by a battery of tests. PK7088 increased the amount of folded mutant protein with wild-type conformation, as monitored by immunofluorescence, and restored its transcriptional functions. It induced p53-Y220C-dependent growth inhibition, cell-cycle arrest and apoptosis. Most notably, PK7088 increased the expression levels of p21 and the proapoptotic NOXA protein. PK7088 worked synergistically with Nutlin-3 on up-regulating p21 expression, whereas Nutlin-3 on its own had no effect, consistent with its mechanism of action. PK7088 also restored non-transcriptional apoptotic functions of p53 by triggering nuclear export of BAX to the mitochondria. We suggest a set of criteria for assigning activation of p53

    Exploiting transient protein states for the design of small-molecule stabilizers of mutant p53

    The destabilizing p53 cancer mutation Y220C creates an extended crevice on the surface of the protein that can be targeted by small-molecule stabilizers. Here, we identify different classes of small molecules that bind to this crevice and determine their binding modes by X-ray crystallography. These structures reveal two major conformational states of the pocket and a cryptic, transiently open hydrophobic subpocket that is modulated by Cys220. In one instance, specifically targeting this transient protein state by a pyrrole moiety resulted in a 40-fold increase in binding affinity. Molecular dynamics simulations showed that both open and closed states of this subsite were populated at comparable frequencies along the trajectories. Our data extend the framework for the design of high-affinity Y220C mutant binders for use in personalized anticancer therapy and, more generally, highlight the importance of implementing protein dynamics and hydration patterns in the drug-discovery process
