    Keeping it Real : An Evaluation Audit of Five Years of Youth-led Program Evaluation

    Youth are increasingly seen as competent in participating in research and program evaluation, two activities previously reserved for adults. This paper is a report of the findings from an evaluation audit of Stand Up! Help Out!, a participatory action after-school youth leadership development program for disadvantaged urban youth that utilized youth evaluations to develop a best practices service model. The youths’ feedback assisted providers in improving services so that youth engagement in the program was 99% (by comparison with national highs of 79%). Here, we describe an important aspect of the process of youth-led program evaluation leading to such high youth engagement: How youth interviewed each other so as to optimize the authenticity of their program evaluations and contributions to program design. Drawing from over five years of program evaluation data collected by youth, the authors report on the youths’ experiences as informants and co-researchers, consider strategies used to help youth best describe their experiences in the program, and describe implications for other settings looking to incorporate youth-led program evaluation.     Infections and risk-adjusted length of stay and hospital mortality in Polish Neonatology Intensive Care Units

    Background: The objectives of this study were to analyze the impact of infections on prolonging hospital stay with consideration of underlying risk factors and determining the mortality rates and its association with infections. Methods: An electronic database developed from a continuous prospective targeted infection surveillance program was used in the study. Data were collected from 2009 to 2012 in five Polish tertiary academic neonatal intensive care units (NICUs). The length of stay (LOS) of 2,003 very low birth weight (VLBW) neonates was calculated as the sum of the number of days since birth until death or until reaching a weight of 1,800 g. Results: The median LOS for neonates with infections was twice as high as for neonates without infection. LOS was significantly affected by the overall general condition of the neonate, as expressed by both gestational age and birth weight as well as by the Clinical Risk Index for Babies (CRIB) score; another independent factor was presence of at least one infection. Risk of in-hospital mortality was significantly increased by male sex and vaginal birth and was lower among breastfed neonates. Deaths were significantly more frequent in neonates without infection. Conclusions: The general condition of VLBW infants statistically increase both their risk of mortality and LOS; this is in contrast to the presence of infection, which significantly prolonged LOS only

    Comprehensive detection and identification of bacterial DNA in the blood of patients with sepsis and healthy volunteers using next-generation sequencing method : the observation of DNAemia

    Blood is considered to be a sterile microenvironment, in which bacteria appear only periodically. Previously used methods allowed only for the detection of either viable bacteria with low sensitivity or selected species of bacteria. The Next-Generation Sequencing method (NGS) enables the identification of all bacteria in the sample with their taxonomic classification. We used NGS for the analysis of blood samples from healthy volunteers (n = 23) and patients with sepsis (n = 62) to check whether any bacterial DNA exists in the blood of healthy people and to identify bacterial taxonomic profile in the blood of septic patients. The presence of bacterial DNA was found both in septic and healthy subjects; however, bacterial diversity was significantly different (P = 0.002) between the studied groups. Among healthy volunteers, a significant predominance of anaerobic bacteria (76.2 %), of which most were bacteria of the order Bifidobacteriales (73.0 %), was observed. In sepsis, the majority of detected taxa belonged to aerobic or microaerophilic microorganisms (75.1 %). The most striking difference was seen in the case of Actinobacteria phyla, the abundance of which was decreased in sepsis (P < 0.001) and Proteobacteria phyla which was decreased in the healthy volunteers (P < 0.001). Our research shows that bacterial DNA can be detected in the blood of healthy people and that its taxonomic composition is different from the one seen in septic patients. Detection of bacterial DNA in the blood of healthy people may suggest that bacteria continuously translocate into the blood, but not always cause sepsis; this observation can be called DNAemia.     “Charity Begins at Home”: Informal Caring Barriers to Formal Volunteering Among Older People

    Formal volunteering is an important economic and social activity. In many countries, prevalence of volunteering is decreasing overall, including among older people who constitute a major volunteering resource. This qualitative study explored reasons for non-volunteering among seniors, with a focus on those who attribute their non-volunteering to their existing helping commitments. Forty-nine Australian interviewees aged 60 + years described a range of social, psychological, and temporal factors that resulted in their prioritization of informal rather than formal volunteering activities. These factors are mapped onto a theoretical framework matrix, with social identity and social capital theories appearing to possess the most explanatory power. The findings suggest that programs designed to encourage formal volunteering among older people need to be implemented in a manner that recognizes that members of this group can hold many other responsibilities that limit their ability to participate, especially those assisting in the care of multiple generations

    Using Transformative Learning Theory to Enhance Professional Development

    Purpose: Pediatric occupational therapy and physical therapy interventions for children with disabilities are designed to increase function and often focus on developmental skills instead of on the environmental context and promoting self-determination skills. A professional development course was developed to help therapists develop intervention strategies to meet developmental outcomes, while also promoting children\u27s self-determination and creating opportunities for children to authentically participating in daily routines, including leisure and community play. Transformative learning theory was used as a basis to develop this course. The purpose of this article is to report on course activities and the assessment of participants\u27 transformation at the completion of the course. Method: A pre-test/post-test design was used to determine whether participants in this course (n=3) had made transformations in their habits of mind when designing client goals, intervention plans and recommendations for a fictional case study of a child with a disability. Results: Participants wrote pre-test goals that focused on building skill in typical developmental sequences; whereas post-test goals focused on using strengths the child had and changing the environment to encourage authentic participation of the child. In addition post-test goals and interventions focused on determining the child\u27s preference and choice about activities to participate in. Pre-test referrals were for equipment and other programs to address skill development; while post-test referrals focused on play and recreation opportunities. Conclusion: Following completion of a course using transformative learning strategies, participants demonstrated changes in habits of mind upon completion of a post-test case study in which goals, interventions, and referrals were more consistent with themes of the course including self-determination and authentic participation
