    The influence of a serious game's narrative on students' attitudes and learning experiences regarding delirium:an interview study

    Background: Delirium is a neuropsychiatric syndrome that affects patients' attention and awareness as a result of a physical condition. In recent years, persistent gaps in delirium education have led to suboptimal delirium care. Still, little is known about what are the most important aspects of effective delirium education. Serious games are both entertainment and an interactive, safe learning environment where players can experiment and create new knowledge. They have the potential to contribute to improved delirium education. We used a video-based serious games' narrative to explore aspects essential to enhance students' attitudes and learning experiences regarding delirium. Methods: We created a semi-structured interview guide and interviewed seven nursing and nine medical students about their attitudes and learning experiences, after they had played the game. A qualitative descriptive design and inductive content analysis with constant comparison were used. Results: The patient's and nurse's perspective, interactivity to experiment, realistic views on care options, and feedback on care actions were important for enhancing students' attitudes and learning experiences regarding delirium. Students felt these aspects encouraged them to get actively involved in and experiment with the study material, which in turn led to enhanced reflection on delirium care and education. Our findings highlight the importance of a more patient-oriented focus to delirium education to drive attitudinal change. Students' learning experiences were further enhanced through their affective responses provoked by the perspectives, interactivity, realism, and feedback. Conclusions: Students considered the characters' perspectives, interactivity, realism, and feedback important aspects of the game to enhance their attitudes towards delirious patients and enrich their learning experiences. A patient-oriented narrative provides a clinically relevant experience in which reflection plays an important role. The serious game also serves as medium to actively experiment with care solutions to create better understanding of how healthcare professionals can influence a delirious patient's experience.</p

    Unfolding-Based Process Discovery

    This paper presents a novel technique for process discovery. In contrast to the current trend, which only considers an event log for discovering a process model, we assume two additional inputs: an independence relation on the set of logged activities, and a collection of negative traces. After deriving an intermediate net unfolding from them, we perform a controlled folding giving rise to a Petri net which contains both the input log and all independence-equivalent traces arising from it. Remarkably, the derived Petri net cannot execute any trace from the negative collection. The entire chain of transformations is fully automated.     Measurement of fraction unbound paclitaxel in human plasma

    The clinical pharmacokinetic behavior of paclitaxel (Taxol) is distinctly nonlinear, with disproportional increases in systemic exposure with an increase in dose. We have recently shown that Cremophor EL, the formulation vehicle used for i.v. administration of paclitaxel, alters drug distribution as a result of micellar entrapment of paclitaxel, and we speculated that the free drug fraction (fu) is dependent on dose and time-varying concentrations of Cremophor EL in the central plasma compartment. To test this hypothesis, a reproducible equilibrium dialysis method has been developed for the measurement of paclitaxel fu in plasma. Equilibrium dialysis was performed at 37 degrees C in a humidified atmosphere of 5% CO(2) using 2.0-ml polypropylene test tubes. Experiments were carried out with 260-microliter aliquots of plasma containing a tracer amount of [G-(3)H]paclitaxel with high-specific activity against an equal volume of 0.01 M phosphate buffer (pH 7.4). Drug concentrations were measured by both reversed-phase HPLC and liquid scintillation counting. Using this method, fu has been measured in three patients receiving three consecutive 3-weekly courses of paclitaxel at dose levels of 135, 175, and 225 mg/m(2) and found to range between 0.036 and 0.079.     This chapter is devoted to QCD and, more generally, Strong Interaction studies in γγ collisions. For our purposes, LEP2 is a continous energy γγ collider with a reach of up to 100 GeV center of mass energy for some observables. At low energy, the main studies concern resonance production and quasi two-body processes which probe the meson and baryon wavefunctions. At high energy, the partonic structure of the photon plays a dominant role and, as for hadronic processes, several tests of perturbative QCD, using many different observables, are possible. A specific feature of γγ collisions is the variability of the mass of the incoming photons which can be used to tune the non-perturbative component of the photon

    Learning through social spaces: migrant women and lifelong learning in post-colonial London

    This article shows how migrant women engage in learning through social spaces. It argues that such spaces are little recognised, and that there are multiple ways in which migrant women construct and negotiate their informal learning through socialising with other women in different informal modes. Additionally, the article shows how learning is shaped by the socio-political, geographical and multicultural context of living in London, outlining ways in which gendered and racialised identities shape, construct and constrain participation in lifelong learning. The article shows that one way in which migrant women resist (post)colonial constructions of difference is by engaging in informal and non-formal lifelong learning, arguing that the benefits are (at least) two-fold. The women develop skills (including language skills) but also use their informal learning to develop what is referred to in this article as 'relational capital'. The article concludes that informal lifelong learning developed through social spaces can enhance a sense of belonging for migrant women

    Thermal water delivery in the nose: experimental results describing droplet deposition through computational fluid dynamics

    Thermal water therapies have a role in treating various inflammatory disorders dating back to ancient Greece. Several studies have demonstrated beneficial effects of thermal water inhalations for upper respiratory disorders, such as improvement of mucociliary function and reduction of inflammatory cell infiltration. This experimental study describes the numerical investigation and clinical implications of thermal water droplet deposition in the nasal cavity of a single patient. To our knowledge, the numerical flow simulations described are the first investigations specifically designed for thermal water applications. To simulate nasal airflow, a patient-specific 3D computer model was created from a CT scan. The numerical approach is based on the Large Eddy Simulation (LES) technique and builds entirely upon open-source software. Deposition on mucosa was studied for two droplet sizes (5 and 10 ”m diameter), corresponding to common thermal therapy applications (aerosol and vapour inhalation). The simulations consider steady inspiration at two different (low and moderate) breathing intensities. The results of this preliminary study show specific deposition patterns that favour droplet deposition in the middle meatus region to the inferior meatus, with particle size- and breathing intensity-related effects. These global data on particle deposition differ from findings related to the single-phase nasal airflow, which is more evenly distributed between the middle and inferior meatus. The potential clinical consequences of deposition data are discussed. The study furthermore provides evidence for the effectiveness of thermal aerosol and vapour inhalation therapies in reaching important areas of nasal mucosa with considerable clinical significance
