    Forced Medical Treatment of Pregnant Women: Compelling Each to Live as Seems Good to the Rest

    This Article addresses the question of when, if ever, it is ethically and legally permissible to compel a competent, pregnant woman to undergo medical treatment for the sake of her fetus. The Article begins with a discussion of the various clinical situations in which maternal-fetal conflict may arise and the perceptions of clinicians faced with a maternal refusal of treatment. It then explores the ethical questions raised if a physician were to compel a pregnant woman to undergo treatment for the benefit of her fetus. This discussion is followed by an investigation of the legal status of the fetus and the legal interests of the pregnant woman. The Article concludes that pregnant women should not be legally compelled by judges, doctors, or society to accept medical treatment against their wishes

    Infective endocarditis: Clinical features in young and elderly patients

    The elderly constitute an increasing percentage of patients with infective endocarditis. The disease manifestations and outcomes in 53 episodes of endocarditis in patients over the age of 60 were reviewed and compared with 55 episodes of endocarditis in patients less than 40 years of age and 46 episodes of endocarditis in patients aged 40 to 60. The percentage of cases caused by staphylococci and streptococci were roughly equal in all groups. Enterococci, Streptococcus bovis, and coagulase-negative staphylococci were more common in the elderly. In the elderly, invasive vascular procedures were the most common source of infection. Endocarditis acquired nosocomially accounted for 23 percent of all episodes in older patients. The elderly reported fewer symptoms and showed a diminished febrile response. Errors in diagnosis were noted in 68 percent of elderly patients, and a delay in initiating appropriate therapy was more common in this age group. The mortality rate was significantly higher in the elderly (45.3 percent) than in the middle-aged (32.6 percent) and young (9.1 percent). Endocarditis in elderly patients is often nosocomially acquired, is difficult to diagnose, and is associated with a higher mortality than noted in younger patients.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/26976/1/0000543.pd

    Current method of high level disinfection (HLD) of duodenoscopes appears to prevent Carbapenem-resistant Enterococcal (CRE) colonization and transmission: a QI project

    Conclusion: HLD appears to eliminate the risk of colonization and transmission of CRE. Periodic cultures of the elevator tip and the scope tip might be a cost effective way to monitor for potential CRE contamination and risk of transmission. Larger data from other centers might be useful to validate the protocol used in this study

    Acetylation site specificities of lysine deacetylase inhibitors in human cells

J.E.B. is supported by a grant from the Doris Duke Charitable Foundation.Lysine deacetylases inhibitors (KDACIs) are used in basic research, and many are being investigated in clinical trials for treatment of cancer and other diseases. However, their specificities in cells are incompletely characterized. Here we used quantitative mass spectrometry (MS) to obtain acetylation signatures for 19 different KDACIs, covering all 18 human lysine deacetylases. Most KDACIs increased acetylation of a small, specific subset of the acetylome, including sites on histones and other chromatin-associated proteins. Inhibitor treatment combined with genetic deletion showed that the effects of the pan-sirtuin inhibitor nicotinamide are primarily mediated by SIRT1 inhibition. Furthermore, we confirmed that the effects of tubacin and bufexamac on cytoplasmic proteins result from inhibition of HDAC6. Bufexamac also triggered an HDAC6-independent, hypoxia-like response by stabilizing HIF1-α, providing a possible mechanistic explanation of its adverse, pro-inflammatory effects. Our results offer a systems view of KDACI specificities, providing a framework for studying function of acetylation and deacetylases.PostprintPeer reviewe
